Tracking your blood sugar results is an important part of managing your diabetes. Checking your blood sugar is one of the best ways to learn if your diabetes is in control. The guide below explains who may be required by Medicare to record and provide documentation of checking their blood sugar as prescribed. Medicare may not cover the cost of the supplies if your mail service pharmacy does not have a current log sheet on file. To use the 5-tests per day or the 10-tests per day Blood Sugar Log, you can either print it out and hand-write your results, or you can save the electronic file and enter your results onto the log sheet from your computer. These log sheets allow you to record results for four weeks. Use the log sheet that reflects how often you test your blood sugar to log your results. OptumRx customers will receive log sheets in their welcome packet as well as with each mailed statement if documentation is required by Medicare.
