• Greater Manchester Devolution Agreement settled with Government in November 2014, building on GM Strategy development.

• Powers over areas such as transport, planning and housing – and a new elected mayor.

• Ambition for £22 billion handed to GM.

• MoU Health and Social Care devolution signed February 2015: NHS England plus the 10 GM councils, 12 Clinical Commissioning Groups and NHS and Foundation Trusts

• MoU covers acute care, primary care, community services, mental health services, social care and public health.

• To take control of estimated budget of £6 billion each year from April 2016.

• Commitment in July 2015 budget to align the Spending Review process for health and social care to our Strategic Sustainability Plan

The roots of poor health are found across society and the public service – we need to do more than just respond at the point of crisis. This requires integration of not just health and care, but contributing wider public services focussing on health, wealth and wellbeing

• Improve the health and wellbeing of all Greater Manchester people – of all ages

• Close the health inequalities gap faster within GM, and between GM and the rest of the UK

• Integrate physical health, mental health and social care services across GM

• Build on the Healthier Together programme

• Continue to shift the focus of care closer to homes and communities where possible

• Strengthen the focus on wellbeing, including a greater focus on prevention and public health

• Contribute to growth and connect people to growth, eg helping people get in to and stay in work

• Forge a partnership between the NHS, social care, universities and science and knowledge industries for the benefit of the population

• Make significant progress on closing the financial gap

By 2020, this approach will mean: We have a strong, safe and affordable health and social care system which is fit for the long term future We have a happier, healthier and wealthier population, with, for example (and these are currently estimates but will be firmed up as our work progresses): • 4,000 fewer children and 60,000 fewer adults living with chronic conditions • 6,000 fewer people being told they have cancer • 10% fewer visits to urgent care due to those in good general health and those with chronic

conditions being better supported to self-care • 18,000 at risk children will be better supported by local services to live in stable, caring

homes helping them to progress in education and reducing crime and anti-social behaviour • Nearly 700,000 people with chronic conditions will be supported in the community to

better manage their health in a way that suits them, reducing demand for planned hospital services by 15% and helping these individuals to experience 20% fewer unplanned admissions

• There will be 25,000 people with severe and enduring mental illness five years from now – they will all benefit from proactive, effective, community based care that prevents their mental health deteriorating and reduces their involvement with urgent services by 30%

• A £100m benefit in terms of increased wages and output as people are healthier and supported into work.

• Greater Manchester will remain within the NHS and social care systems and continue to uphold standards in national guidance and statutory duties in NHS Constitution and Mandate – and for delivery of social care and public health services

• Decisions will continue to be made at the most appropriate level to the benefit of people in GM – sometimes locally and sometimes at a GM level

• Organisations will work together to take decisions based on prioritising their people and their place

• From 1 April 2015 ‘all decisions about GM nationally are taken with GM’

Next Steps

Strategic Plan Draft ready by mid December The Strategic Plan will set out how the NHS and local authorities in GM think we may be able to bring the fastest and biggest improvement in health and wellbeing to the people of GM over the next five years. It will have three building blocks: • Ten locality plans – these have been developed in the ten GM areas and are the

culmination of, in many cases, years of work with partners and the public. They are being developed with local health and wellbeing boards and will reflect our commitment to things being done locally where local people think that’s best

• Big ‘transformation initiatives’ and other GM wide plans – these reflect those areas where we think it makes sense to approach things just once across GM to have the biggest positive impact (though this might not mean delivering services in the same way in every area). This includes, for example, our approach to supporting people with mental health problems

• Plans for how hospital and other services direct to the public can work together more efficiently and effectively.

IM&T Enabler Group

• To oversee and review each localities self-assessment against the agreed key standards around individual locality roadmaps.

• To represent the IM&T community from each sector (e.g.: CCG, NHS provider, local authority, NHS national); feeding in knowledge, local/national intelligence on sensitive or contentious issues

• To help deliver, co-ordinate and support IM&T developments in Greater Manchester localities, through local/sector IM&T leads/groups ensuring no locality/group feels disengaged.

• To act as a central point of contact for all GM IM&T opportunities/developments
