your healthcare provider. No matter the source, chances are good that you will read or hear something about

healthcare reform. Healthcare is constantly changing—transforming.

The staff at Grinnell Regional Medical Center transform healthcare every day for you. They do so by

participating in improving the delivery of care and striving for excellence. Across the nation, standards of

care are changing. We are being challenged more than ever before to provide the best possible care, in the

safest manner, at the most affordable price. A trip through the halls at GRMC demonstrates many examples

of how GRMC staff are transforming healthcare. GRMC continues to lead the way in regional healthcare

with new processes, innovation, and technology. But don’t take our word for it. GRMC has been recognized

twice nationally by The Leapfrog Group for our quality, safety, and value. We work hard to keep our costs

as low as possible and our care exemplary not only because we have had to, but because it is the right thing

to do.

It may seem totally contrary to running a hospital, but these days our focus is on keeping people well and

out of the hospital. Even though wellness has always been part of our mission at GRMC, funding for care

came only through billing for healthcare services. The systemic change underway across the healthcare

spectrum recognizes the shift that not only is it better for people to stay well in the first place, it is less

expensive to pay for preventive care and wellness. GRMC is well-positioned for this change.

Healthcare is always changing. In this year’s annual report, we frequently take a sneak peak back at what

GRMC was like just 10 years ago, in 2003. We think you’ll agree that while many things have changed, one

important fact hasn’t. GRMC is your community hospital. We are here for you, 24 hours a day, seven days a

week, to provide the highest quality healthcare possible—safely and affordably.

Our ServicesThe year 2013 will be remembered as a year of great advancements at GRMC—much like 2003.

During the hospital’s 35th anniversary year in 2003, GRMC launched the Blueprint for Health campaign that made possible the Candace Packard Lambie Intensive Care Unit, the Tomasek Conference Center, the Brooklyn Medical Clinic expansion, and the Mitchell Meditation Room.

Likewise, in 2013, GRMC experienced several unique opportunities that allowed us to make significant strides forward to meeting the ever-changing needs in medical care. First and foremost, GRMC launched the quiet phase of the Moving at the Speed of Life comprehensive campaign. For full details of this campaign, please turn to page 15.

Over a four-year period beginning in 2003, the Blueprint for Health campaign raised $8.3 million toward projects totaling more than $12 million. In its first full year, the Moving at the Speed of Life campaign has raised $2.7 million. TransformationsThanks to the early success of the Moving at the Speed of Life campaign and significant community support, GRMC was able to start several projects on the campaign’s list this past year.

1. In 2003, GRMC was in the midst of the constructing the new Warren H. Bower Surgical Center. Today, the surgery center is home to the most up-to-date robotic surgery equipment in the nation. In January 2013, GRMC negotiated a sweet deal to lease, with the option to purchase, a da Vinci Surgical System, one of the most advanced robotic technologies available. This was made possible thanks to a generous gift from Grinnell Mutual Reinsurance Company and its employees. The system can be used for a range of minimally invasive procedures in bariatric, gynecology, urology, thoracic, and general surgery. Throughout its first year in use, GRMC surgeons completed 193 robotically assisted surgeries at GRMC, including two highly complicated Whipple surgeries to remove or repair bile duct and pancreatic tumors. Surgeons also completed single-site incision surgeries to repair or remove disease in the abdomen. With the new robot came the opportunity for a naming contest. The winning name was Razer, submitted by Jaxon Goodrich, who was a second grade student at Fairview Elementary School in Grinnell.

2. In honor of the incredible work that the GRMC Auxiliary has done for nearly 90 years and for their gift of $150,000, the GRMC Auxiliary Chemotherapy and Infusion Suite opened to serve patients in June 2013. This space is beautifully designed for patients with amenities that support our medical and clinical staff. The new space houses six chemotherapy stations, which doubled the number of patients who may receive treatment. The space includes a refreshment station, lounge for family members who accompany patients, and a much improved nurses station for better observation and care of patients. A highlight is the way the room incorporates botanical, natural light, and décor to create a welcoming environment. In the design phase, GRMC choose to have local Iowa artist, Lee Emma Running, incorporate art into the suite.

