• Read carefully before using.

Introduction

Software description

The "FORUM Viewer App" for the iPad allows access to patient data available in a FORUM database. With the help of this app, patient documents1 stored in FORUM can be viewed on an iPad at any location with access to the local wireless network or VPN2. In addition, examination orders can be generated for patients. The FORUM Viewer App expands the FORUM user options, since the iPad also allows access to FORUM data via wireless networks in rooms without PC equipment and even at remote locations via VPN.

The standard iPad gesture control allows simple and intuitive operation of the app.

Interfaces

The FORUM Viewer App allows you to read and display patient data and doc-uments stored in FORUM, as well as generate examination orders. A local wireless network or a VPN connection is required to connect to the FORUM server.

Gesture control

The FORUM Viewer App can be operated using the following gestures (finger movements) common to the iPad:

Scroll (up/down)

Swipe your finger up or down:

– scroll through patient and/or examination list

– scroll through a multipage document

Scroll (right/left)

Swipe your finger to the right or left:

– scroll through the selected files in the display

– Exchange the displayed files with the previous or the next document of the current selection in Compare View

Zoom Two finger swipe up: Continuously zoom in display to original document size.

Two finger pinch: Continuously zoom out the image section to the size of the display field.

Intelligent zooming

One finger double-tap: Gradually zoom in the display to the original document size.

Two-finger tap on a zoomed-in image section: Gradually zoom out the image section to the size of the display field.

Two-finger double-tap on a zoomed-in image section: Fit the document to the size of the display field.

Move Swipe the finger in a random direction (zoomed-in document display):Moving the indicated image section.

The following safety information is incorporated into the Instructions for Use. Observe this safety information and take particular care in these situations.

Security measures for IT systems and networks

The consequences resulting from attacks in unprotected networks cannot be foreseen.

An analysis, evaluation and possibly measures required for controlling the risks by the operator (user) are recommended. This data assessment must be updated after each change to the (data) network.

Data protection and information security

The user and/or the person responsible for IT must be responsible for observing national data protection laws and regulations.

Operators of IT systems and IT networks are responsible for determining the necessary security standards, i.e. for creating the necessary technical and organizational framework conditions.

Misuse has occurred if protected personal data are collected, processed or used without proper authorization.

Suitable measures must be taken to ensure that only authorized users can access the iPad on which the FORUM Viewer App is installed (e.g. by locking the iPad). The FORUM Viewer App must only be operated in secured net-works. The operator operating the software is responsible for the security of the network.

Observe the following definitions:

– Personal data: All information regarding personal or material circum-stances of a particular or identifiable person. All data (e.g. name, age, gen-der, etc.) that can be directly assigned to a person (patient, employee, customer, supplier) must be protected.

– Data processing: The storage, transfer (transmission to third parties out-side of the organization), modification (including anonymization and pseudonymization), deletion, and blocking (marking to limit further pro-cessing or use) of data.

– Use: The use of data (e.g. transfer within the company).

– Third parties: Persons or entities that are not part of the entity responsi-ble (legal entity). The transmission of data to third parties is considered data transfer.

Securing the iPadSecure your iPad and your data against being accessed by third parties by acti-vating a code-lock as well as the automatic lock (in the "Settings" under "General").

Connection to data networks

With data that the software sends into the data network or data available in the data network, there is the danger that they may be falsified or transferred incompletely. Therefore, no liability can be accepted for the correctness of the data.

The operator of the data network is responsible for compliance with the legal requirements regarding data protection and for the protection of personal rights.

RA005 Suitable measures must be taken to ensure that only authorized users can access the iPads on which the FORUM Viewer App is installed (e.g. by locking the iPad).

Notes on the hardware platform

Use of an inappropriate hardware platform may impair the function of the software. The operator (user) is responsible for the correct selection and function of the hardware (see information about "Hardware and software configuration" on page 15).

Information on use

• Make sure you are thoroughly familiar with the contents of the Instruc-tions for Use before starting up the software.

• Rx OnlyCaution: Federal (USA) Law restricts this device to sale by or on order of a physician.

• Keep the Instructions for Use available for operating personnel at all times.

• The FORUM Viewer App may only be used by medical experts.

• Familiarize yourself with the software user settings.

• Use the software only for the intended purpose as described.

• Warranty and liability depend on the contractually stipulated conditions.

