There is research that states that a person's name could have an

impact on the person's nature or character. For some, names can

be thought of as a promise of what that person will be and do

what his/her name expresses. But the exciting thing is that

names, to some, are much more than just revelations of character

and promises but also contain presence. Research sums it up this

way—People's names matter — your name is your identity and

you want that identity to be reflective of who you are. Thus, for

some, names are selected to suit our aspirations.

A New Service, A New Start

What does all of this mean for GPS? The strategic mission of GPS is reflective of our

commitment to assist our community in achieving their goals and aspirations. Those we serve

have given us privilege and permission to assist them on their journey towards recovery. GPS

has revised our Mission Statement to be reflective of the actions, habits, behavior and character

traits of our staff necessary to have an impact on the health and wellbeing of our community.

The revision of our mission statement will guide GPS in our day-to-day actions and decisions.

When launching into revising our mission statement, we could have filled pages with the

flowery language of our deepest wishes for our customers, and stakeholders. However, the

mission statement of GPS is not meant to be a love letter—it is a POWER statement. Thus, a

reflection of the actions and decisions of each employee of GPS relative to the organization’s

impact on the community and as we grow nationally.

Who we are? GPS

What we do ? Save Lives

How? By providing Integrated Healthcare.

Why? People with Serious Mental Illness tend to die prematurely.

A strong business name identifies your business, tells customers and stakeholders something

meaningful about your brand and helps to differentiate your business from your

competition. As a business, GPS’ presence in the healthcare arena has expanded exponentially

in the past year; resulting in a transformation of our identity in the community. Expanding our

business isn’t just renaming – it’s also rebranding. Part of a successful rebranding process is

figuring out the authentic brand story we want our audience to associate with our organization

and the services we provide. As our business has outgrown its current name, relative to service

delivery; it became apparent that it was time to rethink our current tagline. As a business, our

tag line, “Behavioral Healthcare,” does not tell our whole story as an organization. Thus, our

tagline has been revised to, “Integrated Healthcare.” This revision of our tagline signifies big

changes in our organization. Our new tagline - Integrated Healthcare - marks the start of a

new relationship, or a new phase in our relationship with the community and those we serve.

We look forward to continued success for those we serve as we collaborate with our partners to

ensure coordination of services necessary for successful recovery.

Strengthening our Community by Increasing Access to Healthcare

For more than 67 years, Grand Prairie Services (GPS) has been dedicated to the

well-being of the community. In 2015, GPS conducted our first Community Needs

Assessment.

The purpose of the needs assessment was to:

Gather information required to bring about change beneficial to the unique behavioral

healthcare needs of the southland community. To identify gaps in services, and propose a system of care that addresses the unmet

needs of the community.

Understand health issues facing underserved populations who experience health

disparities, using existing data and the perspectives of community stakeholders and

families.

Identify strengths, existing programs and activities, that are helping the community

thrive.

Inform our community of benefit efforts by determining where Grand Prairie

Services’ strategic plan or areas in which we have expertise align with community

needs.

The report highlighted the findings of the southland health status, unique needs of those

utilizing behavioral health services, community themes, strengths and forces of change.

Finally, results of the assessment served as a call to action for the transformation of the

current behavioral healthcare systems of Illinois to an integrated system of care which is

accessible and equitable. It is our belief that in order to serve the needs of the southland

community, systems of care must continually assess, prioritize and address community

needs.

Through this work, we have gained a better understanding of the needs and resources of

the communities we serve. The Needs Assessment laid the foundation for our current

Strategic Plan which guides how GPS fulfills our commitment to children, adults and

families, in partnership with community stakeholders. Thus, GPS has transformed to an

integrated healthcare organization and has been certified as an Integrated Health Home

by the State of Illinois.

Care

The Psychosocial Rehabilitation Services (PSR) of Grand Prairie

Services helps reintroduce individuals into the community.

Although participants in the PSR skill building program have

histories of multiple psychiatric hospitalizations, they have proven

capability of utilizing their full potential, living a fulfilling life,

establishing meaningful relationships with their peers and applying

the therapeutic and educational skills learned necessary for

reintegrating into the community independently.

