Incident Sharing

One of our Mechanical maintenance Staff was working for the steam cleaning of GT parts near GTA-02. He was using cable drum. In order to arrange his work he was carrying the cable drum from one place to another. While he was holding the cable in his hand he suffered a mild shock because of the damaged cable.

Immediate Root Cause;

Date & Time; 19-Jan-2012 @ 10:30 hrs Location; RAF A GT HallType of Incident; Electrocution – First Aid

DAMAGED OR HOME MADE TOOLS…A BIG NO.

Do not use: wrenches when jaws are sprung impact tools (chisels and wedges) when heads have mushroomed tools with loose, cracked or splintered handles a screwdriver as a chisel tools with taped handles – they may be hiding cracksTools with broken insulation and plugs

Fingers and hands may be lost when working with moving / rotating / cutting machinery.

HOW CAN YOU GET INJURIED ?

Pay attention to sharp edges ofmaterials.!Cover them to protect yourselfand your colleagues from hands and finger cut wounds and lacerations.!Manual handling and positioningheavy objects and equipment may often result in trapped fingers or hands.

HOW CAN YOU GET INJURIED

If the workplace is poorly lighted, high speed rotating machinery may seem stationary.

ENSURE GOOD WORKPLACE LIGHTING IN THE WORKING AREA.

Repetitive motion or awkward posture of

worker may also lead to hand and finger injury.

ENSURE ERGONOMIC WORKINGPOSTURE. TAKE BREAKS AND VARY

YOUR TASKS.

Hand ProtectionGive your hands adequate protection.Wear gloves appropriate to the job.

They must be comfortable and must fit your hands properly.

Chemical protection gloves.

General purpose“Polka-dot” gloves.

Heat Resistant and fire retardant gloves.

“Riggers” gloves.

Power Tools – PrecautionsElectric Cords

Don’t carry portable tools by the cord

Don’t use electric cords to hoist or lower tools

Don’t yank cord or hose to disconnect it

Keep cords and hoses away from heat, oil, and sharp edges

Check for damaged insulation and missing plugs before each use and report.
