1.Planning/Site preparation

Slip, Trips & Fall. Obtain PTW prior to the commencement of the work and 1st copy to be kept at the work site in compliance to KJO work permit procedure. Survey work area and address safe work method, hazards and precautions.

Toolbox talk shall be conducted by the concerned supervisor prior to start of work.

JSA shall be followed and to be explained to all employees during Tool Box Talk prior to start of activities and it must be made available at the work site together with the work permit. All equipment as well as Operators and truck drivers shall have in their possession current 3rd party certificates and SAG license respectively.

All lifting tackles use for lifting shall be inspected and color coded prior to use.

Proper tag lines shall be provided to control the load while being lifted by boom truck.

Boom Truck shall be assisted by flagman all the time of activity specially when maneuvering for proper position. If boom truck is used to load or unload materials, outriggers shall be used fully. Boom truck operator shall follow all the safety precautions pertaining to boom truck operation safety.

Boom truck operator shall have valid SAG driving license.

Remove all debris from work area daily as minimum. All tools and materials shall be properly kept in in a safe are to avoid slip, trips and fall.

2.Grounding Cable Installation. Slip, Trips & Fall.

Sharp Edges.

Manual handling of Materials. Personnel shall be made aware trough tool box talks and trainings about safe movement and good housekeeping in the area to avoid slip, trip & fall, pinch points, safe. Sharp edges shall be supported/covered to prevent from causing injury to person.

Personnel shall be briefed / trained on proper & safe manual material handling techniques.

Wear appropriate and approved PPE at all times.

Area shall be clear from slipping & tripping hazards.

Cable drum support shall be rigid and stable. Supervisor concerned shall closely supervise the job.

Grounding Installation above Ground. Fall from Heights. Fall of tools/materials.

Sharp Edges.

Slip. Trip and fall. All personnel shall be oriented on work at heights procedure prior to work. Work at heights shall be done only when there is safe work platform/s.Personnel shall be required to wear full body harness with 100% tie-off. Hand/power tools as well as materials to be used at heights shall be safely secured. Tools shall be placed on tool box and shall not be thrown from heights. Tool bags shall be used to lower or raise required tools. Power tools shall be plugged to GFCI outlets and double insulated type. Sharp edges shall be supported /covered to prevent from causing injury to person.

Remove all debris from work area daily as minimum. All tools and materials shall be properly kept in a safe area to avoid slip, trip and fall. The surrounding area of activity must be barricaded with safety signage posted.

3.Grounding Connection Termination.(Cad Welding) Fire/Gas. Hot Surface.

Slip, Trip & Fall. Prior to hot works in the area ( Cad Welding of Joints) a hot work permit shall be secured and gas test has been conducted. A currently calibrated gas test detector shall be used for gas testing and records shall be maintained.

A fire watch shall be assigned with his firefighting equipment( fire extinguisher) and ensure that area is properly covered with fire blankets before hot work starts.

Personnel shall be briefed not to touch newly welded joints. Cover shall be provided.

Only competent and experienced electricians shall be allowed to do the job.

Work shall be closely monitored by concerned supervisor.

4.Grounding Electrode, Assembly and Grounding Pit Installation. Hit Against. Pinch Points.

Sharp Edges.

Flying Particles.

Underground Utility Damage. Workers driving the grounding rod into the ground shall wear leather hand and other basic PPEs. Pinch points shall be identified and avoided during the course of work.

Parts of the body exposed for sharp objects shall be protected.

Safety goggles shall be worn by workers doing the job.

A cable/metal detector shall be used to detect underground utilities or trial pit shall be done prior to installation of grounding electrode.

5.Housekeeping Slips, Trip and falls incident Accumulation of debris

Access obstruction Remove all debris from work area daily as minimum. All tools shall be placed inside tool boxes and should be scattered anywhere.

Maintain good housekeeping standards- all walkways/access ways are kept obstruction free/small tools contained.

Keep all materials organized / secured/ free from potential displacement/falls. Tidy up area at each end of the shift.

Close work permit promptly each day before leaving the site.

Required Training.: Safety Orientation; Manual Handling; Hand and Power Tools; Hot Works, Fire Protection and Prevention; Fire Watch Training; JSA and Risk Assessment; Crane and Rigging; Electrical Safety; Lock Out/Tag Out.
