Request/Consent for Medication at School

Hart County School Policy for medication: 1. Medication should be brought to school ONLY if it MUST be given during school hours. Bring all

medicine (including over the counter) directly to the office. Students are not allowed to keep medication with them, except for inhaler or EpiPen with signed consent.

2. ALL medication must be in original container. Prescription labels must have student’s name, healthcare provider’s name, and instructions for dose and time. Ask pharmacy to give you an extra container with label for school.

3. Parent/guardian must complete a request/consent for medicine to be given at school.

4. Student is responsible for coming to the office for medication. School staff will assist, however, it is ultimately the student’s responsibility.

5. Parent/guardian is responsible for picking up medication when treatment is complete or at the end of the school year. Any medication not picked up by the last day of school will be destroyed.

6. ALL prescription pain medication must have a written healthcare provider’s request stating this medication is necessary during school hours. It must include specific instructions and precautions.

Petición Parental/Consentimiento de Medicación en la Escuela

Dear Parent:The Compulsory School Attendance Law (O.C.G.A. 20-2-690.1) makes education mandatory for children between the ages of six (6) to sixteen (16). If a child is enrolled and not yet six years old, but remains enrolled in school for 20 days, that child becomes subject to this law. This law defines truancy to be when a student exceeds ten (10) unexcused absences and further requires that local school systems provide parents with a written summary of the possible consequences of violating this law. Should your child reach five unexcused absences you will be sent a notice and asked to sign and return that notice. If the notice is not returned, the school attendance clerk will send another notice, and should that notice not be returned, a certified letter will be mailed.

A parent/guardian has two days following an absence to submit an excuse. After that time, the absence will automatically default to an unexcused absence. More information on our school systempolicy on attendance can be found on the Hart County School System website, http://www.hart.k12.ga.us/. Go to the Board of Education tab, select Board Policies, then on the Policies tab, select section “J” for Students.

When a person who is responsible for ensuring the student’s compliance with mandatory education violates the law they could be found guilty of a misdemeanor in Superior Court. Each day of an unexcused absence is a separate offense, which may carry the following consequences:

A fine of not less than $25.00 and not greater than $100.00 Up to 30 days of imprisonment Community service Any combination of the above

When the student is found guilty of truancy the following consequences may be applied by the juvenile court:

Complaint to Department of Juvenile Justice as a Child In Need of Services (CHINS) Probation Community service Youth Detention Center Any combination of the above

ACCEPTABLE USE POLICY

STUDENTS MAY NOT USE COMPUTERS UNTIL THIS DOCUMENT IS SIGNED AND RETURNED TO SCHOOL.

We are proud of the many technological advantages available to students in all Hart County Charter System schools. Students have access to networked computers, storage space, computer labs, Chromebooks, and iPads. In addition, at times, students are allowed to use personal devices at school. To protect all students, the Hart CountyAcceptable Use Policy must be signed by parents and students before any technology may be used on school grounds. The faculty and staff at each school believe technology offers a unique opportunity for students to explore information that enhances learning. The world is literally at the fingertips of the children through access to the internet. We are aware that there are some resources we would not want our students to investigate; therefore, schools are committed to helping students use the internet responsibly. We have a filter on our server that helps block objectionable material, and teachers are diligent in supervising students as they use computers. Students will be given lessons in the appropriate use of technology, and they will be instructed to use resources thatsupport grade level curriculum goals. Students are responsible for appropriate behavior while on the school’s computer network just as they are in a classroom setting or on the school’s playground. Often, stories and photographs of students are used to show the community the great work of the Hart County Charter System. The Family Education Rights and Privacy Act (FERPA) permits school districts to release “Directory Information” to certain people or institutions, such as the news media, unless the child’s parent or guardian requests that such information not be released.

1NOTICE TO PARENTS/GUARDIANS AND ELIGIBLE STUDENTS OF RIGHTS UNDER

FAMILY EDUCATIONAL RIGHTS AND PRIVACY ACT (FERPA)AND PROTECTION OF PUPIL RIGHTS AMENDMENT (PPRA)

FAMILY EDUCATIONAL RIGHTS AND PRIVACY ACT (FERPA)

Under the Family Educational Rights and Privacy Act (20 U.S.C. 1232g) (the "Act"), youhave a right to:

(1) Inspect and review, within 45 days of a request, the education records of a studentwho is your child, or in the case of a student who is eighteen (18) or older, or your owneducation records. Parents or eligible students should submit to the Superintendent a writtenrequest identifying the record(s) they wish to inspect. The Superintendent will makearrangements for access and provide notice of such arrangements.

(2) Request the amendment of the student's education records to ensure that they are notinaccurate, misleading, or otherwise in violation of the student's privacy or other rights. Torequest the school district to amend a record, parents or eligible students should write the schoolprincipal, specify the part of the record they want changed, and specify why it is inaccurate,misleading, or otherwise in violation of the student’s privacy or other rights. If the districtdecides not to amend the record, it will notify the parents or eligible students of the decision andinform them of their right to a hearing. Additional information regarding the hearing procedurewill be provided with the notification of the right to a hearing.

