The information in this leaflet is to offer you advice, support and guidance when
you are accessing any LTHT service or are supporting an individual with a
learning disability to access an LTHT service
LTHT and community services should develop positive relationships and share
information so that any individual is supported appropriately and has a positive
experience.
Good Communication and Planning are key to successfully supporting a patient.
On the opposite page is the LTHT Learning Disability Charter.
These are the things that people with a learning disability feel it is important
for other people supporting them to think about and consider.

Prompt Sheet
1. Please email or telephone Sadie Dunne Lead Nurse learning
Disabilities/Autism to advise her that someone you support is going to be
in hospital, is currently in hospital or has an OPA. Sadie can then check to
see if the appropriate flag is attached to this persons name on LTHT
electronic system (this flag helps hospital staff to identify that the
patient may need some reasonable adjustments).
2. Does this individual have a completed hospital passport they can bring
with them? (see page 11)
3. There is lots of information on the LTHT internet LD/Autism page, you
can download a blank hospital passport. (see page 24)
4. Would a planned visit before the OPA or inpatient stay help to reduce any
anxiety? (see page 8)
5. Contact the outpatient department or ward as soon as possible before or
as soon as you arrive to discuss any reasonable adjustments.
6. Bring any medication with you - but only a few days worth - don`t bring
whole boxes. (see page 17)
7. Look at the Discharge planner leaflet as soon as you arrive on the ward
and start to fill it in and share with the named nurse. (see page 16)
8. Check the end of bed board magnets to see if the correct ones are being
used, speak to the named nurse. (see page 17)
9. Talk to the named nurse about any carer support, will a carer be staying
overnight? Staying for long periods? What support will the carer be
providing? (see page 13,14,15)
10. When you visit or support anyone speak to the named nurse for an update
when you arrive.
11. If you are a paid carer or support worker please ask to read the nursing
notes when you arrive on the ward and write in the nursing notes at the
end of your visit or support shift so that there is clear understanding of
the role of the home care staff and the hospital staff. Sign and date this
with your job title. If a member of hospital staff refuses to let you do
this please ask to speak to the nurse in charge or contact Sadie Dunne.

LTHT LEARNING DISABILITY SERVICE

Sadie is not an acute liaison nurse. There is no learning disability or Autism
team.
Therefore, one of the aims is to encourage LTHT and the community to speak to
each other, to encourage positive relationships to develop and lead to positive
hospital experiences for people with a learning disability.
If you would like more information about learning disabilities or Autism please look on
the LTHT Learning Disability/Autism internet site or contact Sadie.
There is a Learning Disability/Autism Steering Group meeting held every 3 months.

`On NHS Change Day I made a personal pledge as Chief Executive to “understand and respond to the needs of our patients with learning disabilities.” At LTHT we are committed to ensuring that every patient experiences the highest standard of care possible, but for many people living with learning disabilities the stresses of being in hospital can be compounded due to a lack of understanding and poor communication; it is for these reasons that I made my pledge.`
The 'Get Me Better Champions' have made an enormous contribution to improving patient experience for people with learning disabilities.

The Get Me Better Team Office

The Get Me Better Team Office is open Monday - Friday 9 am - 3.30 pm.
The Get Me Better Champions support the teams based in THQ with laminating, photocopying, delivering and picking up information on all 5 LTHT sites, picking post up.
The Get Me Better Champions are paid £20 expenses per week so that no benefits are affected.
The longer term aim is that LTHT will be able to offer a part time or full time substantive post.
Some of the other Get Me Better Champions support other wards and departments.

Information for a planned admission or Out Patient visit
Think about any reasonable adjustments that will need to be discussed and
contact the ward or department before the admission or OPD visit to discuss
these. Make a note of who you spoke to and what was discussed. (this could be
documented in the hospital passport)
Pre Visits
The wards and departments can facilitate visits before a planned admission or
OPA if this will help with any anxiety.
Contact Sadie Dunne if you would like to discuss this further or speak to the
ward or department manager.
I would just like to say thank you on behalf of everyone you showed around the hospital on Wednesday, I feel the visit was very beneficial and a positive experience for people, it was interesting hearing them tell their friends about it when they got back to Strawberry Lane.

Community Support Services, Bramley Fulfilling Lives
Some schools in Leeds have participated in the Get Me Better Workshops, a
series of workshops facilitated by members of staff from the Children`s
services. The aim is to let young people become familiar with hospital, which will
help with desensitisation and reduce any anxieties.

PLEASE USE THE HOSPITAL PASSPORT
IF POSSIBLE LET THE WARD OR DEPARTMENT HAVE A COMPLETED ONE BEFORE THE ADMISSION OR OPA AND TAKE ONE WITH THE INDIVIDUAL YOU ARE SUPPORTING, ASK THE HOSPITAL STAFF TO LOOK AT IT.
Ask if you can e mail a copy to the ward sister/ward clerk so they can print one off to put in the notes.
Take 2 copies of the passport,
leave one in the end of bed folder (on a ward) or give a copy to the out patient clinic nurse and keep one with you.