3. In late 2013, the medical center started construction on its new urgent care center, named after the Manatt family who made the clinic possible with a significant donation. The clinic opened in February 2014, and seespatients from 7 a.m. to 7 p.m. Monday through Friday and from 9 a.m. to noon on Saturday. This service bridges the gap when physician clinics aren’t open or a provider isn’t available and gives patients an alternative to using the emergency department. This helps to alleviate pressure on the emergency department and its focus on patients who need a higher level of care and treatment.

• After more than 40 years of providing mental health services to county residents, the Poweshiek County Mental Health Center announced it would cease operation. GRMC had not planned to enter the mental health clinic arena, but with the closure of the only large clinic in the county, administrators knew what had to be done. Within one month, GRMC developed a plan and opened Grinnell Regional Mental Health in May 2013.

• The GRMC laboratory celebrated its 30th anniversary in its present location on campus. This critical diagnostic service area has been remodeled and renovated over the years with new equipment added to meet the growing needs of physicians as they care for patients.

• GRMC collaborated with the University of Iowa Stroke Center to launch a mobile stroke detection robotic unit in GRMC’s emergency department for faster diagnosis of strokes. GRMC staff completed the training on the stroke robot and took the unit live at the end of August. This was the second unit that the University of Iowa Health Care had placed in rural hospitals at the time. The ability of GRMC to connect patients to the University of Iowa stroke team enhances the quality of care that we can bring to our residents.

Our People

Awards and RecognitionsState of Iowa Accreditation – Achieving accreditation from the State recognizes that GRMC complies with the highest standards for safety and quality of care.

The Leapfrog Group – GRMC was named by the Leapfrog Group in December 2012 and in December 2013 as one of the nation’s top rural hospitals. GRMC has been identified as a leader in patient safety, quality, and cost efficiency in three of the past four years.

AARP The Magazine – GRMC garnered national recognition in the April/May 2013 issue of AARP The Magazine where GRMC was the only Iowa hospital listed in the map of safest hospitals in the nation.

National Organization of State Office of Rural Health – GRMC earned two awards during the National Organization of State Office of Rural Health

Changes in healthcare over the past 10 years have dramatically changed the personnel needs at Grinnell Regional Medical Center.

In 2003, GRMC employed 506 employees. Electronic medical records, automated protocols for care and discharge instructions, and healthcare costs have shifted staffing needs. In the summer of 2013, in response to these changes, GRMC underwent a restructuring and reduced 41 positions within the medical center. GRMC historically has worked to re-adjust staffing balances through attrition. Yet after several years of poor reimbursement and continued changes in the healthcare, the administrative team and the GRMC board made the difficult decision to reduce the hospital’s workforce to better align the hospital’s staffing levels with the work that needs to be done. At the end of 2013, GRMC employed 390 people.

DAISY Award Winner – Diana Fenner, RN, of Grinnell Regional Home Health was our DAISY Award Winner. This award honors the work nurses do in direct care of patients and families every day.

Aster Award in Healthcare Marketing – GRMC received the prestigious Aster Award in the 2013 Total Advertising campaign category. The silver award went to the 2012 annual fund drive and the “Buy-a-Bed” campaign. The campaign raised $240,177 for 22 new patient beds.

How We Make a DifferenceDefining the needs of a county starts with a comprehensive Community Health Needs Assessment. In 2013, GRMC completed a nine-month process to survey, interview, and host town halls to gather input from county residents and health services providers. This process identified the top 10 health concerns for Poweshiek County. Following the assessment, task force groups were created to make recommendations and plans to address the top concerns.

The primary areas of focus are: mental health, obesity/nutrition needs, preventive care, wellness education, access to care, urgent care, insurance/quality of care, and transportation. Key elements include increasing access and providers at the mental health center, offering preventive care education and literature, and expanding collaboration with community partners to offer services such as health education and transportation.

Advocacy for Fair and Equal Access to HealthcareBack in 2003, the Medicare Prescription Drug Improvement and Modernization Act made its way through Congress. GRMC supporters and leadership took a strong voice to Washington, D.C., to address concerns about inconsistent and possible unjust distribution of Medicare funds.

Skip ahead 10 years. The Affordable Care Act was finally becoming enacted in 2013 after several false starts. The GRMC leadership team and supporters once again took to Washington, D.C., and our legislators to ensure local residents and healthcare were not compromised.