Individuals who take part in PSR programs develop:

Independent living skills, including coping with life

stressors, self-management of behavioral symptoms, self-

management of medication, interpersonal relations and social-occupational skills.

Skills to maintain community adjustments without structured intervention.

Skills to foster generalization of new skills learning to other community settings.

Find and maintain meaningful employment or explore career employment

opportunities.

Individuals are offered diverse groups that focus on the skills needed to maintain community

living. Programs and activities are evidenced-based and proven to address the needs of

program participants. Individuals participate in many community activities that help with

community integration. PSR services are recovery focused and teach individuals the skills

necessary to live, learn and work successfully in the community.

Clinical Training Program

Services

Grand Prairie Services offers a clinical training program to

master level and doctorate level students who are in the process

of completing their advanced degrees. The student clinicians

are trained to provide intake and assessment services,

individual, family and group therapy services and case

management services from a community mental health setting

perspective. Each student has the opportunity to work with an

ethnically and racially diverse population of adults, children

and adolescents. The training approach is generalist in nature

and prepares the student to function in an outpatient setting.

The overall goal of the training program is to facilitate

outpatient mental health recovery to children, adolescents and

adults who are trying to cope with recent emotional and

behavioral challenges.

The therapy practicum begins August 1st and is a year-long training experience. Each student

clinician is on site for a minimum of 20 hours per week. Supervision is provided weekly and

includes one hour of individual supervision and two hours of group supervision. Students are

trained through the utilization of individual supervision, group supervision, mirrored intakes,

formal case presentations, live in-vivo supervision, didactic lectures, audio taping of sessions

and reading material. Training goals are aligned with the Association of Psychology and

Internship Centers (APPIC) standards, the Association of Chicagoland Externship and

Practicum Training (ACEPT), the professional graduate schools, and the mission of Grand

Prairie Services.

Transforming Community Services Utilizing a Team Approach

Acute Community Services

Transforming community services involves partnership and

passion. Providing ‘acute’ services in the community is both a

challenge and an opportunity, with the potential for cost

effective care closer to home. In recent years, the way care is

organized has evolved from a focus on acute episodic care to

more integrated care in the community. Acute hospitals

continue to have an important role, but organizations are

examining how services that may previously have been

offered in hospitals can now be offered closer to home. This

includes providing emergency care in the community but also, perhaps more commonly, offering

outpatient and specialist services in community venues. Transitioning from hospital to community

care involves much more than merely changing the place at which services are provided. Instead,

there needs to be a change in the way that care is conceptualized and organized. While shifting the

location of services is an important step, this needs to be coupled with changes in the roles and

relationships of service users, commissioners, and providers.

Team Based Services

Team based services outcomes include significant improvements in patient quality of life, more

independent living and higher rates of treatment retention. Team based services has been shown to

reduce emergency room visits, use of psychiatric crisis services, and psychiatric hospitalization.

The Assertive Case Management (ACT) model involves an

intensive and comprehensive approach to case management

defined by smaller caseloads, a multi-disciplinary team

approach, shared caseloads, services delivered by the team in a

person’s natural environment, unlimited timeframe, and 24-hour

coverage. Because clients are assigned a team and not just a

single case manager, treatment and services are highly

integrated. The ACT team can integrate everything from mental

health treatment to housing assistance, and work together

seamlessly to tailor treatment and services to the individual

needs, goals, and circumstances of each client.

Community Support Team (CST) is recovery and resiliency

oriented, intensive, community based rehabilitation and outreach

service for adults and youth. Services consists of mental health

rehabilitation interventions and supports necessary to assist the

recipient in achieving and maintaining rehabilitative, resiliency

and recovery goals. Community Support Team is designed to

meet the educational, vocational, residential, mental health, co-

occurring disorders (MH/SA, MH/DD, and MH/Medical), and

financial, social and other treatment support needs of the

recipient. Interventions are provided primarily in natural settings.

Community Support Team assists in the development of optimal developmentally appropriate

community living skills, and in setting and attaining recipient (and family in the case of children)

defined recovery/resiliency goals. It is available 24 hours per day, 7 days per week.

