Please email [email protected] to schedule an appointment. PRIMARY DOCUMENT SUBMITTALS: 1. Provide a dated as-built hydrology study with professional’s seal and signature. This document should be able to stand on its own and contain ALL information provided in the most recently county authorized site design hydrology report updated to reflect as-built conditions (i.e. updated calculations, maps – see attached sample maps, models, etc.). 2. Provide a current dated as-built survey with a land surveyor’s or professional engineer’s seal and signature (containing information outlined below – see attached sample survey). Surveys are used for field inspections so should be included as full size (min. 24”x36”) attachments to the report. Smaller copies of the survey are encouraged to be included within the body of the report. 3. Provide the most recent county authorized site design hydrology report. 4. Provide an annotated copy of this checklist. AS-BUILT REPORT: 5. Provide TSS Spreadsheet from most recently authorized site design including completed BMP tracking forms. 6. Provide hydrograph recaps, hydrographs, and hydrograph summaries for 1, 2, 10, 25, & 100 yr. storm events. 7. Provide as-built adjusted calculations for WQ & CP volumes and orifice sizes. 8. Provide stage/storage table for entire pond starting at dry pond’s lowest discharge invert or wet pond’s bottom.

9. Provide a pond summary report with a stage/storage table starting at the required routing elevation with orifices, weirs, culverts, and discharge data used to develop pond routing hydrographs.
10. Provide on-site and off-site delineated basin area maps with impervious areas to the as-built pond.
11. Provide OCS diagram and fill out applicable information in the diagram:
AS-BUILT SURVEY:
12. Show contours at 2-foot elevations and spot elevations (underground storage dimensions & elevations).
13. Show lowest elevation in front of outlet device and opposite end of pond to verify positive drainage.
14. Label top of wall/dam’s lowest elevation to verify freeboard.
15. Label and locate the top of the dam’s minimum width.
16. Indicate all forebay areas for each pond inlets.
17. Provide 100 yr. storm event ponding elevation and show limits of ponding.
18. Location of pond in respect to property lines, road R/W, and other easements.
19. Outlet control details showing pertinent elevations & dimensions for inlets, outlets, piping, & drain protection.
20. Label access easement and 10 ft. BMP drainage easement outside the 100 yr. ponding limits of the pond.
OTHER COMMENTS:
21. Gwinnett County final BMP field inspection is required.