GRMC, with the strong support of the GRMC Auxiliary, continued to address healthcare at the state level and Medicaid reimbursement. Reimbursement remains woefully low, especially for mental health services and preventive services. Two areas that directly impact residents in the GRMC service area.

GRMC advocates for local healthcare and residents with the American Hospital Association and the Iowa Hospital Association. Ed Hatcher, GRMC board chair 1997-1998 and 2011-2012, was nominated by the AHA Board of Trustees to serve on its Committee on Governance. He began serving in January 2014.

Giving Back As a community partner, GRMC supported local charitable groups with about $10,000 in gift certificates, food, and sponsorships last year. In addition, the GRMC medical staff along with GRMC, area businesses, and individ-uals demonstrated their commitment to head-injury prevention by giving 544 new bicycle helmets – one to each third grader in the GRMC service area. Over the previous 17 years, over 4,880 students in the GRMC service area have received a free bike helmet. 2013 was no exception. The GRMC Auxiliary continued to welcome first graders from area school districts into GRMC. The goal is to introduce young people to the facility in a manner that is educational, so they are not scared of hospitals.

GRMC continued to offer several free educational classes to support residents. The Advance Directive program helps residents learn about end-of-life wishes and how to put those wishes in writing so that loved ones know what type of care is wanted in the event of life-threatening conditions. Other classes included the Better Choices, Better Health funded by the Community Transformation Grant. GRMC also provided a free program on the healthcare reform and insurance marketplace to assist residents as they delved into the new program.

Prevention’s Community Transformation

throughout the county. The goals are to

improve walkability and biking access

in rural communities. The program also

works with employers and restaurants to

improve access to healthy choices.

In the Spring of 2013, GRMC hosted a

Taste of Home Cooking school and

welcomed 700 area residents to an

evening of educational health booths,

a little shopping, a meal, and cooking

demonstrations by Taste of Home chef,

Karen Davis. GRMC has hosted a public

event for the past seven years, welcoming

residents to an evening with friends. Each

year, we provide educational booths,

a meal, and a program or speaker to

improve the wellbeing of guests.

In our 2003 annual report, we published a table of information comparing GRMC stats to when the medical center first opened in its current location in1968—a 35-year comparison. We thought it would be interesting to add 2013 to this table to demonstrate at a glance how healthcare has changed and hasn’t changed over the years.

In 2003, the Medicare Prescription Drug Improvement and Modernization Act of 2003 made its way through Congress. It was controversial at the time because there were concerns about the potential inconsistent and unjust distribution of Medicare funds to physicians and hospitals in rural America. It turned out to be beneficial for GRMC in that it avoided significant cuts to revenue and supplied new money for reimbursement.

And in 2013, much of the nation’s attention was focused on the Affordable Care Act, the Health Insurance Marketplace at www. healthcare.gov, and getting people enrolled into health insurance. GRMC had extra staff trained to assist individuals in getting online to sign up for coverage through early 2014. By the time the first deadline for enrolling arrived, more than 7.1 million Americans were enrolled.

In the GRMC service area, an estimated 10 percent of individuals were uninsured at the beginning of 2013. It will be interesting to see what that figure is in the months to come as a result of this sweeping change to healthcare coverage.

The FutureOur 2013 annual report documents our efforts to honor almost 50 years of providing healthcare in the greater Grinnell area, and also to look to the future.

Predicting the future is never an easy task. While we have no definitive view of what lies ahead, we can see some trends that help us in planning for tomorrow.

Grinnell Regional Medical Center takes great pride when our organization is helping to pioneer yet another innovative approach to healthcare. This time, it’s health insurance.

GRMC is the first healthcare provider and self-funded employer to sign on with CoOportunity Health. GRMC is also using CoOportunity’s claims administrator, HealthPartners, as its health benefits processor.

Created through the Affordable Care Act, CoOportunity Health, a nonprofit, member-owned health insurance CO-OP (Consumer Operated and Oriented Plan) began providing coverage in Iowa and Nebraska on January 1, 2014.

For years, GRMC has maintained a self-funded insurance plan. Individual employees and the hospital pay into a pool to create its own insurance plan. Any financial surplus is returned back into the insurance pool for those who are covered to help keep both the employer’s and the employee’s financial contributions down.

