Gift Card Order Forms are available from your Vantiv representative. To customize a Template Design, we will need your artwork, including company name and/or logo. A proof will be submitted for your approval via email, unless otherwise requested. Production will not begin for either Template or Custom Cards until your written approval has been granted. Finished cards and carriers will be mailed directly to your company.
