Introduction

The NHS in Scotland and Social Care Services are developing in partnership at significant pace. Delivering a high quality and deserved service to the Scottish population that requires access to Health and Social Care in the community. To achieve a high quality service, professionals must work in greater partnership and within models of integration, to ensure the services are able to be deployed in the best way, to ensure greatest effectiveness.

The professional workforce in Health and Social Care Services are working collaboratively with partners from other statutory agencies and voluntary and private organisations, to provide better outcomes for children and families, vulnerable adults, older people and individuals requiring support as they progress through the criminal justice system.

Health Boards and Local Authorities recognise that to improve the joint working performance of the professionals within the services, there is a requirement to develop structures to support joint working including joint funding of services, joint management and joint training. This tool intends to focus on the joint training arrangements and to augment the joint training activities, by developing opportunities for a shared learning within integrated services.This process would begin within the induction period for all new staff to an organisation including support staff and professionally qualified staff.

This tool recognises that induction is the foundation to the development of individuals and the organisation. All organisations have induction processes that support new employees. This tool maps a process that allows induction of staff to integrated services and should be conducted to allow this experience to be carried out together with interagency colleagues, enabling shared learning and experience, and supporting joint working in the near future.

It was agreed that NHS Education for Scotland (NES) and Scottish Social Services Council (SSSC) would develop an induction tool to support staff within the integrated services. The key outcome of this simple tool is to prepare the employee to provide the best quality of service within an integrated service. The tool would support this by allowing the employee to work through a number of activities and exercises assisting their adjustment and early development in their new place of employment.

This tool is not expected to be prescriptive. However, it is hoped it can be used as a template and reference point for new employees. Additionally, the format is such that the activities are allocated time to complete. This is not intended to inhibit learning, but more to support the planning of induction with dedicated time requirement.

1. Background

1. 1NHS Education for Scotland and the Scottish Social Services Council commissioned OLM consulting to carry out a scoping exercise entitled the Nature and Extent of Joint Training in Scotland. This report was submitted to the Joint Future Human Resources group and it highlighted a wealth of Joint Training programmes across all care groups/service users. However, there was evidence that this could be further developed. One particular area for potential development was in induction programmes for Integrated Services.

1. 2 Major Policy DriversRecent national policy has made significant strides towards improving and sustaining more integrated and partnership working arrangements between health and social care services. The Joint Future agenda crystallised the requirement for health and social care services to develop models and approaches of partnership working which would improve services to care groups/service users.

The momentum and vision of closer partnership working is captured in the most recent policies that effect health and social care in Delivering for Health 2005 and Changing Lives: The report of the 21st Century Social Work Review.

Both documents recognise the importance of building and developing a workforce that works in partnership to deliver quality and effective services to the population of Scotland.

1.3 Target audience for Induction ToolThis tool is aimed to be useful to a wide range of staff:• support staff and professionals working within integrated services for older people, mental health, children, adults with disability, adults with learning disability and integrated criminal justice services;• operational managers of integrated services;• Health and Social Care service managers;• Health and Social Care training providers and planners;• Integrated Service Users;• other Statutory, Voluntary and Private agencies working in partnership with integrated services.

This list is not exhaustive as it is hoped that this tool will increase understanding between all potential partners working with integrated services including other statutory, voluntary and private agencies.

1.4 Guidance on the use of the toolThe tool has been developed specifically to be flexible and adaptive for potentially all staff working within or closely with integrated services. The tool is an individual resource, with each staff member completing the activities within an agreed timescale.

Use of the tool will support the line manager to guide new staff towards key learning opportunities during this early stage of employment. Additionally, the tool will support managers in the planning and preparations of the personal development plans to support the continued professional development. Staff will receive an introduction to the tool at the commencement of employment. This will be reviewed by the staff member and line manager after approximately 3 months, with a final review at approximately 6 months.

There may be elements of the exercises that can be completed very early on and others that will require further investigation and planning. As people all learn at a different pace the time allocated to each exercise is only an estimate and this factor would be agreed and discussed with the staff member and line manager. Currently, the estimated dedicated time is 16 hours in total.

1.5 Professional values and principlesThere are professional values and principles that underpin the identity of each professional group and it is important that there is mutual understanding and respect for these values.

There is a wide variety of professionals and support staff working within an integrated service.

