Advance Care Planning: Next Step

The greater Rochester area has been a leader in advocating for end-of-life advance care planning for many years. We are fortunate to have community leaders who recognized the need for advance care planning and worked to ensure there was a standard tool where a patient could indicate their personal wishes for interventions. This tool is based on the belief that individuals have a right to make their own medical decisions, including decisions about life-sustaining treatment, to communicate those wishes to their health care providers and to receive comfort care while wishes are being honored.

Early progress in this area resulted in development of the “hot pink” MOLST form that many people have posted on the back of their residence door. MOLST stands for Medical Orders for Life Sustaining Treatment.

In 2016 G.R.A.P.E. requested that our NYS legislators take the MOLST one step further by developing legislation creating a state-wide electronic “eMOLST registry.” To their credit, Assemblyman Joe Morelle and Senator Rich Funke introduced bills to do just that. Their bills would amend the Public Health

Law to establish and maintain an eMOLST statewide registry that mandates use by medical profes-sionals. It will assure prompt ac-cess by those medical professionals no matter where the patient is be-ing treated. The bills were referred to their respective Health Com-mittees in the spring.

Fast forward to 2017! Passage of this legislation is a high priority for the G.R.A.P.E. Advocacy and Emerging Issues Committee and we will pursue further discussions with Assemblyman Morelle and Senator Funke.

Our thanks to Dr. Pat Bomba, VP and Medical Director, Geriatrics, for Excellus BlueCross BlueShield for her leadership and persistence in New York’s creation of the MOLST and her on-going efforts to create the eMOLST registry!

The eMOLST registry, along with several other important issues will be brought to the attention of our legislators at the 15th Annual Legislative Town Hall Breakfast on Friday, February 3rd. You can register through the G.R.A.P.E. website and we look forward to seeing you at Locust Hill!

Sarah Hepler is the Executive Director of Hepler and

DiMarco, Inc. and the co-chair of our Marketing Committee. She was introduced to G.R.A.P.E. by a few friends that were already members of G.R.A.P.E.

Sarah was told she had to join a committee (sounds like Kathleen working her magic) and she joined the committee she felt she could learn the most from. She was briefly on the Annual Dinner Committee before joining the Marketing Committee.

She has been able to develop and grow professionally. Being a one-person business, Sarah has to take on a lot of tasks. Through her involvement in G.R.A.P.E., with professionals from many diverse areas, Sarah has been able to learn a lot.

Outside of G.R.A.P.E., Sarah’s two horses take up the majority of her time. She serves on the Fundraising Committee for Finger Lakes Thoroughbred Adoption Programand on the Development Committee for The Community Place of Greater Rochester and she helps run the Inner City Slickers Rochester, a non-profit that works on building trust and confidence at the Rockow Equestrian Center.