(3) Consent to disclosures of personally identifiable information contained in thestudent's education records, except to the extent that the Act and the regulations promulgatedpursuant to the Act authorize disclosure without consent. One exception which permitsdisclosure without consent is to school officials with legitimate educational interest. A schoolofficial is a person employed by the district as an administrator, supervisor, instructor, or supportstaff member; a member of the school board; a person with whom the district has contracted toperform a specific task (such as attorney, auditor, or therapist); or a parent or student serving onan official committee (such as a disciplinary or grievance committee.) A school official has alegitimate educational interest if the official needs to review an educational record in order tofulfill his/her professional responsibility. Upon request, the school district forwards educationalrecords without prior consent to another school in which the student seeks or intends to enroll.

(4) The Hart County School District has designated the following information asdirectory information:

(a) Student’s name, address and telephone number;(b) Student’s date and place of birth;(c) Student’s participation in official school clubs and sports;

(d) Weight and height of student if he/she is a member of an athletic team;(e) Dates of attendance at schools within the school district;(f) Honors and awards received during the time enrolled in the district schools;(g) Photograph; and(h) Grade level.

Unless you, as a parent/guardian or eligible student, request otherwise, this informationmay be disclosed to the public upon request. In addition, two federal laws require schoolsystems receiving federal financial assistance to provide military recruiters or institutions ofhigher learning, upon request, with the name, address, and telephone numbers unless parentshave advised the school system that they do not want their student’s information disclosedwithout their prior written consent. You have the right to refuse to allow all or any part of theabove information to be designated as directory information and to refuse to allow it to bedisclosed to the public upon request without your prior written consent. If you wish to exercisethis right, you must notify the principal of the school at which the student is enrolled in writingwithin 10 days after officially enrolling in school or within 10 days of the date of the release ofthis notice.

(5) You are also notified that from time to time students may be photographed,videotaped, or interviewed by the news media at school or some school activity or event; unlessyou, as a parent/guardian object in writing to your student being photographed, videotaped orinterviewed to the principal of the school where your student is enrolled. You must notify theprincipal of your objection by the date specified above. The principal will take reasonable stepsto control the media’s access to students. However, your submission of a written objection doesnot constitute a guarantee that your student will not be interviewed in circumstances which arenot within the knowledge or control of the principal.

(6) File with the United States Department of Education a complaint concerning allegedfailures by the School District to comply with the requirements of the Act or the regulationspromulgated there under. PROTECTION OF PUPIL RIGHTS AMENDMENT (PPRA)

PPRA affords parents certain rights regarding the conduct of surveys, collectionand use of information for marketing purposes, and certain physical exams.

Parents and eligible students (18 or older or emancipated minors) shall be notifiedat the beginning of the school year if the school district has identified the specific orapproximate dates during the school year when any of the activities listed below areexpected to be scheduled. For surveys and activities scheduled after the school yearstarts, parents will be provided reasonable notification of the planned activities andsurveys and be provided an opportunity to opt their child out of such activities andsurveys. The Board of Education has developed and adopted policies, in conjunction withparents, regarding the activities described below. In accordance with Board policies,prior written consent must be obtained from parents before students are required tosubmit to any survey that contains questions about one or more of the areas listed inparagraph (A) and that is funded in whole or in part by the U.S. Department ofEducation. You have the right to inspect any survey or instrument used in the collectionof information under subparagraphs (A) and (B) before the instrument is administered ordistributed to a student and to opt your student out of participation in any activitiesdescribed below in accordance with regulations developed by the Superintendent.

(A) The administration of any survey containing one or more of thefollowing items:

(i) Political affiliations or beliefs of the student or thestudent’s parent;

(ii) Mental or psychological problems of the student or thestudent’s family;

(iii) Sex behavior or attitudes;(iv) Illegal, anti-social, self-incriminating, or demeaning

behavior;(v) Critical appraisals of other individuals with whom

respondents have close family relationships;(vi) Legally recognized privileged or analogous

relationships, such as those of lawyers, physicians, andministers;

(vii) Religious practices, affiliations, or beliefs of the studentor student’s parent; or

(viii) Income (other than that required by law to determineeligibility for participation in a program or for receivingfinancial assistance under such program), without priorwritten consent of the parent or eligible student.

(B) Activities involving the collection, disclosure, or use of personalinformation collected from students for the purpose of marketing or for

selling that information (or otherwise providing that information to othersfor that purpose).

(C) Any nonemergency, invasive physical examination or screening thatis required as a condition of attendance, administered by the school andscheduled by the school in advance, and not necessary to protect theimmediate health and safety of the student, or of other students.

You may, upon request, inspect any instructional material used as part of theeducational curriculum for your student.

The school is required by federal law to give this notice to parents. However, the school does not have scheduled any such activities as are described above. In compliance with the requirements of the Every Student Succeeds Act (ESSA), the Hart County Charter System would like to inform you that you may request information regarding the professional qualifications of your child’s teacher(s) and/or paraprofessional(s). The following information may be requested:

Whether the child’s teacher-o has met State qualification and licensing criteria for the grade level(s) and subject area(s)

in which the teacher provides instruction;o is teaching under emergency or provisional status through which State qualification or

licensing criteria have been waived; ando is teaching in the field of discipline of the certification of the teacher.
