IX. Free and Reduced Price Meals and Free Milk: 2016/2017 School Year Be It Resolved, that the Board of Education authorizes the District Clerk to sign all forms necessary to enable Garden City Public Schools to participate in the Free and Reduced Price Meals and Free Milk programs for the 2016/2017 school year so that the school district may be eligible to receive program reimbursements.

X. Re-Appointment of School District Physician Be It Resolved, that the Board of Education hereby approves the re-appointment of Ronald A. Feinstein, M.D., as the School District Physician/School Medical Officer for the 2016/2017 school year at a cost of $46,454. Be It Further Resolved, that the Board authorizes the Board President to execute the consultant agreement between Steven and Alexandra Cohen Children’s Medical Center of New York, a Division of the Long Island Jewish Medical Center, and the Garden City Union Free School District for these services on its behalf.

XI. Depositories for District Funds Be It Resolved, that the Board of Education adopts resolutions for depositories for district funds, pursuant to §2130 of the Education Law, designates the Board of Education President, Board of Education Vice President, District Treasurer, or District Deputy Treasurer, to sign checks.

XII. Night Depository Be It Resolved, that the Board of Education adopts resolutions and agreement for night depository service by JP Morgan Chase.

XIII. Bonding Be It Resolved, that the Board of Education approves the bonding of the following titles in the amount of $5 million for the 2016/2017 school year: President Vice President Treasurer Deputy Treasurer

XIV. Designation of Employees Authorized to Open Bids RESOLVED, that the Assistant Superintendent for Business and/or her designee in the Business Office are designated to perform this function. In their absence it is recommended that the Assistant Superintendent for Personnel be designated to perform this function.

XV. Certification of Payroll Reports RESOLVED, that the Assistant Superintendent for Business and Finance, or in her absence, the Assistant Business Administrator.

XVI. Authorization to Invest District Monies RESOLVED, that the District Treasurer and Deputy Treasurer and/or the Assistant Superintendent for Business and Finance, are hereby authorized to invest district monies in accordance with Article 35, §1723 of the Education Law.

XVII. Authorization of Facsimile Signature RESOLVED, that the District Treasurer, and Deputy Treasurer are authorized to either sign checks manually or by use of electronic means for the purpose of inscribing facsimile signatures; and shall maintain control of his/her own facsimile signature disk.

XVIII. Establishment of the 2016/2017 Tax Levy WHEREAS, the 2016/2017 Budget for the Garden City Union Free School District has been approved in the amount of $110,827,444, now THEREFORE, Be It Resolved, that the 2016/2017 school tax levy for school purposes for the Garden City Union Free School district be established at $95,936,646.

XXVI. Lead Educational Agency for Nassau TRACT Be It Resolved, that the Board of Education approves the designation of Garden City Union Free School District as Lead Educational Agency (LEA) for Nassau Tract.

XXVII. Educational Data Services, Inc. – Cooperative Bidding Be It Resolved, that the Board of Education approves the participation of the Garden City Union Free School District in the Educational Data Services Cooperative Bidding Program effective for the 2016/2017 school year.

XXVIII. Re-Adoption of District Emergency Plan RESOLVED, that the District Emergency Plan be re-adopted for the 2016/2017 school year. A copy of the District Emergency Plan will be on file in the office of the Assistant Superintendent for Business.

XXIX. Collection and Review of Prevailing Wage Data RESOLVED, that the District Director of Facilities be designated as the individual responsible for the collection and review of prevailing wage data paid to contractors in conformance with New York State Labor Law.

XXX. Designation of Persons Authorized to Accept Service of Pleadings and Supporting Papers RESOLVED, that in the event that the Garden City Union Free School District is named as a party respondent, service upon such school district shall be deemed proper only if the petition is personally served upon one of the following individuals: District Clerk, any member of the Board of Education, Superintendent of Schools, Treasurer or her designee.