Pastoral Counseling GPS recognizes that spirituality is an important part of recovery for many people. While some

individuals turn to a psychologist, social worker or psychiatrist alone, others want support and the

opportunity to discuss their problems or illness in a spiritual context. Therefore, GPS partners

with the Centre for New Beginnings to provide training for clinicians and support for individuals

and families who requests spirituality be addressed in treatment.

GPS also provides training for clergy. The Pastoral Counseling Training is a twelve-hour course

which includes the following:

Pastoral Counseling History and Foundations of Pastoral Care

Pastoral Counseling: Foundation of Counseling Models

Spirituality and the Grieving Process

Good Grief: Children and the Grieving Process

Integrating Psychology and Spirituality

Pre-Marital and Marital Counseling

A two-hour training is also available for those who require just an overview of Pastoral

Counseling:

GPS’ Summer Youth Employment Program offers

temporary summer jobs to students of all ages.

Through the Summer Youth Employment

Program, we strive to provide young people with

the opportunity to:

commitment necessary to succeed in today’s

world of work.

We are pleased to announce Grand Prairie Services partnership and ownership

stake in the Illinois Health Practice Alliance (IHPA). IHPA is an Independent

Practice Association comprised of providers, Community Behavioral Health

Association and a major insurance company, joined together to achieve cost

efficiencies, management services and other economies of scale. IPA membership

gives single providers the ability to participate in programs that would otherwise

not be available to them.

Grand Prairies services benefits from its ownership stake by taking on a risk

sharing program that will share the costs of providing patient care and earning

additional compensation for providing quality care, beyond the standard Medicaid

“Fee for Service” reimbursement.

Joining an IPA will allow GPS to focus on providing care. Having the benefit of

the IPA providing technology services, marketing support, and practice

management guidance are tangible benefits that improve efficiency to better

support the communities we serve. Grand Prairie Services is at the forefront of

providing excellent patient care in a fast changing and ever improving healthcare

market. Our partnerships and contracts in multiple payer markets will provide

growth opportunities and the ability to hire and retain staffing needed to meet the

growing patient population. Secondary agency benefit should also allow for

individual promotion potential and the career development opportunities that are

the essential requirements of a healthy and stable employer.

Expanding Housing Options in our Community

One of the biggest issues some people face is the availability of housing. Most people on the road to

recovery can and do live independently in apartments or in their own homes. For others, the cascading

effects of life challenges might leave them in a precarious housing situation, or even cause them to lose

their homes. However, there is cause for hope. GPS believes that having a safe and secure place to live

is an important part of recovery, along with access to services that enable individuals and families

requiring integrated healthcare to live as independently as possible. As a result, GPS has further

expanded housing options to include more independent apartment based housing which provides

individuals with services, support and affordability needed to achieve goals towards recovery.

Maple Leaf Supported Housing (MLSH) is located in University Park, Illinois. MLSH integrates

people into the community. This housing option also provides residents choice and autonomy. The

degree of support residents receive while living in supportive housing includes frequent visits by a

housing counselor, to independent living with minimal support. While residing in supported housing,

residents have access to integrated health services, receive help with transportation and supported

education.

What’s in a Name?

Names have significance, telling us something about the person's attributes or circumstances.

Furthermore, identifies the person, but also reflects the nature and character of the person.

Historically, people were given new names, usually after they or their situations changed. When

choosing a name for their child, parents from ancient cultures might consider the circumstances of the

child's birth, their gratitude for the child, their hopes for the child's future.

Traditionally, GPS has used the names of the street that our residential houses were located as the

names of our homes. However, after reviewing the names of our residential sites, we realized that the

names of the homes did not tell our story as an organization, that of our residents nor reflect our

business. This year we are pleased to announce the naming of two of our residential homes.

Bradley House located in Bradley, Illinois, has been officially named. Joy House. Joy is defined as

happiness over an unanticipated presence of good and reflective of restoration, generally experienced

in corporate life, and successful undertakings. This name was selected in an effort to elevate the

happiness of residents as they gain a new start, in their quest towards a new life.

Bensley House Apartments located in Burnham, Illinois, has been officially named Ruth Apartments.