• The manufacturer does not accept any liability for damages caused by un-authorized tampering with the product. Furthermore, this will forfeit any rights to claims under the warranty.

• Modifications to this product are not permitted. Making modifications to the device that are not described in these Instructions for Use makes you a manufacturer of a medical device as defined by the applicable national law (legal manufacturer).

• After the system time or date is changed on the server, the iPad must be restarted before the FORUM Viewer App can be used further.

• Please refer to the latest release notes for the installed software version. They are available for download with these Instructions for Use.

• The electronic Instructions for Use is available for download in PDF format. You can order a printed copy of the Instructions for Use by email (to: forum-app.meditec@zeiss.com) free of charge. Delivery time is about 7 days.

• Observe the legal regulations on accident prevention and occupational safety applicable in the particular country.

Safe functioning

The FORUM Viewer App is a support system, and does not assume any diag-nostic functions that are critical to safety. Treatments can be performed without the aid of FORUM Viewer App at any time.

If a fault arises that you cannot correct based on the chapter "Trouble-shooting", mark the product as non-functional and inform ZEISS Service.

Intended use / Indications for Use

The FORUM Viewer App is a FORUM application for the display and view of patient and diagnostic data and images stored in FORUM. It is designed to be used by medical experts on mobile devices. The FORUM Viewer App is not intended as a replacement for FORUM Viewer.

Contraindication

FORUM – as well as the FORUM Viewer App – is not intended to be used for radiology image (X-ray) diagnostic support.

Use in accordance with the designated purpose

The FORUM Viewer App is an application for the Apple® iPad®. It allows read-only access to patient and examination data (images and reports), which are stored in FORUM. Examination data can be viewed in detail. There is an option to view two documents at the same time to allow for an easy comparison.

The FORUM Viewer App can be used for patient training or for the assessment of patient data by trained medical personnel.

Hardware and software configuration

The following is required for the use of FORUM Viewer App:

for the iPad: iPad 2 or newer

iOS 8 or higher

the iPad must be configured for a network connection via the local wire-less network or via VPN (see also "Configure VPN" on page 17)

An appropriate app must be installed on the iPad for the playback of video files. Functional requirements:

– Playing MPEG2 files

– URL transmission

Recommended: GoodPlayer in Version 9.9 or higher

to FORUM the FORUM server must be tied into the local wireless network or be

accessible via a VPN

Required licenses

The FORUM Viewer App requires a FORUM Tablet Interface License to be installed on the FORUM server, as well as a free user license for FORUM. The FORUM Tablet Interface License is required to activate the interface. The user license will be used during the data transmission from the FORUM server to the iPad. Data transmission takes place during user registration, with each entry in the search field of the patient list, with each manual data update and when documents are displayed. 60 seconds after completion of the data transmission, the user license is reactivated.

Basic iPad configuration

Setting the brightness

To ensure optimum display of FORUM files on the iPad, set the screen bright-ness to maximum and deactivate automatic brightness adjustment:

• Open the iPad "Settings" and select "Display & Brightness".

• Deactivate the function "Auto-Brightness" and set to maximum brightness using the slider bar.

CAUTION Insufficient display qualityRA022 Ensure that environmental conditions. e.g. brightness and reflection as well as

the display brightness used allow evaluation or assessment of diagnostic data. The operator (user) has an obligation to comply with local display regulations.

Configure VPN

If communication between the FORUM Viewer App and the FORUM server takes place via a VPN connection, it can be configured as follows:

Prerequisite The required settings for VPN access are known.

The iPad is connected to a 3G, 4G, LTE or wireless network.

Actions • Open the iPad "Settings" and select the entry "General" and after that "VPN".

• Select "Add VPN Configuration".

• Enter all relevant data.

• Tap on "Save".

Result The VPN configuration is complete.

Under "Settings" "VPN" is displayed as a new entry. This entry allows you to activate or deactivate the VPN connection quickly.

Installation

Ensure the iPad meets hard and software requirements ("Hardware and software configuration" on page 15) for the installation of the FORUM Viewer App. Furthermore, you will need an Apple account (Apple ID with password) with sufficient funds or payment information on file (a nom-inal fee is charged for the App download). The FORUM Viewer App is installed analogous to any other app from the App Store via iTunes or directly on the iPad. The App can be found in the App Store under the description "FORUM".