XXXI. Determination of Residency RESOLVED, that the Board of Education, in accordance with §100.2(y) of the Regulations of the Commissioner of Education of the State of New York, appoints the Superintendent of Schools and the Residency Hearing Officer, or the Superintendent’s designee, to conduct hearings and to determine whether a child is entitled to attend the schools of the District for the balance of the school year.

XXXII. Authorized Signature for Nassau County Civil Service Commission RESOLVED, that the Board of Education, in accordance with Sections 95, 100, and 101 of the New York State Civil Service Law, authorizes the President of the Board of Education to accept responsibility for signing payroll certifications. This responsibility cannot be designated.

XXXIII. Authorized Signature for Tuition Payment to Public, Non-Public and Residential Facilities RESOLVED, upon receipt and approval by the Board of Education of recommendations from the Committee on Special Education, the Board of Education authorizes its president and/or district clerk to sign the necessary documents to approve tuition payment to public, non-public and residential facilities for enrollment of Garden City school-age children for educational programs of special education.

XXXIV. Budget Transfer Authorization RESOLVED, that in accordance with the Regulations of the Commissioner of Education, the Superintendent of Schools is authorized to make transfers between and within functional unit appropriations of the expenditure budget so long as such transfers are not in violation of a law or regulation and are in support of activities categorically approved by the Board of Education in the annual expenditure budget. Transfers between accounts that exceed $5,000 shall be approved by the Board of Education in accordance with district Policy #6150.

XXXV. Designation of Official Newspapers of the District RESOLVED, that the Board of Education designates the following two official newspapers of the Garden City Union Free School District: Garden City News Garden City Life

XXXVI. Designation of District Advertising Agency RESOLVED, that the Board of Education appoints Miller Advertising Agency, Inc., to provide advertising services for employment recruitment ads for the 2016/2017 school year.

XXXVII. Bank Depository – Bank Accounts – Authorized Signatories RESOLVED, that pursuant to §2130 of the Education Law, the following banks: TD Bank JPMorgan Chase Flushing Bank Community National Bank be and the same are hereby designated as the depositories for funds of Garden City Union Free School District, Town of Hempstead, Nassau County, New York, during the ensuing fiscal year. FURTHER RESOLVED, that the bank accounts of Garden City Union Free School District, Town of Hempstead, Nassau County, New York, be designated as follows: General Account No. 1; General Account No. 2; Payroll Account; Cafeteria Account; Capital Bond Account; Capital Fund Account; Federal Aid Fund; Trust and Agency Fund; Money Market Account; Scholarship Funds; and Debt Service Account.

FURTHER RESOLVED, that at least one of the four authorized signatories are required upon said bank accounts. The authorized signatories are the President, Vice President, Treasurer, and Deputy Treasurer; FURTHER RESOLVED, in respect of the General Account No. 2, no direct deposits shall be made in this account and no funds shall be segregated in this account except funds made available out of the General Account No. 1 and Capital Fund Account and such segregation shall only be made under specific instructions of the Treasurer, or Deputy Treasurer; and electronic transfers of the District upon the depository or depositories of the General Account No. 2 shall only be made under the specific instructions of the Assistant Superintendent for Business and Finance. FURTHER RESOLVED, in respect of the Payroll Account, and pursuant to §1720 of the Education Law of New York, electronic transfers of funds shall be drawn upon the General Account No. 1, and Federal Aid Fund in the total net amount of a duly certified payroll, after payroll deductions, for the salaries of regularly appointed employees and officers legally entitled to be compensated for their services, the net proceeds of such electronic transfer to be credited to the Payroll Account from which checks shall be signed by the authorized signatories, or in the absence or inability of the authorized signatories to sign, and that the aforesaid signature may be a facsimile signature, and checks shall be drawn payable to individual employees or officers legally entitled to be compensated for services, and no deposits shall be made in the Payroll Account except through the electronic transfer of funds as aforesaid; FURTHER RESOLVED, that the check-signing device, in the form of appropriate USB device, is hereby authorized and approved for continued use. FURTHER RESOLVED, that the Treasurer be and she hereby is authorized to file specimens of the facsimile signatures hereinbefore authorized in these resolutions with the depositories of the accounts of the District on which checks signed with facsimile signatures are authorized by the foregoing resolutions, and said depositories are hereby authorized to honor checks to which said facsimile signatures are affixed to the same extent and in the same manner as though said signatures had been manually signed; FURTHER RESOLVED, that the Treasurer or in the absence of the Treasurer, the Deputy Treasurer and/or the Assistant Superintendent for Business and Finance, is hereby authorized to invest District funds in accordance with the provisions of §1723-a of the Education Law of the State of New York.