The name Ruth means compassionate friend, companion, and vision of beauty. What a wonderful

way to welcome those transitioning to independence in the community and reflective of the services

provided by the staff of GPS as they assist residents to achieve greater independence.

Patient Satisfaction Survey

The Mental Health Corporations of America (MHCA) Customer Survey analysis is

designed to be part of the GPS Quality Improvement system. The intent of the survey is to

provide a picture of GPS’ patients’ satisfaction with our organization and the services

provided. The information is used to identify GPS’ strengths and opportunities for

improvement. The information is obtained form the MHCA Customer Survey for patients

and GPS is compared nationally, with other corporations.

Grand Prairie Services conducted a Satisfaction Survey of the patients seen at the Health Center in

order to obtain baseline data to assist the Health Center in providing the highest quality of care possible.

Overall, seventy-nine percent (79%) of individuals reported overall quality of services was good, very

good or excellent. Ninety-two percent (92%) of individuals responded that their problems were a great

deal better or somewhat better since starting treatment.

Grand Prairie Services and its highly-qualified trained professionals impacted the 32

municipalities of the south suburban area by doing the following:

Residential Services provided 6,892 hours of care.

Specialized Recovery Support Services provided 5,823 hours of service.

Adult Crisis Services provided 2,570 hours of service.

Child & Adolescent Crisis Services provided 10,257 hours of service.

Grand Prairie Services trained professionals provided support services to youth and

their families involved in crisis situations and adults facing life altering events

through our Adult Outpatient Services. Grand Prairie Services also demonstrated

its unique ability to address the needs of families and individuals by providing

Individualized Psychiatric Services. Lastly, the needs of our south suburban area

were also addressed through Grand Prairie’s Specialized Recovery Support

Services, Residential Services, and our Developmental Training Programs. As

evidenced by the above numbers and the long hours of dedicated staff, Grand

Prairie Services’ consumers receive care needed and are treated with dignity and

respect.

Grand Prairie Services’ is moving toward its goal of diversifying funding and

decreasing dependence on state grant funds. In 2018, grant-based income was

reduced by 15% and fee-for-service income was increased by 20%.

The Award for Excellence was established in 1987 by the Board of Directors

to honor the memory of John and Gloria McAfee. The purpose of the award

is to recognize GPS staff who provide outstanding clinical service. All full-

time clinical and supervisory staff are eligible for nomination.

The 2017 award recipient was Carolyn Smith.

Award for Excellence

Volunteer

Quality Award .The Quality Management Award is presented to a GPS program that displays consistent

application of the highest standards within the realm of behavioral health services.

Programs awarded the Quality Management Award have met and exceeded the standards of

the State of Illinois and the CARF International

accrediting organization, and exhibit continuous

quality practice improvement.

The 2017 award recipient was Developmental

Training.

The Spirit of Recovery Award recognizes the outstanding achievements

of people and programs that provide the necessary support to help

improve the lives of individuals with serious mental illnesses. The

award recognizes a team that has contributed positively to fostering

relationships through the development and implementation of services

and activities which promote understanding and/or awareness of mental

illness and associated stigma.

The 2017 award recipient was First Episode Psychosis Services.

Spirit of Recovery

Excellence in Leadership

The Excellence in Leadership Award recognizes individuals in

leadership roles who go the extra mile. Individuals who do more than

required, and those whose efforts often go unnoticed.

The 2017 award recipient was Griscillia Moore.

The Volunteer Award is an award to recognize an individual

whose generous commitment of time and energy has had a

profound impact on GPS and the individuals we serve.

The 2017 award recipient was presented in honor of the late Ken

Dedrick to Kim Miggenburg.

communities by supporting the mission of Grand

Prairie Services.

By far the most significant financial

contributions from Repeat Boutique have been to

Residential Services operated by GPS. Over the

past years, the dedicated volunteers worked

tirelessly to provide the funding to purchase

vehicles which are now used to meet the

transportation needs of our residents. Funding

has been utilized to renovate several homes, to

purchase new appliances and furniture, and to

provide gifts during the holidays for individuals

living in our residential homes.

New volunteers are always welcome. If you are

interested in making a difference in the lives of

people in need through volunteerism or a