In the past five years, GRMC has “bent the cost curve” when it comes to rising insurance rates, meaning that the plan’s costs have outperformed healthcare inflation year after year. While other companies have experienced double-digit increases in premiums, GRMC’s costs and usage of services have remained steady.

GRMC’s administration and board of directors is excited about the partnership with CoOportunity Health and HealthPartners. With the kind of self-funded plan that GRMC has used for its employees for several years, this feels like a good fit for our organization. GRMC and CoOportunity Health shares the same focus on wellness, prevention, and nonprofit healthcare.

Across the nation, individuals, families, and employers have been getting enrolled in insurance plans that fit the mandated standard coverage. GRMC has three individuals specially trained and certified to help people find and enroll in the right program for their own particular interests and needs. As a community hospital, GRMC knows that it is in everyone’s best interests, as well as the hospital, to have healthcare insurance coverage. For GRMC, it means reimbursement for the cost of care.

On a chilly January evening in 2013, the Moving at the Speed of Life comprehensive campaign officially got underway. With a goal of raising $7.5 million, a small army of volunteers has been quietly and effectively working with GRMC staff and physicians to match donors with our needs for the future. Ably led by campaign co-chairs, Dan Agnew, Dr. John Bambara, Dr. Nicholas Kuiper, and Debby Pohlson, volunteers and staff have been reaching out to major businesses and individuals for their support of the following projects:

• Update the emergency department and create an urgent care clinic.• Create a new chemotherapy and infusion suite.• Upgrade to a 64-slice CT scanner and replace other radiology and ultrasound equipment.• Purchase the da Vinci Surgical Robotics System.• Renovate Postels Community Health Park for the new home of the Paul W. Ahrens Fitness Center.• Establish an endowment for Grinnell Regional Mental Health Center.• Purchase new fetal monitoring system for obstetrics and a special microscope for cataract surgery.

We began 2014 with almost $2.5 million raised toward the $7.5 million Moving at the Speed of Life goal.

Because of significant lead gifts from Grinnell Mutual Reinsurance, Manatts Inc., the GRMC Auxiliary, Grinnell College, Grinnell State Bank, and others, GRMC has been able to begin work on some of these projects to benefit our patients as quickly as possible:

• Grinnell College made a $200,000 gift toward its belief in the importance of the emergency department, urgent care, and mental health. The college encourages and challenges others to join them in support of this campaign.

• The GRMC Auxiliary Chemotherapy and Infusion Suite handled 590 patient visits in the first six months it was open, from June 2013 through December 2013. A pledge of $100,000 from the GRMC Auxiliary kicked off the campaign and made it possible for this project to be completed even though it is not yet entirely funded. In addition to the campaign pledge, the auxiliary also pledged $50,000 over five years to the annual fund.

• Grinnell Mutual Reinsurance Company, its employees, and foundation, contributed more than $263,000 toward funding for the da Vinci Surgical Robotic System. This gift made it possible for the medical center to take possession of the minimally invasive equipment in January 2013, making GRMC one of just 17 Iowa hospitals offering this type of surgery. GRMC still needs about $1 million to complete payment on the robot.

• A lead gift from Bruce and Martha Voyles established the Grinnell Regional Mental Health Center endowent at the Greater Poweshiek Community Foundation. This gift was quickly followed by an anonymous gift to bring the total of funds in the endowment to nearly $65,000.

• Manatts Inc. provided a gift of $250,000 toward the Manatt Family Urgent Care. This new walk-in clinic has seen patients during the past holiday season and opened full time on February 13, 2014.

• Grinnell State Bank has made a gift of $250,000 to the campaign in memory of Marion A. Jones. Jones was actively involved in hospital governance, serving as GRMC’s first female chair of the board of directors. She was also a dedicated member of the GRMC Auxiliary, organizing fundraising galas and working through the gift shop. The first floor south patient wing will be named in her honor.

• More than 400 guests attended the 2013 Blue Jean Ball that raised nearly $34,000 toward the Moving at the Speed of Life campaign.

• GRMC employees raised more than $176,000 in their own internal campaign. The results of this internal campaign were excellent with 67 percent of GRMC staff making a gift or pledge, more than any other campaign in the organization’s history.

In our annual list of donors, you will find a significant number of donations to this comprehensive campaign. Next year, we anticipate an even greater response to this campaign challenge.