12. Apple, Inc. Agreement Be it Resolved that the Board of Education approve the agreement with Apple Inc. This is Apple’s Single Purchase Agreement and the Board is accepting their terms and conditions of purchase. This agreement is for the period of July 1, 2016 through June 30, 2017. Administrators’ Retreat Agreement – Hofstra University Be It Resolved, that the Board of Education approves the agreement with Hofstra University for the use of its facilities for the Administrators’ Retreat on August 29, 2016, at no charge. Be It Further Resolved, that the Board of Education, subject to review of district counsel, authorizes the Board President or District Clerk to execute said agreement on its behalf.

30. Football Camp Agreement – Camp Wah-Nee Be It Resolved, that the Board of Education approves the agreement with Camp Wah-Nee for the Varsity Football Team to attend training camp, August 21st to 25th, 2016. Be It Further Resolved, that the Board of Education, subject to review of district counsel, authorizes the Board President or District Clerk to execute said agreement on its behalf.

31. Donation – The Men’s Association (Varsity Soccer) Be It Resolved, that the Board of Education accepts a donation of $3,200 from The Men’s Association to cover the cost of coach bus transportation for the Girls Varsity Soccer Team trip to Queensbury, New York in the fall.

32. Donation – The Men’s Association (Kickline) Be It Resolved, that the Board of Education accepts a donation of $1,544 from The Men’s Association to be used to purchase twelve new Kickline uniforms to cover the growth of membership. Be It Further Resolved, that the Board of Education approves an increase in the 2016/2017 General Fund Revenue and Appropriation Budgets code A2855-500-02-1000 in the amount of $1,544 for this purpose.

33. Donation – The Men’s Association (Football) Be It Resolved, that the Board of Education accepts a donation of $2,600 from The Men’s Association to cover the cost of bus transportation for the Varsity Football Team trip to Camp Wah-Nee in August 2016.

34. Smart Schools Investment Bond Be It Resolved, that the Board of Education approves the Garden City Smart Schools Investment Bond as presented to the Board on May 17, 2016.

35. 2016/2017 Annual Professional Performance Review Plan (APPR) Whereas, the District has reached agreement on a revised Annual Professional Performance Review (APPR) Plan for the 2016/2017 school year; Whereas, the District acknowledges that the District’s 2015/2016 Annual Professional Performance Review Plan (APPR) Plan that was approved by the Commissioner of Education, remains in effect until such time as the Commissioner approves a revised and/or subsequent Plan for the 2016/2017 school year. Be It Resolved, that the Board of Education of the Garden City Union Free School District, upon recommendation of the Superintendent of Schools, hereby approves the revised APPR Plan for the 2016/2017 school year for submission to the Commissioner of Education and authorizes the Superintendent of Schools to submit the revised APPR Plan and all other required documentation to the Commissioner of Education; Be It Further Resolved, that the Board of Education hereby authorizes the President of the Board of Education and the Superintendent of Schools to execute any and all necessary certification forms required by the Commissioner of Education for the District’s 2016/2017 APPR Plan, including but not limited to the Implementation Certification Form.