After successful installation, the following App symbol is displayed on the iPad screen:

Configuration

Before the first use, the FORUM Viewer App must be configured for access to the Forum server:

• On the iPad under "Settings" select the entry "FORUM".

• Enter the required and desired parameters:

The FORUM Viewer App uses the AutoConnect™ function: If the iPad uses the same network as the Forum server, it will connect automatically. Once the app is opened for the first time, the server IP address in the login window is displayed and, after registration, it is automatically saved in the "Settings". If the connection cannot be established automatically, the FORUM server address can be entered manually.

Ensure that the FORUM server port (8080) is accessible through your firewall or the router.

Show hidden images Documents that cannot be displayed with the FORUM Viewer App are by default not displayed as a thumbnail. If these documents need to be dis-played with a placeholder icon as a thumbnail, this function must be activated.

Show newest documents first

If the function is activated, thumbnails and docu-ments of an examination are displayed chronolog-ically in descending order (newest document first); if the function is deactivated, it is displayed in ascending order (oldest document first).

Log out at next start Function to open the login window (the FORUM Viewer App does not have a button for logout).

See section "Logout" on page 32.

FORUM server time zone By default, document date and time specifications on the iPad are always displayed according to the time zone of the FORUM server. If the iPad is located in a different time zone from that of the FORUM server, the time zone of the server can be specified. Date and time specifications are then converted to the iPad time zone according to the time difference.

Hide demo mode By default, the login window offers the option to select demo mode (example database). If this function is activated, demo mode is not available any longer.

Actions • Open the FORUM Viewer App on the iPad screen.

• Enter login data1, taking into account capitalization requirements.

Using the button <Demo mode> of the login window, you can access the example database.2

Tapping the icon opens an information window on the FORUM Viewer App containing conformity information and a contact address. This window also displays the UDI (Unique Device Identification).

Result The FORUM Viewer App reads the current data on the FORUM Server and indicates all patients whose files contain displayable documents of the current date (List "Today’s patients")3:

The imported data pool is not updated automatically. Updates are made after each login, each character entry in the search field or when a manual update is performed (see "Updating data" on page 32).

Touch ID loginIn addition to the traditional login by using a user name and a password, the FORUM Viewer App also supports login using a fingerprint (Touch ID).

Prerequisite:

The iPad to be used has a fingerprint sensor.

Touch ID is installed and activated.

After you first open the FORUM Viewer App using your user name and pass-word, every time you open the app after that, you are automatically prompted to login with Touch ID, i.e. using the captured fingerprint.

You can login with your user name and password at any time by canceling the prompt to authenticate via Touch ID.

Viewing documents

Selecting a patient

In the left-hand section of the screen ("Patients"1), all patients are listed who correspond to the current search criteria.

The patient list can be filtered as follows:

– <Today’s patients>A list appears with all patients who have documents of the current date. This filter is automatically activated when the app is started.

– <All patients>All patients with displayable documents are listed.

NOTE MisapplicationRA025 If the FORUM Viewer App is used for medical assessment or an evaluation of

diagnostic data, the operator (user) is obliged to comply with local display regulations.

– "Search patients"All patients whose last name1 contains the character string entered in this field or whose patient ID2 contains the number string entered.With each character that is entered or deleted in the search field, a data update is performed, i.e. the hit list in the FORUM Viewer App is updated according to the data stored on the FORUM server.

By swiping your finger up or down you can scroll through the patient list.

Actions • Tap the patient whose documents you wish to view.

Result The right-hand section of the screen("Visits") lists all examinations of the patient, sorted by examination date in descending order. Examinations performed on ZEISS devices are generally indicated by an icon of the ex-amination device; all others have a thumbnail.

1) During the search, the FORUM Viewer App supports the FORUM function of multiple com-ponent names (alphabetical, ideographic or phonetic representation of names). In a patient search, the data in all three components of the name are taken into account (but not displayed).

2) The FORUM Viewer App also supports the NHS format (required in the UK) when searching for patient IDs if FORUM has been configured accordingly.

Display patient information

The most important information on a patient can be accessed via the FORUM Viewer App:

Actions • Tap the icon of the selected patient.

Result

It is possible to generate examination orders for this patient from this win-dow (see "Create automatic order" on page 31).

To hide patient information, tap any area on the screen outside the informa-tion window.