Many of these roles can be allocated to one on the following categories:

• Nursing;• Social Work;• Allied Health Professionals;• Medical staff.

Staff working in these roles require to be registered with a regulatory body to ensure public confidence and safety. Each of these professional groups must fulfil their professional codes of practice and meet requirements for ongoing professional development. Staff that need to be registered with a regulatory body will be familiar with the standards of practice required. Appendix 1 provides links to the different professional regulatory and professional bodies.

2. Developing an Induction Tool for Integrated Services2.1 IntroductionThis tool recognises that within partnership organisations there are specific induction requirements that may not be relevant to a joint induction programme. Furthermore, the induction will vary in terms of meeting requirements of the partnership organisations. This tool is designed to compliment the local arrangements. The focus will be on understanding, shared learning and reflecting on how the impact of the joint future agenda and other policies have on the day to day delivery of a high quality and effective service.

2.2 Current delivery of Joint TrainingWithin current integrated services there will undoubtedly be a wide variety of training available. This may be delivered in-house or from a commissioned training provider. Furthermore, the types of training can be delivered to a single discipline, multidisciplinary or in an interagency format dependant on the subject.

As the focus for this document is on the development of the benefits and principles of joint training across professional groups and organisations, suggested potential subjects and examples of this training may include:

• Single Shared Assessment Training;• team development and building sessions;• Carers Awareness Training;• Care Management Training;• Child Protection Training;• Domestic abuse awareness;• Adults with Incapacity (Scotland) Act 2000 Training;• The Mental Health (Care and Treatment) (Scotland) Act 2003 training;• Risk Assessment Training;• alcohol and drug addiction awareness;• information sharing training• management of long term conditions/people with complex needs.

Within the exercise section in Chapter 6 information can be gathered to indicate the training that is available and explore with line managers timescale and prioritisation of training programmes that require to be accessed.

2.3 Opportunities to develop togetherThe opportunities to develop together can be described as falling within a number of themes including:

• practitioners one to one sharing of knowledge and experience;• organisational in-house type training programmes;• topic specific training provided by commissioned organisations.

The diversity and blending of the differing professional staff within integrated services generates a host of potential training, learning and sharing experiences opportunities, which can be available to individuals and teams.

Many learning opportunities are formally arranged and targeted for a specific teams/groups. Training programmes and initiatives are developed to actively encourage the multi-professional approach within the workplace. This is evident in various Higher Education Institutes developing under and postgraduate programmes for health and social care professions to support integrated way of working.

The use of formal training programmes is not the only method of learning and sharing experience together. The activity section within this tool will allow professionals to identify the best ways for them to share experience and learning. Examples include the use of practitioner forums, regular information sharing sessions/tutorials, multidisciplinary dedicated learning sessions and day to day sharing of information and experience between professionals.

2.4 Models of inductionThis tool should compliment existing and evolving local induction programmes and frameworks which support induction into integrated services.

This section gives some examples of models of induction that are currently being used and reflect on the key values of joint induction and commencing the process of increasing the understanding of each others’ roles, defining areas of shared values and dedicating time and commitment to learning together.

The traditional approaches to induction remain extremely important. These models will often include topics which are mandatory for employees to attend. These may include:

• Health and Safety Policy and Practice for example Fire Safety, Move and Handling, Lone Working and Infection Control.• Corporate Policy for example Equality and Diversity, Freedom of Information, Communication Standards and Information technology.

There may also be specific disciplinary/professional programmes that you may be encouraged to attend. Furthermore, there are initiatives available for newly qualified health professionals by supporting learning with a range of web based activities; this is called Flying Start. 3. Themes for learning together3.1 Key OpportunitiesThis session is dedicated to searching out the key opportunities where learning together can have the greatest value. This Chapter will highlight core themes which will be relevant to all staff working within integrated services.

The exercises and activities within Chapter 6 are designed to reflect the following themes to allow staff to develop understanding and learning as part of their induction.

3.2 CommunicationUnderstanding communication methods, systems, processes and pathways is an extremely important part of the role for all staff to deliver effective quality care to service users. Within Chapter 6, exercises 3, 4, 5 and 6 will offer an opportunity for staff to explore the communication processes that they will be using. Within any multi agency working there are a number of different professionals working together, with staff coming together from different professional backgrounds and different working environments there may be incidents of different usage of terms, words or language.

