PROCEDURE1. Accomplish the Application Form which is available at the Guidance/Assistant Principal’s Office, St. Martin de

Porres Building, UST-Legazpi Rawis Campus.2. Pay the Testing Fee of P300.00 at the Cashier’s Office, UST-Legazpi Rawis Campus, Legazpi City. (Business Hours: 7:30 am to 5:00 pm, Mondays to Fridays, NO NOON BREAK)3. Submit to the Guidance Office (located at St. Martin de Porres Building, AUL Rawis Campus) the accomplished

form and the receipt of payment. In return, you shall be given the TEST PERMIT.4. Take the Admission Test on the date specified in the test permit.

DEADLINE OF SUBMISSION OF APPLICATION FORMThe allotted number of examinees per schedule is limited; therefore, the above-mentioned requirements must be submitted immediately to the Guidance Office. First come, first served

TEST COVERAGEThe test will cover the following subject areas: English (with Reading Test for incoming Grade 1), Math and Science,Oral Communication, Social & Math Readiness Skills for Kinder 1 & Kinder 2.

INTERVIEW SCHEDULEInterview shall be done after the test.

RESULTSTest results can be CLAIMED from the Guidance Office on the date of release specified in the test permit. Please be reminded that PHONE INQUIRY on Test Results is strictly DISCOURAGED.

SCHEDULEThe Preschool and Elementary Department of University of Santo Tomas-Legazpi will administer the admission tests to incoming pupils on the following dates:• Grades 5 & 6: January 20, 2018 • Preschool:• Grades 3 & 4: January 27, 2018 February 10, 2018• Grades 1 & 2: February 3, 2018

REQUIREMENTS • Duly accomplished Application Form • Testing Fee of P300.00 • 2 copies of 2”x2” picture with name tag

• Photocopy of Report Cards (with grades until Third Grading Period – Grades 1 to 6 only)
