Threeinterventionsdesignedforfollowupofpatientswhoareidentifiedwithsuicideriskinhospitalemergencydepartmentssavelivesandarecosteffectiverelativetousualcare.AstudyledbyresearchersattheNationalInstituteofMentalHealth(NIMH)modelledtheuseoftheapproachesinemergencydepartmentsandfoundthatallthreeinterventionscomparefavorablywithastandardbenchmarkofcost-effectivenessusedinevaluatinghealthcarecosts.

Oneintervention,sendingcaringpostcardsorlettersfollowinganemergencyvisit,ismoreeffectiveandlessexpensivethanusualcare.ThereportappearsintheSeptember15issueofthejournalPsychiatricServices.AccordingtotheCentersforDiseaseControlandPrevention(CDC),suicideisthe10thleadingcauseofdeathintheUnitedStates;44,193peoplediedbysuicideintheUnitedStatesin2015(themostrecentyearforwhichstatisticsareavailable).Oneapproachtoreducingthesuiciderateistodirectpreventionstrategiesathigh-riskgroupsorsettings.Anexampleisemergencydepartments,whichaccordingtotheCDC,treatmorethan500,000peopleeachyearforself-harminjuries.“Inthefaceofagraduallyrisingsuiciderate,theneedforeffectivepreventionstrategiesisurgent,”saidNIMHDirectorJoshuaGordon,M.D.,Ph.D.“Thesefindingsofcost-effectivenessaddtotheimpetusforimplementingtheselife-savingapproaches.Importantly,theyalsomakeastrongcaseforexpandingscreening,whichwouldallowustoreachmanymoreofthoseatriskwithlife-savinginterventions.”Researchhasfoundseveralemergencydepartment-basedinterventionstobeeffectiveinpreventingpost-emergencysuicideattempts,butnonehasbeenwidelydisseminatedoradoptedyet.Theyare:• Postcards:hospitalstaffmailfollow-uppostcardseachmonthforfourmonthstoall

patientsidentifiedasatrisk,andtheneveryothermonthforatotalofeightcards.• Telephoneoutreach:Onetothreemonthsafterdischarge,hospitalstaffcallpatientstooffer

supportandencourageengagementinfollow-uptreatment.• Cognitivebehavioraltherapy:Hospitalstaffconnectpatientstoasuicide-focusedcognitive

Dr.ShaylaSullivantsaidstigmaandeasyaccesstogunsareproblemsthatcanberemediedinindividualhomes.Shepointedtofamousfigureswhoexperienceddepressionandsuicidalideology,rangingfromAbrahamLincolntoactressHalleBerryandauthorJ.K.Rowling."Thinkofallthesepeopleandthecontributionstheyhavemadetooursociety,"shesaid."Thereisreasonforustohopeandthereisreasonforustoalsothinkabouthownothavingaccesstoafirearmwhenthesepeoplewereinthedepthsofdespairispartlywhytheycontributedwhattheydid."Sullivantsaidsheoftencounselsparentsofherclientstoeitherremovegunsfromtheirhomesorkeepthemlockedinasafe.Shesaidit'samisconceptionthatyouthsalwaysplansuicidewellinadvanceoftakingtheirlives.Often,shenoted,teenswhoattemptedsuicidetellhertheymadetheirdecisionjust10minutespriortotheact.Ifchildrendon'thaveahighlylethalmeanstotaketheirlivesduringthisimpulsiveperiod,Sullivantsaid,theirlivesoftencanbesaved.Ninetypercentofindividualswhosurviveasuicideattemptdonotgoontodiebysuicideatalaterdate,accordingtoDr.ShaylaSullivant.It'ssimplynottrue,Sullivantsaid,thatvictimsofsuicidewillalwaysfindawaytocompletetheactifthemostlethaloptionisn'tavailable.ShepointedtothenationofSriLanka,whichhadthehighestrateofsuicideintheworldwhendeadlypesticideswerereadilyavailable.Oncerestrictionswereputonthemostlethalpesticidesandsafestoragewasprioritized,thesuiciderateplummeted.ShealsorecalledthatintheUnitedStatesinthe1970s,manypeopledidn'twearseatbelts"andhownoonewantedtodoit-andhownowwedon'tevenblink,wejustgetinthecaranddoit."Automobilefatalitieswereskyrocketinginthe'70s,butthenumberofdeathspercapitainautocrasheshasdroppedbymorethan35percentsincethattime.Sullivantsaidsuicideratesamongyouthsalsocanbedramaticallycurtailedbyreducingstigma,keepingfirearmsstoredorbyfamilymembersmakingthedecisiontoremovegunsfromthehomesofstrugglingyouths. KirstenMcNelis,M.A.,AmericanUniversityThis38-pagewhitepapercoversrecentliteratureregardingmalehelp-seekingbehaviorincludingasectiononfactorspromotinghelp-seekingamongmen.Abstract:Withbutoneexceptionworld-wide,malescompletesuicideatratesexceedingthoseoffemales.ThemaletofemalesuicideratiointheUnitedStatesisgreaterthan4:1.Explanatoryhypothesesforthesefindingsfocusongenderrolebehaviors,specificallythegreaterinvolvementofmeninhigh-riskhealthbehaviorsandthegreaterpropensityofwomentoseekandusesupportsforhelpwheninneed.Thispaperexploreswhatisknownaboutgenderdifferencesinhelp-seekingbehaviortodeterminefactorsthatmaypromoteincreasedhelp-seekingamongmen.Onthebasisofthisresearch,weproposepublichealthinterventionapproachesthathavepotentialtomotivatemenat-riskforsuicidetoseekandreceivehelp. Sometimesimportantresearchisfilledwithjargon,ishardtounderstand,ordoesn’tseemtomakesense.We’vetriedtosummarizeresearchincommonEnglishlanguage.SomePhysicalConditionsAssociatedwithHigherSuicideRisk

