Top 100 in the U.S.

For the first time, Community Home Health was named in the top 100 HomeCare Elite ranking last year. The publicly-reported HomeCare Compare statistics measured patient outcomes of the nation’s 9,000 Medicare-certified home health agencies. Ranking in the top 100 means that we produced better patient outcomes than 99% of the other agencies. We were very pleased to be able to reduce our acute care hospitalization rate by 3% and our emergent care use by 5%. Continuing education for our staff remains a high priority. What does this mean to our patients? It means high quality care resulting in fewer return visits to the hospital and peace of mind for families. To better serve the community, we continue to improve patient outcomes and exceed regulatory requirements for staff education and training.

Advanced healthcare technology

Healing at home is often key to a successful recovery for individuals convalescing from surgery or suffering from debilitating illnesses. Our new telemonitoring program helps people heal at home while reducing unnecessary doctor and emergency room visits. We currently have 35 telemonitors placed in patients’ homes. The size of a small answering machine, the telemonitor prompts the patient to take vital health signs each day. The information is sent via a secured telephone line that is monitored by a specially-trained registered nurse who assesses the patient’s condition and determines a course of care. This might include calling the patient, contacting their physician to coordinate follow-up care, or making a home visit.

Community Home Health & Hospice is a nonprofit community-based healthcare agency caring for Washington and Oregon families. Our services have been an integral part of our community since 1977. In addition to the outstanding patient care that we have given, our agency continues to find new ways to contribute and be involved. Our ongoing commitment is to the well-being of every patient, family member, staff person, volunteer, and loved one who comes in contact with our agency. We are pleased to provide this report to show the many ways in which we make our mission real, and how we give back to our generous supporters. We are humbled and proud to be of service to you!

Bereavement program expands

For teenagers, losing a loved one makes them different during the years when all they want is to fit in. Until we started a teen bereavement support group in 2008, there was no place they could go to find support from other grieving teens. Last year we also developed an adult bereavement program in Vancouver, adding a fulltime bereavement coordinator.

All bereavement services are free and available to anyone in the community, not just hospice families. Forty-eight volunteers contributed more than 2,000 hours in 2008 to assist with bereavement programs including Camp Cedars, our free three-day summer camp for youth. Some of the children plan to return when they are older to train as volunteers and help other grieving children through a difficult time of life.

Prepared for the future

Community Day Care opened last year to recruit and retain critical staff needed to care for the community. Children of employees as well as community families thrive in a stimulating environment that encourages creative and developmental growth appropriate to each child’s individual needs. Staff members receive a discount on day care costs. From babies to toddlers to preschoolers and children arriving after school, Community Day Care is busy and near capacity on most days. All day care staff receive ongoing early childhood education.

Wound care and technology

On any given day, approximately 144 patients need wound care at home. Before we started a wound care program last year, it was difficult to travel to each patient. On a daily basis, about 16 home health field nurses visit wound care patients. In 2008, we hired a wound care specialist who implemented advanced wound care products, camera cell phones, and ongoing staff and community education. The results are improved patient care which often includes faster healing, reduced risk and better cost management.

Camera cell phones allow our nurses to photograph patients’ wounds at home and then transmit them to our wound care specialist for analysis. The wound care specialist immediately reviews how the wound is healing and updates the plan of care, collaborating with other specialists and the patient’s physician.

Hospice means comfort

Last year we cared for 841 hospice patients. Our goal is to comfort and care for people so that their final weeks or months are spent with dignity and quality, surrounded by loved ones. While most patients receive hospice care in their home, we also provide care in our Hospice Care Center and in assisted living, nursing homes and other facilities.

We encourage evaluations earlier rather than in the last days of life so that patients and their families experience the full benefits of hospice services, which include emotional and spiritual support unique to each family. No eligible hospice patient is turned away for lack of ability to pay.

Independent at home

As more people in their golden years enjoy an independent lifestyle at home, our Community Home Care program provides assistance with everyday activities. Clients choose how much—or how little assistance they need, ranging from an hour to 24/7.

A number of caregivers have 10 years or more of service which means stable and reliable care for clients. In 2008 we invested in new training programs for staff, including helping home care aides continue their education in the medical field. Several long-term employees became registered nurses and now work in home health or hospice.

Fifty Hospice Thrift Shop volunteers have found a meaningful way to make a difference in our community. We often share donated items with other nonprofit agencies.