3.3 Understanding and defining roles.In the early part of induction, staff will be orientated to their new working environment and developing an understanding of the role they will have within the integrated service. It will be important for staff to have the opportunity to relate their role to the others within the service. Exercise 5 within Chapter 6 will offer staff dedicated time to strengthen their understanding.

3.4 Working in partnership and across multi-agency boundariesThe climate of integrated working across multi-agency boundaries has been growing in response to national policy. The relationships and referral pathways that staff have access to is extensive. Exercises 1, 2, 5 and 6 within Chapter 6 will give new staff the opportunity to explore this at a very early stage of their employment.

3.5 Sharing experiencesOpportunities to share experience from peers and colleagues can be an important support to all staff. Exercise 7 within Chapter 6 gives this area some focus and allow staff during their induction process time to identify where they can share experience.

3.6 Developing awareness of all services and resourcesThe services and resources that integrated services will access are varied. For new staff it is important that there is understanding of what is available. The majority of teams will have access to a number of different directories and networks. Exercise

6 within Chapter 6 will allow new staff to orientate themselves to services and resources within that area for the care group they are working with.

3.7 Defining the capacity of the serviceIntegrated services have clear operational procedures. These will include areas such as referral and discharge protocols. Exercises 2 and 3 within Chapter 6 will allow you to consider the capacity of the service.

3.8 Supporting service improvementAll services are striving to develop and improve the standards and outcomes for service users. There will be many innovative ways in which services are being delivered to achieve improvement. Exercises 1 and 7 in Chapter 6 will allow staff to be aware of how outcomes and service improvement is being developed and assist them in identifying their role in this area.

3.9 Developing opportunities to share learning/ joint trainingThe development of joint training opportunities available to staff has grown significantly. The different partnerships across the country have developed varied and diverse opportunities for learning together. Exercises 4 and 7 within Chapter 6 will allow staff to identify the opportunities and methods available to them for joint learning.

3.10 Implementing safe and effective practiceStaff commencing employment will be eager to access up to date information, training and support to allow them to practice safely and effectively. Exercises 3, 4, 7 and 8 within Chapter 6 will allow staff the opportunity to identify the supportive methods which will assist them to deliver a safe and effective practice.

3.11 Understanding the systems of supervision, mentoring and leadershipSupervision, mentoring and leadership are of key importance to new members of staff within an integrated service. Exercise 8 within Chapter 6 will give staff an opportunity to clarify support systems available to them.

4. Self Assessment

This section is designed to allow individual staff members to review their current level of knowledge.This process should assist them in identifying learning/ knowledge priorities for the first few months.

These exercises are rated at 1, 2 and 3 with:

1 being for staff who require to update their knowledge at the first opportunity;2 being for staff who have a limited amount of knowledge and require to update;3 being for staff who are confident in their knowledge in this area and will keep themselves up to date with any developments.

Staff should complete each question by ticking their level of knowledge and make any comments or observations and importantly the learning actions that are required to complete to support their induction process. This information can be discussed with your line managers/ supervisor, team colleagues or mentor or any person that is supporting the induction programme.

5. Evaluating your learning during your induction to this integrated service.

5.2 Exercises to support individual and shared learning This section is designed for staff to complete during their period of induction as agreed in discussions with the line manager. It is a resource staff can refer to on an ongoing basis and to discuss with the line manager, team members and colleagues.

Staff should be able to complete these exercises, individually or with others in some dedicated time during their induction period.

Exercise Seven: Developing opportunities to share learning/joint training

Guidance Notes: This exercise will allow you to focus on the learning opportunities available to you in the early stages of your employment within this integrated services. This will assist you in the planning to attend joint training.

As part of your standard induction you will be required to attend mandatory training. This may include moving and handling, management of aggression and fire safety and infection control. However, for the purposes of this activity we will focus on the training that will support your knowledge and early development of within this integrated service.

It is important that you are aware of the type and appropriateness of training available to you. The training within your area may be wide and varied. Opportunities may be available in-house or delivered by commissioned training providers. Furthermore, there may be other methods within your service that will support your learning. This exercise will give you the opportunity to identify opportunities potentially available to you.

Activity: Take some time to identify the training opportunities that are available to you. The following session/ themes may be available.