Selecting documents

Actions • Select the desired patient (see "Selecting a patient" on page 22).

• In the section "Visits" tap all examinations for which you want to view the associated documents.

The selected examinations are indicated by a blue frame.

• Select <Preview>.

Sorting the previewFor each selected examination a line with thumbnails for the included docu-ments is displayed. The examinations are listed chronologically and in descending order.

Within the examinations, the thumbnails are first displayed, sorted by lat-erality (first OD, then OS), then sorted by time of compilation. Depending on configuration, (see page 20) either the newest or the oldest document will be displayed first1.

• If necessary, tap <Visits> to change the selection of examinations.

• Tap all thumbnails to be selected for the display.

Result The selected documents are indicated by a blue frame.

All available documents pertaining to the selected examinations are displayed with a thumbnail. Each line corresponds to an examination.

Display documents

Actions • Select the desired documents (see "Selecting documents" on page 25).

• Tap <Display> to open the selected documents for the display.

or

• Tap the icon on the right edge of a specific examination to pull up all available documents exclusively associated with this examination for the display.

Result The selected documents are opened up for a full-screen display. While the data is being loaded, the following icon appears:

On the bottom right, the number of the displayed document, as well as the total number of opened documents is indicated. Swiping your finger left or right lets you scroll through the opened documents.

Sorting opened documentsThe opened documents are first displayed sorted by laterality (first OD, then OS followed by OU), then sorted by time of compilation. Depending on con-figuration, (see page 20) either the newest or the oldest document will be dis-played first1.

RA013, RA014 During transmission, data is not changed by compression or other methods, but is displayed in the same quality with which it is stored in FORUM.

Compare View

In order to be able to compare two documents directly with each other, two documents can be displayed side by side.

Prerequisite At least two documents are pulled up for display.

Actions • Tap the icon on the top right.

Result

On the right, below each display, the number of the displayed document as well as the total number of opened documents is indicated.

Swiping your finger left or right in each display field allows you to swap the displayed document with any of the other opened documents.

Opening Compare View while displaying the last (oldest) of the selected doc-uments causes this document to be displayed side by side twice.

Leave Compare View• Tap the icon to return to Normal View.

The document from the left display field is displayed.

Scale / move displays

Generally, every document for the display, be it single display or comparison display, is scaled to a degree that it is displayed at its full width and in full-screen format.

By swiping up or pinching the screen with two fingers, you can zoom in the document display to its maximum resolution or you can zoom back out to its original size.

Double-tapping the screen displays the indicated document at its maximum resolution.

A displayed zoomed-in screen section can be moved by using one finger.

Multi-page PDF documents

By swiping your finger up or down you can scroll through multi-page PDF doc-uments. The current displayed page as well as the total number of pages is indicated on the top left while scrolling.

Image series

Image series display a field for scrolling on the top left, which also contains information on the total number of images as well as the number of the cur-rent displayed image:

• Tap "+" or "–" to scroll through the individual images of the image series.

Videos

To play videos, the iPad must have an appropriate app installed which must be configured in the settings of the FORUM Viewer App (see "Hardware and software configuration" on page 15 and"Configuration" on page 19).

If a video file is opened and started, the corresponding app automatically opens and plays the video.

A manual return is required to get from the video player back to the FORUM Viewer App.

Create automatic order

Using the integrated function "Create automatic order" allows you to request standard procedures for a patient directly from the FORUM Viewer App with-out any further intermediate steps. The request is transmitted to FORUM and forwarded to the respective examination devices.

Prerequisite FORUM in Version 4.1 or higher

At least one examination device is connected to FORUM and has been configured in FORUM as "Modality Worklist User" with the "Automatic Orders" option.

Actions • Select the desired patient (see "Selecting a patient" on page 22).

• Open patient information (see "Display patient information" on page 24).

• Tap <Create automatic order>.

Result In FORUM orders with examination requirements are created for all appro-priately configured devices. The respective devices will immediately be able to call up the requested procedures.

Updating data

Data displayed in the FORUM Viewer App are updated automatically with each user login and/or when characters are entered in the search field. The stored data can also be updated manually at any time:

Actions • Tap the icon above the patient list .

Result The FORUM Viewer App connects to the FORUM server. The stored data is updated.

All filters used prior to the update stay active.

Logout

The FORUM Viewer App remains open, even if a different app is used in between.

