Smaller goals that will help your clinic succeed in transitioning include increased asthma care, increased immunizations, increased well-child visits, increased behavioral health resources, decreased ED visits and decreased hospitalizations. When you enroll in P-TCPI, you will be assigned a practice facilitator based in your region. Your practice facilitator will contact you via email to set up your clinic’s first appointment. Within 24hrs of your first visit, your practice facilitator will: Email you a copy of your completed PAT with their notes on the visit. Start processing the paperwork for your incentive payment. During your first appointment, you practice facilitator will: Answer all of your questions about the initiative. Administer a Practice Assessment Tool (PAT). Things to KnowThe overall goal of P-TCPI is to get your clinic ready for a value-based payment system. Smaller goals that will help your clinic succeed in transitioning include increased asthma care, increased immunizations, increased well-child visits, increased behavioral health resources, decreased ED visits and decreased hospitalizations.

When you enroll in P-TCPI, you will be assigned a practice facilitator based in your region. Your practice facilitator will contact you via email to set up your clinic’s first appointment.

Frequently Asked Questions:Does P-TCPI cost money?

No. P-TCPI offers incentive payments for completing PATs and signing memorandums of agreement (MOAs) with specialists. Your practice facilitator can also assist you with programs, such as Health Home Coordinated Care, that can help your practice increase reimbursement rates.

What do you mean when you say “incentive payment”?

Every 6 months you are required to complete a PAT and you will receive an incentive payment based on the number of clinicians who work within your clinic.

How many of our clinic staff should attend the first meeting?

It is helpful to have one MD, one ARNP, one MA, and one receptionist in the meeting so that everyone can offer their unique perspectives.

What if I don’t have time for more change?

You are welcome to move as fast or as slow as you want through the phases of transformation. The only requirement is that you complete a PAT every six months.

Things to KnowWhen value-based payments become a reality, your data will tell the story that determines how you are paid. Your practice facilitator can help you understand who is on your patient panel as well as connect you with your Molina data reports.

The following can help you collect data:• Your Electronic Health Record Provider can run reports for you.• You can access your clinic’s immunization reports at CDC’s Immunization Information Systems (IIS)

webpage (https://www.cdc.gov/vaccines/programs/iis/index.html)• Your practice facilitator can help you through this process.

Things to KnowYour Aim Statement should answer: Where are we going and how will we get there? What are three areas that you want to improve?

Prior to Setting Your Aims• Set up an appointment to meet in person with your practice facilitator.• Check out this training from AMA, Quality Improvement Using Plan-Do-Study-Act:

• One goal should be a short-term goal, one should be longer (6-12 months), and one should be a long-range plan.

Things to KnowValue-based payment is coming. By 2019, many clinicians will be reimbursed based on what their HEDIS measures say. Value-based payment will replace per member per month payments for many clinicians.

How can P-TCPI help you get ready for value-based payment?• Provide you with quarterly HEDIS utilization reports so that you know where you need to improve.• Help you with empanelment so that you receive credit for patients who actually utilize your clinic.• Help you start work flows so that you can increase your immunization and well-child visit rates.