Researchersstudiedphysicalhealthconditionsthroughpopulation-basedinvestigationthatprevioussmallerresearchstudiessuggestedputindividualsathigherriskforsuicide.Researchersselectedonehundredpeopleenrolledinoneofeighthealthinsuranceplansacross15statesforeachenrolledpersonwhodiedbysuicide(2,674individuals)duringa14-yearperiodforatotalof267,400matchedcontrolcases.Afterreviewingelectronichealthrecordsandaccountingforothermitigatingfactors(e.g.,mentalhealth&substanceabusediagnoses),theresearchersfoundthattraumaticbraininjurywasthemostinfluentialriskfactor.Additionally,havingasleepdisorder,HIV/AIDS,ormultiplediagnosedphysicalhealthconditionsalsowereassociatedwithasignificantriskofsuicide.Anemphasiswasplacedonimplementingeffectivesuicidepreventionstrategiestothosewithatraumaticbraininjurybecauseofthestrongassociationfoundbetweenbraininjuriesandsuiciderisk.Theresultssupporttheneedtoimplementprevention-basedstrategiesacrossallhealthcaresettings.Healthsystemsnormallytargetsuicidepreventioneffortstowardspatientswhohaveexpressedthoughtsaboutsuicide,mentalhealthconditions,andsubstanceusedisorders.Limitingpreventionstrategiestopatientsinthebehavioralhealthsystemsrunstheriskoffailingtorecognizeindividualsatriskforsuicidewhohavenotreceivedamentalhealthdiagnosis.Theauthorsconcludedthatsuicidepreventioneffortscouldbemoreeffectiveifimplementedacrossbroaderhealthcaresettingsasopposedtorestrictingendeavorstothebehavioralhealthsettings(SummarybyStephenRoggenbaum&YaritzaCarmona).

1.What’stheonepieceofadviceyougivethemostoften?Thattheoldsaying“timehealsallwounds”isn’tnecessarilytrue.Instead,Itellpeople“timepasses,itdoesn’theal.”Healingishardandmustnotbeignoredoritwillcomebackandhurt.2.Whataresomeoftheuniversalexperiencesyou’veseeninyourwork?Shock,feelingnumb,denial,disbelief,fear,questioningaboutpost-death,deepsadness,healing,laughter,recovery.3.What’sonethingyou’dliketosharewithanewgriever?Bepatientwithyourself.Everyonegrievesdifferentlyandthewayyouexperiencegriefisyours.Don’tfightitanddon’tdenyit.Experienceit.4.Whataresomewaysotherscanhelpsupportsomeoneintheirgrief?Spendingtimewiththem.Lettingthemgrieveforaslongastheyneedto.Bringinguptheirlovedoneformonthsandyearstocome.5.Arethereanyaspectsofgriefothersmayfindsurprising?I’mnotsureaboutsurprising,butgriefcanincludebeingangryandthatsometimesisdifficultforpeople.Anotherthingthattheydon’texpectishowhardthegriefhitsthemaftersometimehasgoneby.Theymighthearasong,seesomeonewholooksliketheirlovedone,hearasimilarvoiceandthatcanbe2yearsafteradeathbutinasplitsecondbringsitallbacklikeitwaslosingthepersonalloveragain.Finally,dreamscanbereallyhardforpeople,goodandbaddreams.6.Isthereanythingelseyou’dliketosharethatyouthinkcanhelpothers?Wedon’tdealwithdeathwellinourcultureandthismakesgrievingreallyhard.Ifwecouldtalkaboutitmore,findwaystoacceptitaspartoflife,thatwouldhelpusintheworsttimesofourlife.Ialsothinkpeopledon’tknowthemajorimpactofearlychildhoodexperienceswithgriefandlossthataffectyouasadults.Ifgriefandlosswereachallengeforyourfamilyandforyouasachild,itlikelywillbeasanadult.Finally,it’shelpfultoknowthatwhilewecanneverknowalloftheanswersastohoworwhywe’rehavingtofacegriefheadon,wecanknowthateveryonehastodothisintheirlifetimeatsomepoint.Relyonotherstohelpyouthroughit,youdon’tneedtocreateanewpathorevenhavetotraveltheroadalone,butyoudoneedtogothroughitandthatisapainfulrealityweallhavetoexperience.

• Guilt–Youmayfeelguiltyaboutthesuicideandthinkabouthowyoushouldhavestoppedthepersonfromcommittingsuicide.

• Despair-Youmightfeelcompletelyalone,sad,andhelpless.Youcouldbecomephysicallyandemotionallysickandpossiblyconsidersuicideyourself.

CopingwithasuicideAlloftheemotionsthatsurvivorsofsuicideexperiencearecompletelynormal.Healingfromsuicidetakestime.Hereareafewthingstothinkaboutwhentryingtorecoverfromthelossofalovedoneorfriend:• Keepintouch–Besuretostayintouchwithlovedonesandfriendsforcomfort,