Automatic logout of the user takes place after 10 minutes, if

– another app is active on the screen,

– the iPad was turned off,

– the lock screen is active.

There is no automatic logout as long as the FORUM Viewer App is active on the screen, even if it is not operated.

Logout can be forced at any time by selecting the function "Log out at next start" in the settings for the FORUM Viewer App (see page 20). Subsequently, after restarting the app, the old user is logged out and a new user can log in via the login window.

This function is only active once for the next login.

As a rule, error messages contain meaningful information with solutions for the user. Should an error occur that cannot be corrected using these instruc-tions or with the help of this chapter, mark the product as malfunctioning and contact ZEISS Service.

Malfunctions

Malfunction Cause/procedure

No connection can be established with the FORUM server.

Ensure that a network connection between the iPad and the FORUM server exists.

Authentication fails. Ensure that the combination of user name and password is valid for the FORUM server you are connecting to.

Also ensure that a FORUM Tablet Interface License is installed on the FORUM server and a free user license is available.

Despite successful user login, the FORUM Viewer App reports an authentication or license error.

Always ensure that a free FORUM user license is available on the FORUM server when using the FORUM Viewer App.

Furthermore, take particular care to ensure that authentication information (such as the password) is not changed during the use of the FORUM Viewer App.

No patients are displayed. Review the search criteria entered. If necessary, check if patients corresponding to the entered search criteria actually exist on the FORUM server.

The button <Preview> is not active.

Ensure that you selected a device for at least one visit.

The button <Display> is not active.

Ensure that you selected at least one document.

Patient list and search field are not displayed in portrait position.

Push the button <Patients> to display the patient list and the search field.

In the settings for the FORUM Viewer App no hostname or IP address was entered for the FORUM server.

Enter the correct hostname and/or the correct IP address for the FORUM server in the settings for the FORUM Viewer App.

Network communica-tion error! Please check your settings and try again.

In the settings for the FORUM Viewer App an incorrect hostname or an incorrect IP address was entered for the FORUM server.

Enter the correct hostname or the cor-rect IP address for the FORUM server in the settings for the FORUM Viewer App.

Authentication failed or no license available.

The user name does not exist or the password that has been entered is wrong.

Please check capitalization.

No FORUM Tablet Inter-face License has been installed or the number of users who are allowed ac-cess to the FORUM server has been exceeded.

Check the installed licenses; if necessary, wait until a user logs off.

The requested docu-ment was not found.

The requested document is not available on the FORUM server or is not available any more.

Update the data to ensure that only actu-ally existing docu-ments are displayed.

Release Notes for FORUM Viewer AppSoftware Version 2.0

Please read the release notes before using the FORUM Viewer App.The release notes apply only to the use of FORUM Viewer App Version 2.0:

No remaining issues have been found in the FORUM Viewer App that affect the security and effectiveness of the application.

A risk assessment was performed to identify potential risks of the FORUM Viewer App. The risks were evaluated and minimized as far as possible to ensure the security and effectiveness of the FORUM Viewer App. The follow-ing list of risks will serve as an overview for the user, along with the Instruc-tions for Use:

• Where possible, the use of the correct hardware and iOS version is ensured by using the Apple App Store rules.

• Malware attacks can lead to unexpected FORUM Viewer App behavior.

• The FORUM Viewer App ensures that all relevant data are visible and are displayed for the user correctly and with the highest quality possible. The FORUM Viewer App also ensures that the user recognizes which data are available by using visual indicators. If overlapping images obscure parts of an image, zoom and movement functions ensure that the user can see all parts of the image.

• To minimize the risk of mixing up patients, only one patient can ever be chosen at one time. Furthermore, the patient name of the currently selected patient is displayed in all relevant views.

• The FORUM Viewer App is not intended for viewing radiological images.

• When filtering the patient list, the FORUM Viewer App displays the filter criteria, so the user can recognize that data may not be displayed.

• Data displayed in demo mode are clearly marked as such to avoid being confused with real data.

• The FORUM Viewer App complements FORUM. Hence, to connect the FORUM Viewer App with FORUM, a valid license for FORUM is required.

• To ensure that the user can operate the FORUM Viewer App correctly, the Instructions for Use contain all necessary information. These Instructions for Use are available for download and can be requested as a hardcopy free of charge.

• The FORUM Viewer App is not intended for medical emergencies.
