    REQUEST FOR VOLUNTARY LEAVE DONATIONS02/19/2013

    1. Under the voluntary Leave Transfer Program, employees with emergency medical situations thatare anticipated to result in a loss of pay for three work days may request voluntary leavedonations. Due to the nature of the situation, the following employees fall under the provision ofthe Family Medical Leave Act. Any donations will help pay back any advanced sick leave as well

    as any leave without pay that may occur. The following employees needs your help:

    1) Evelyn Sandoval-Villalobos. Account # FY 12, Ms. Evelyn Sandoval-Villalobos isrequiring absence from work due to a medical emergency.

    2) Dena Travis. Account # FY 12, Ms. Dena L. Travis is requiring absence from work dueto a medical emergency.

    3) Elizabeth Tucker. Account # FY 13, Ms. Elizabeth S. Tucker is requiring absence fromwork due to a medical emergency.

    4) Susan Alderman-Schaefer. Account # FY 13, Ms. Susan Alderman-Schaefer isrequiring absence from work due to a medical emergency.

    2. Employees wishing to donate annual leave to the above account should submit their requests onOPM 630-A (OPM 630-B if outside agency) to the Directorate of Resource Management,Building 237, Room 110.

    3. Employees with use or lose annual leave may wish to consider donation of leave as a means ofexhausting excess leave. All donations may be made in whole hours only. The maximum amountsof annual leave that may be donated during the leave year are as follows:
