Registration Form Completed registration form, faxed, mailed or e-mailed to FMM Institute would be deemed as confirmed. There will be no refund or

cancellation within 2 days prior to the programme, 50% refund for cancellation between 3 – 6 days and full refund for cancellation 7 days prior

to the programme. However, replacement can be accepted at no additional cost. The FMM Institute reserves the right to cancel or re-schedule

Assessment

Participants will be assessed on all subjects/ modules via an assignment and a written examination. The written examination will be conducted at

the end of the course. The assessment breakdown is as follows :

Written Examination - 75 %

Assignment - 25 %.

Mr. Steven Khong is a certified motivational speaker and self -development coach in the MLM and MNC fraternity since 1993. He has conducted a lot of seminar courses for many entrepreneurs, managers and leaders in the industry. Trainer Mr. Lim Kim Yook, who has published his first book on 5S: “5S: Asas Pengurusan Untuk Semua”, has more than 12 years vast experience in Quality, Engineering and Production areas. Majoring in Mechanical Engineering and Electrical Engineering, he has acquired strong knowledge and hands-on experience, mainly in the Quality Assurance and Engineering departments. He has presented enormous training locally especially in the northern region in 5S Activities and Implementations, QC 7 Tools, Failure Mode and Effect Analysis, Poka-Yoke, Statistical Process Control and Supervisory Development Program. With more than 6 years experience at the managerial level in the Quality and Engineering departments, he had lead the Quality Assurance Department for several companies such as Meisei Electric (M) Sdn Bhd, Taiyo Technology (M) Sdn Bhd, and Precico Sdn Bhd. and Precico M&D Sdn Bhd. in implementing quality control and quality assurance systems for continuous improvement and also for achieving and maintaining the ISO 9000 Quality Management System. He was the key personnel who had managed to develop ISO/TS 16949 Quality Management System for Precico Sdn Bhd. Apart from his major role in Quality Assusrance Department, he had provided consultations on quality systems to many key departments. Nevertheless, he had designed numerous Poka-Yoke devices for machines and products during his service in Toshiba Electronics (M) Sdn Bhd. and Meisei Electric (M) Sdn Bhd. He was an Assistant Engineering Manager at Armstrong Cycle parts Sdn Bhd in which he led the Engineering, Product Development and Maintenance sections. He had involved and participated in the development of automotive control cables for Proton, Perodua and Honda cars such as Waja 1.8, Kelisa, Kenari, Kembara, and Civic 1.7. He had set-up the Reliability Test Section and designated the Speedometer Cable Endurance Tested and Gear Shift Endurance Tester (Manual and Automatic Transmissions) for Armstrong Cycle Parts Sdn Bhd.In addition, with his wide experience in semiconductor, electronics, and automotive industries, Mr Lim is an active consultant, trainer and analyst in the area of 5S and Quality Management System. Most of his analysis have made several companies grew with solid foundation in quality and 5S. Ms. Cheryl Neoh has been working in Supply Chain Management field for more than 12 years with exposure in semiconductors industry, fibre optics industry at Multi-National Company, hands-on experience in setting up whole Materials Department in SMT (PCBA / OEM) industry, Sales & Marketing and in entrepreneurial skills. Throughout this 10 over years, she has developed solid leadership and management skills, motivating and bringing up successor. Her involvement in the management level of manufacturing industry throughout these years enables her to unleash effective skills in coaching, communicating and motivating working partners with great interpersonal skills and influencing power. As the result, she has then successfully developed an effective formula for high performance supervisors and leader-manager. Cheryl Neoh has been involved in training for many years for both in-house training programs as well as public, structural and certification courses. Mr. Selvaraj is an avid public speaker and a Competent Communicator with Toastmasters International. The latest award that he has won is the Toastmasters Humorous Speech Contest Division N Champion – 2012.
