• FISS is the designated claims processing system used by Medicare Contractors who process Part A claims.

• DDE is the external application developed for providers claims submission and inquiries.

FISS – How to Get Connected

• To access the Direct Data Entry, Fiscal Intermediary Shared Systems (FISS) and submit claims electronically to CMS the provider must complete an application to use CMS Electronic Data Interchange services and choose an approved connectivity vendor.

• You may also utilize PC-ACE, the free claims processing software offered by CMS specific to your MAC. Free claims transmission software is also available.

FISS – How to Get Connected

• Connectivity choices vary. Some providers choose to use a billing clearinghouse to handle claims transmission and connectivity.

• Connectivity to transmit claims DOES NOT give the provider access to DDE.

• You must have software to connect to the DDE and submit the appropriate forms required to obtain log on ID and passwords.

FISS – How to Get Connected

• Follow instructions from your MAC to request User IDs

• Set-up and maintain individual passwords. You will also receive a RACF ID. This can be used to reset your password electronically.

• Providers are not permitted to share their RACF ID with ANYONE!

• To gain access to FISS the individual must have their own RACF ID

IDs

• Most MACs require providers to certify user access annually. If this recertification information is not verified and returned, access will be terminated.

• DDE User IDs will be revoked and deleted from the system after 30 days of being inactive.

• DDE IDs cannot be re-activated once this has occurred. Notification will not be sent out prior to deletion.

• This issue can be avoided by signing on to DDE at least once every 29 days so that your ID remains active.

Changing Passwords

• Password will expire every thirty days.

• On the day after it expires, when you type your password, the system will automatically prompt you to change your password.

• Rules for passwords will usually display on the system when you change your password.

• When entering information remember to <TAB> among the fields until you have completed the screen.

• To move on to the next screen/page, press <F8>.

• Depending on the Type of Bill, the cursor will skip fields that are not required.

• If you press <F3> while you are in the middle of data entering your claim before you have ‘stored’ the claim, you will lose all the information you have keyed.

• If, at any time, you press <F4>, you will be bumped totally off the system, and you must sign back on.

• It is recommended that you use FISS to check your billing transactions at least once a week.

• When you sign on to FISS, you should:– Check Option 56 within the Inquiry Menu to see a quick

summary of claims that are processing in FISS.

– Correct any claims that are in your Return to Provider (RTP) file (T status).

– Check for Additional Development Requests (ADRs).

– Research any claims that have appeared on your Remittance Advice as noncovered or denied.

Claim Inquiry - Option 56

• Option 56 gives you a view of your claims currently processing in FISS. It shows you the number of claims and charges submitted by status location and by type of bill. It will not show you which specific claims reside in the status/location.

• You will not see claims that have completed processing (s/loc P B9997, R B9997, & D B9997). You will see claims that have been approved for payment (P B9996) because they have not completed processing.

Status Locations

• The Status/Location (S/LOC) code for Medicare DDE screens indicates whether a particular claim is paid, suspended, rejected, returned for correction, etc.

• The six-character alphanumeric code is made up of a combination of four sub-codes:

– the claim status,

– processing type,

– location, additional location information.

• Each S/LOC code is made up of two alpha characters followed by four numeric characters.

• The second position (position b) is the claim processing type. In this example, “B” indicates batch.

• The third and fourth positions (positions cc) are the location of the claim in FISS. In the example, “99” indicates that the session terminated.

• The last two positions (positions dd) are for additional location information. In the example, “97” indicates that the provider’s claim is final on-line.

• Key the added revenue code line after the 0001 line. You don’t need to rekey the entire revenue code page.

• Be sure to correct the total charge amount to reflect the addition of the revenue code charge.

• Press Home so that the cursor is placed in the upper right corner of the screen (the Claim Page field).

Cancel Claims

• Cancels (voids) can be processed electronically. A bill can only be canceled if it has finalized and is reflected on the remittance advice.

• A cancel claim is sometimes followed with a new corrected bill when an adjustment was not appropriate.

• Providers must be very careful when creating cancels. Once the cancel is created and the F9 key pressed, there is no stopping the cancel. Errors will cause payment to be taken back.

• Claims that are in status/location P O9998 are offline. This means that only summary information is available on the online Claims Summary screen and the detail information is stored in offline tape storage.

• Providers may only submit adjustment/cancel requests via DDE for claims that are online in status/location P B9997. Once the claims are in an online status, providers may submit adjustment/cancel requests via DDE.

Retrieving Offline Claims

• Providers must perform adjustment/cancel requests in DDE within four to six weeks after the status has changed from P O9998 to P B9997.

• If adjustments/cancels are not completed timely, the claim may be returned to offline status. If this occurs, the provider must then retrieve the claim again.

FISS Shortcuts

þ Use your arrow keys and/or TAB key to move between fields. Do not use your ENTER

key. Using the TAB key is preferred, as your arrow keys may not place your cursor in the

correct field position.

þ To move back one data field at a time, press and hold the SHIFT key and then press TAB.

þ To quickly move between claim pages, press your HOME key on your keyboard, which

takes your cursor to the ‘Page’ field. Type the number of the page to which you want to

move, and then press Enter. In FISS, the claim consists of 6 pages. However, two

additional pages, page 7 and page 8, are available for claims in Additional Development

Request (ADR) (status/location S B6001).

