ARRANGE WITH THE VARIOUS UTILITY COMPANIES FOR THE EXACT LOCATION OF ALL EXISTING UTILITIES AND ANY NECESSARY ADJUSTING OF THE OVERHEAD OR UNDERGROUND UTILITIES WITHIN THE LIMITS OF CONSTRUCTION PRIOR TO START OF CONSTRUCTION. UTILITY RELOCATION IS TO BE PERFORMED AT THE CONTRACTOR’S EXPENSE.

    1. CONTRACTOR SHALL PREPARE AND SUBMIT A STORMWATER POLLUTION PREVENTION PLAN (SWPPP) TO IEPA IF REQUIRED,

SOIL EROSION CONTROL MEASURES SHALL BE IN ACCORDANCE WITH THE ’PROCEDURES AND STANDARDS FOR URBAN SOIL EROSION AND SEDIMENTATION CONTROL IN ILLINOIS” AND AS DIRECTED BY THE DISTRICT/ENGINEER.

    2.

THE LOCATION. SIZE AND DEPTH OF THE SEWERS AND/OR VARIOUS UTILITIES, AS SHOWN ON THE PLANS, ARE FROM THE LATEST AND BEST AVAILABLE INFORMATION TO THE ENGINEER. IT SHALL BE THE CONTRACTOR’S RESPONSIBILITY TO HAVE UTILITIES LOCATED.

    2.

    3. ALL STREETS ADJACENT TO THE WORK SITE SHALL BE KEPT FREE OF DIRT, MUD, AND DEBRIS.

    3. LIMIT CONSTRUCTION ACTIVITIES AND STORAGE OF EQUIPMENT OR MATERIALS TO THE AREAS SHOWN ON PLAN. ANY DAMAGE TO PRIVATE PROPERTY OR FENCES SHALL BE REPLACED IN KIND OR REPAIRED AT THE CONTRACTOR’S EXPENSE.

D D 4. NO SEDIMENT WILL BE ALLOWED TO ENTER ADJACENT DITCHES OR STORM SEWERS.

IN ACCORDANCE WITH THE CONSTRUCTION DRAWINGS, TEMPORARY SILT FENCING SHALL BE INSTALLED AND MAINTAINED AS INDICATED ALONG PROPERTY LINES, SWALE AREAS, STOCKPILES AND STORM STRUCTURES UNTIL VEGETATION IS ESTABLISHED AND/OR CONSTRUCTION IS COMPLETED.

    4. WHERE SECTION OR SUB-SECTION MONUMENTS OR BENCH MARKS ARE ENCOUNTERED, NOTIFY THE ENGINEER BEFORE SUCH MONUMENTS ARE REMOVED. THE CONTRACTOR SHALL PROTECT AND CAREFULLY PRESERVE ALL PROPERTY IRON PIPES AND MONUMENTS UNTIL AN AUTHORIZED SURVEYOR OR AGENT HAS WITNESSED OR OTHERWISE REFERENCED THEIR LOCATION.

    5. PROVIDE AND INSTALL TWO WEIGHTED SAND BAGS ON EACH TYPE I OR TYPE II BARRICADE USED. ONE WEIGHTED SAND BAG SHALL BE USED ACROSS EACH BOTTOM RAIL.

    6. NO CONCRETE SHALL BE PLACED UNTIL ALL THE FORMS HAVE BEEN INSPECTED FOR LINE, GRADE AND SUBGRADE CONDITIONS BY THE RESIDENT ENGINEER. INSPECTIONS SHALL BE ARRANGED AT LEAST TWENTY-FOUR HOURS IN ADVANCE OF ANY CONCRETE PLACEMENT.

    LIFT STATION

    1. MAINTENANCE OF OPERATION, CONTRACTOR IS RESPONSIBLE FOR ALL TEMPORARY PUMPING TO CONVEY WASTEWATER FROM THE COLLECTION SYSTEM DURING CONSTRUCTION.7. RECYCLED ASPHALT PAVEMENT MATERIALS WILL NOT BE PERMITTED FOR HOT-MIX ASPHALT SURFACE COURSE CONSTRUCTION.

    2. ALL LIFT STATION SHUT DOWNS NEED PRIOR PLAN APPROVAL.8. SILT FENCE SHALL BE INSTALLED AT LOCATIONS SHOWN IN THE PLANS OR AS DIRECTED BY THE ENGINEER. 3. TESTING OF THE NEW LIFT STATION WILL BE REQUIRED PRIOR TO ANY DEMOLITION

OF THE EXISTING LIFT STATIONS. CONTRACTOR WILL BE RESPONSIBLE FOR SUBMITTING A TESTING PLAN AND EXACT SEQUENCE OF CONSTRUCTION FOR APPROVAL.

    4. THE LIFT STATION WET WELL SHALL BE CONSTRUCTED OF PRECAST SECTIONS CONFORMING TO THE CONSTRUCTION DRAWINGS. ALL JOINTS BETWEEN SECTIONS SHALL BE SECURELY SEALED TO EACH OTHER USING A RESILIENT, FLEXIBLE, NON-HARDENING, PREFORMED BUTYL RUBBER SEALANT (EZ-STIK OR APPROVED EQUAL). HYDROCEMENT SHALL BE APPLIED TO THE INTERIOR OF ALL JOINTS. PROVIDE AN INTERNAL CHIMNEY SEAL.

    SANITARY NOTES

ANY OTHER TREES INJURED OR TO WHICH POTENTIAL IRREPARABLE DAMAGE MAY ENSUE AS DETERMINED BY THE ENGINEER. THE CONTRACTOR SHALL BE HELD RESPONSIBLE FOR THE COST OF THE TREE REPLACEMENT.

    2. 5. THE LIFT STATION WET WELL EXCAVATION SHALL BE SHEETED AS REQUIRED. SHEETING SHALL BE APPROVED BY A REGISTERED PROFESSIONAL STRUCTURAL ENGINEER OF THE STATE OF ILLINOIS. THE SHEETING (STEEL OR WOOD AT CONTRACTOR’S DISCRETION) SHALL BE DRIVEN FROM THIS LOCATION. ALL AREAS BETWEEN THE SHEETING AND THE PRECAST WET WELL SECTION SHALL BE BACKFILLED WITH A COMPACTED GRANULAR MATERIAL (CA-7).
