HARM REDUCTIONHARM REDUCTIONHarm Reduction refers to measures aimed at reducing the harm associated with drug use, without requiring a reduction in consumption. A service delivery model and a model for behavior change. The goals of harm reduction are to help service consumers to be happy, healthy, and contented with their lives as possible, as defined by them. This model can be used with any population, any issue or problem.

The following are three The following are three key aspects of harm key aspects of harm

reduction.reduction.A. The user’s decision to use drugs

is accepted as a fact.B. The user is treated with dignity

as a normal human being. C. Harm reduction is neutral

regarding the long-term goals of intervention.

Focusing on drug users, Focusing on drug users, harm reduction has several harm reduction has several principles.principles.

1. HIV takes priority over drug problems.2. Abstinence from drugs should not be the only goal of service to client.3. Drug use is seen on a continuum.4. Drug use is a fact of life.5. Harm Reduction offers “safety nets” to drug users to protect them from

serious harm.6. Harm reduction programs are designed to be user-friendly and inviting

to consumers.7. Consumers are treated with respect, dignity and acceptance, like

“normal” people.8. Drug users are competent to evaluate the choices presented to them

and to make the right choices for themselves.9. Workers are consultants and facilitators in harm reduction.

Stage 1: Pre-Stage 1: Pre-contemplationcontemplationPeople in this stage have no desire to change at all. They may have come to our agency for varying reasons; usually to get some need of theirs met. The goal is engagement, the creation of a relationship or a therapeutic alliance. The first issue to address is safety. Treating consumers with dignity and respect, as normal people is essential, as is an egalitarian relationship.

Stage 2: ContemplationStage 2: Contemplation

In contemplation stage, people start to think about things they have done unconsciously or automatically. Conversations about drug use, sex, relationships, life, ECT begin to make and impact. Since people are ambivalent about most things in life its important to create situation where people can explore all sides of their ambivalence

Stage 3: PreparationStage 3: PreparationIn this stage the person has decided to make some change or another and now seeks a plan or strategy to accomplish that. For some consumer, it will be a small change such as using clean injection equipment or using condoms. For some consumers it may be a big change like giving up drugs. What ever the change is it’s the worker’s job to support the desired change and explore all the possible options.

Stage 4: ActionStage 4: Action

At this stage the person follows the plan created with the worker or enters the intervention, activity or other agency for services. Worker should keep in touch with consumers during this stage. Some consumers will become frightened and not show up for their referral appointment even though they requested it. Follow up is very important as they may feel ashamed and avoid you.

Stage 5: MaintenanceStage 5: MaintenanceSince psychology indicates that for humans, primacy (the first thing we learn) is stronger than recency (the last thing we learn), we revert to old behavior patterns when stressed or in crisis. Thus, behavior change must be reinforced in order to be lasting.