þ If you attempt to type in an invalid field, your keyboard will lock, and you will see this

icon in the lower left hand corner of your screen. To “unlock” your

keyboard, try to press the ESC key or the left Ctrl key. The method

used to unlock your keyboard depends on your keyboard set up. You may need to contact

your connectivity vendor for assistance. Once you have unlocked your keyboard, you must

press the TAB key to move your cursor into a valid field.

þ Use the ‘SC’ (Screen Control) field located in the upper left corner (under the ‘Page’ field)

of the FISS screen as a shortcut to information within the Inquiry menu. To access this

field, press the HOME key and then the TAB key. To quickly move to one of the

ADJUST OR CANCEL????…..that is the question

Adjustments

You submit adjustments when information on a processed claim needs to be changed. You may need to submit an adjustment to:

• Change information already recorded on the claim • Add new information • Remove information that was billed incorrectly

Cancellations

You may need to enter a cancellation on a processed claim to:

• Cancel a claim with incorrect information and process a new claim with corrected information

• Cancel a duplicate claim that was entered in error

• Cancel a claim that should not have been submitted

Adjustments and Cancellations affect your payments

Adjustments and cancellations result in either a payment or recoupment and Medicare will initiate a action against the original claim initiate a debit and/or credit transaction and the original claim payment will be recouped by Medicare. For adjustments, payment for the original claim will be recouped. You will receive a payment for the adjusted claim once it finalizes. Carefully review Medicare Remittance Advice for the details of the adjusted or canceled clam..

Limitations: Inappropriate Adjustments and Cancellations

Prior to submitting an adjustment or cancellation, the claim must be finalized in FISS. Cancellations can be made to claims in FISS S/LOC code P B9997. Claim adjustments can be made to paid or rejected claims (status/location code P B9997 or R B9997). If the claim is still processing, wait until the claim finalizes. It is not appropriate to adjust a rejected claim in the following situations.

• Eligibility (entitlement date or date of death)

• HIC number change

• Duplicate

• Untimely claims (past timely filing deadline)

Claims cannot be electronically adjusted if they have been through the medical review process and a partial or full denial was issued. If there is a medically denied line item on a claim and a correction needs to be made to a nonmedically denied line, you would send a hardcopy adjustment for this correction.

If a medically denied line was billed in error, a hardcopy adjustment must also be sent to cancel the claim. Be sure to indicate the reason why you need to cancel the claim and include the correct TOB, XX8 and the appropriate claim change reason code

Claims Correction Window

To enter adjustments or cancellations, enter 03 in the Enter Menu Selection field on the FISS Main Menu screen. Press Enter.

Select Provider Type Find the "Claim Adjustments" or the "Cancels" code that matches your provider type. Type the code in the "Enter Menu Selection" field, Press Enter.

Claims Summary Inquiry Screen

The S/LOC (Status/Location) field in FISS identifies the status and location of your claim while it is processing.

This field defaults to P, in order to adjust or cancel paid claims.

To adjust a rejected claim, change the P to R. The TOB (Type of Bill) opens with the default for your provider type. You will need to change this if the claim you need to adjust or cancel is a different type of bill.

Viewing Claims by Beneficiary To find a claim for a beneficiary, press the Shift +Tab keys to move your cursor to the NPI field. Enter your NPI. Tab to the HIC field and enter the beneficiary’s HIC number, you may need to change the TOB field. depending on the type of claim you are adjusting.

Viewing Individual Transactions The system displays paid or rejected claims for the beneficiary.Up to five transactions can be viewed per page. Each has a two-line summary. Use your F5 and F6 keys to scroll through all the claims. Tab to the one you want to adjust, type "S" in the SEL field, press Enter.

Notice the S/LOC field changes to S B0100 because the claim is now suspended. The DCN (Document Control Number) of the original claim is also automatically inserted by FISS into the adjustment or cancellation claim.

Enter Changes on Other Screens

You will need to update information on other claim pages (to adjust or remove information) when you complete an adjustment. Move through the claim pages using the F7 and F8 keys. REMEMBER: If you used Claim Change Reason Code D9 or D6 and Adjustment Reason Code OT you must enter

the appropriate REMARKS on screen 04. Some MACs require SPECIFIC language on this screen descriptive of the

changes you have made to the claim.

Saving Your Changes When you are done entering changes, and the appropriate codes press the F9 key to save them and submit the claim for processing. If the change is successful, you should go back to the Map 1741 screen (claim summary). If not, there are additional errors on the claim. Press F1 to see what you need to correct

Viewing Claims The original paid transaction will remain in FISS. Once the cancellation or adjustment has processed (P B9997), you will see the original bill and the changed bill on the Claim Summary Inquiry screen. The original claim will have a date in the CAN DT field. The adjusted claim will reflect a TOB 217, cancelled claim will reflect TOB 218

THINGS TO REMEMBER ABOUT ADJUSTMENT AND CANCEL CLAIMS

Enter 16 in SC field to get help with Adjustment Reason Codes.

Performing an adjustment or cancellation will impact your claim payment.

FISS will initiate a debit and/or credit transaction and the original claim payment will be recouped by Medicare.

For adjustments, the new claim amount will be paid back to you once the adjusted claim finalizes.

When you make an adjustment to a bill, the Type of Bill (TOB) field will change and the third digit will be 7 (e.g. XX7). For cancellations, it will be 8.

Before you can adjust or cancel a claim, it must be processed. The correct S/LOC code to indicate this in FISS is P B9997 or R B9997