Other useful documents
LTHT have developed an end of life document that documents what the individual wants to happen. This document is offered to people with a learning disability to complete and keep. A patient may share this document with hospital staff during their hospital admission. Please let hospital. staff know if there is a completed document.
LTHT work in partnership with LYPFT to facilitate 2 workshops for people with a learning disability to talk about the end of my life document and other issues regarding dying. There workshops have had very positive feedback. Contact The Lead Nurse Learning Disabilities if you would like a document to complete or more information about the workshops

Working in partnership with the hospital
If you are a paid carer or support worker please wear your name badge and/or identify badge.
Ask hospital staff that you speak to for their name and make a note of this and the discussion.
If you have any concern or issue please speak to the ward or department manager/nurse in charge immediately so that this can be discussed and addressed asap.
Please try to resolve any issue, concern as soon as it arises and bring it to the attention of hospital staff to discuss and resolve.
If you would like further support ask for the name and contact details of the matron.
When you visit or support anyone speak to the named nurse for an update when you arrive.
If you are a paid carer or support worker read the notes at the beginning of your visit or support shift and.
PLEASE WRITE IN THE NURSING NOTES at the end of your visit or support shift so that there is clear understanding of the role of the home care staff and the hospital staff. Sign and date this with your job title.
If a member of hospital staff refuses to let you do this please ask to speak to the nurse in charge or contact Sadie Dunne.
Have a conversation with the ward to clarify if the individual requires 1:1 support and if the ward are providing this support. Please have this discussion before the admission if possible or as soon as possible on admission.
Please provide evidence that the individual already recieves 1;1 support and why this will need to continue or why the individual requires 1:1 support whilst in hospital.
If the ward are not able to provide this support but the home is able to, have a discussion about the cost of this support and if the home has to backfill the hours of the member of staff providing the 1:1 support. The Matron will be the most appropriate person to have this discussion with.
If you are going to or are supporting an individual on a ward please have a discussion with the ward staff to clarify the role and when support will be provided by the ward staff for you to take a break
Car Parking Permits Critical Care wards have a supply of car parking permits for families who are visiting dying patients - they last for 7 days and can be renewed as required. Please ask the ward staff for more information.

There are fold out beds kept in stores at SJUH and LGI if anyone is supporting overnight.
Please ask the ward staff for more information. The porters will be able to provide these beds.
It can help both hospital and home staff if everyone knows who is supporting the individual and when, put this in to a time table and remember to let the ward know if this changes as soon as possible

Discharge
Talk to the ward staff (as appropriate) about discharge as soon as possible after admission.
Consider any needs that may have changed during admission and what measures are required to be put in place for discharge
- The physical environment: stairs, aids and adaptations, - - Care Support: is more care required? More calls each day, a different home
environment - nursing home?
The My Home Planner leaflet should be in the
end of bed folder, please have a look at it.
Make a note of any issues/concerns for discharge
And speak to the ward staff about these.
Discharge Lounge
There is a Discharge Lounge at St James`s
Hospital, this is located on the ground floor of Lincoln Wing at St James's and is open Monday - Friday 8 am - 5 pm
There is one qualified nurse supporting the discharge Lounge.
Can this patient access the Discharge lounge whilst waiting for take home medication or transport?
Being Discharged
Discuss with the ward staff -
Who does the patient live with?
Have the appropriate carers /family been informed that the patient is being discharge?
Does the patient have all the information they require for discharge?
Does the patient have the appropriate transport to get home?
Complete a Discharge Plan document (see appendix 2)for ward staff to share.

Any individual with a learning disability or autism or a learning difficulty can
have a flag attached to their name on PAS to identify that they have a learning
disability or autism or a learning difficulty.
When LTHT staff click on a patients name with this flag, this box appears:

protected mealtimes
What are Protected Mealtimes?
A period of time when all non-urgent treatments, procedures and care stop so patients can eat and enjoy their meals without interruptions.
What is our aim?
For every ward to protect at least one mealtime (lunch or evening). For all available
ward staff and volunteers to help with food distribution and the encouragement and assistance of patients to eat their meals. For other hospital services to respect the
mealtime period and avoid non-urgent interruptions at this time.
What are the benefits?
Patients
Good nutrition and hydration play a key part in a patient’s recovery from operations or illness. Protected Mealtimes enable patients to eat their food in a quiet environment and make
mealtimes more pleasurable. Staff
It gives ward staff dedicated, uninterrupted time to help and encourage their patients to eat. It enables nursing staff to monitor food and fluid intake and review the need for red trays.
How can visitors help?
If you would like to assist your relative/friend at meal times you are very welcome. Please discuss this with the ward manager. Otherwise, please avoid visiting or phoning the ward
during mealtimes.

When the individual you are supporting is being discharged or leaving an opa, please complete the fft (friends and family test),
Ask a member of staff for the form. This gives the hospital feedback and helps us to improve our service.
See appendix 1 for a guide for the FFT
The FFT form is kept at reception or at the nurses station. Each ward has its own FFT card with a unique scan code. Below is an example of what one looks like.
There is a post box on each ward and department, ask a member of staff if you cannot see or find one.

There is a Changing Places in Clarendon Wing Entrance, LGI. If you do not have your own key ask the reception staff in Clarendon wing entrance. If you require a Changing Place on any other sites please ask the reception staff or nurse in charge where you can access a bathroom with the appropriate facilities.
