1. Absence de complications
2. Ostéointégration
3. Intégration biologique et esthétique
4. Satisfaction du patient
4
![Page 5: FINAL REPORT NYU GILLES ABITBOL - JUIN 2015](https://reader034.fdocuments.in/reader034/viewer/2022051520/579057f31a28ab900c9f5c8b/html5/thumbnails/5.jpg)
1. Avant la phase chirurgicale (plan de
traitement)
2. Pendant la phase chirurgicale
3. Après la phase chirurgicale (pendant
la phase de cicatrisation)
Pendant la phase prothétique (non traité
ici)
Two kinds of failures:
During the surgical step
1. During preparation of the
treatment plan
2. During surgery
3. During healing
during the prosthesis step (not
treated here)
5
![Page 6: FINAL REPORT NYU GILLES ABITBOL - JUIN 2015](https://reader034.fdocuments.in/reader034/viewer/2022051520/579057f31a28ab900c9f5c8b/html5/thumbnails/6.jpg)
Causes d’échecs pendant la
phase pré-chirurgicale (relatifs et
absolus)
Causes of failures during the pre-
surgery phase (relative and
complete)

Causes d’échecs absolus
pendant
la phase chirurgicale
• Accidents anatomiques majeure;
atteinte d’organes anatomiques
• Auto blessure suivie d’hémorragie
• Forage excessif ou insuffisant
• Fracture de l’implant
• Fracture de la mâchoire

• Comportement du patient
• Major anatomic accidents; anatomic
organs damage
• Self-injury followed by hemorrhage
• Excessive or insufficient drilling
• Implant fracture
• Jaw fracture
• Material issue
• Bad positioning of implant
• Patient’s discomfort
• Patient’s Behavior
Causes of complete failures
during
surgical phase

• Sutures
Absolus (perte ou retrait)
• Pas d’ostéointégration simple, avec
explantation spontanée
• Anesthésie persistante définitive
• Infection avec mobilisation de l’implant
• Ostéite et Perte osseuse irréversible, retrait
tardif
• Hémorragie excessive et durable
• Contrindication médicale non renseignée
• Contrindication psychologique non renseignée
Relative, that doesn’t lead to implant loss
• Intense pain - painkillers ineffective
• Persisting anesthesia but reversible
• Giant hematoma or/and edema
• Hemorrhage
• problem of surgical suture
Absolute (loss or removal)
• No osseointegration, with spontaneous
explantation
• Definitive persisting anesthesia

Conclusion
14
![Page 15: FINAL REPORT NYU GILLES ABITBOL - JUIN 2015](https://reader034.fdocuments.in/reader034/viewer/2022051520/579057f31a28ab900c9f5c8b/html5/thumbnails/15.jpg)
1. L’échec est indispensable pour progresser
2. L’échec en implantologie a remplacé l’échec en
prothèse amovible

For years we implantedwithout asking any question
o We were too happy to get rid of the removable
prosthesis that made unhappy patients, devalued
us making us aware of our powerlessness
o We were too happy especially in France to have a
new source of growth in dental offices crushed by
an obsolete nomenclature and absurd and
unrealistic controls our fees
o We are too happy to escape adviser dentists that
do not understand implantology
o We are too happy to develop a craft that purred

➡I exaggerated
➡Implantology is
necessary in general
practice but an implant
is not better than a tooth
➡That periodontology not
only helps to implant in
the best conditions, but is
primarily used to keep
the teeth
➡Implantology should not
disappear periodontists
and endodontists73

Ma pratique quotidienne - moins d’implantMy daily practice led me to put less implant for 4 years
1. Dispositif compliqué à
déposer et à rattraper .
2. L’esthétique compliquée /
pontique
3. La perte de proprioception
4. L’économie d’une prise de
conscience
5. Les mucosites et les péri-
implantites
6. Greffes /bridges
1. The implant is a complicated device to drop when osseointegrated. Difficult to rectify when it is in a wrong position.
2. The aesthetic is more complicated to manage than a bridge
3. The loss of proprioception by the proliferation of Implants can cause big problems for the mandibular joint. and even throughout the skeleton
4. The implant can give the impression that patient can neglect her dental hygiene
5. Mucositis and periimplantitis are too hard to manage, are resented by the patient; the dentist tends to zap in case of absence of clinical signs
6. Grafts versus bridges
74
![Page 75: FINAL REPORT NYU GILLES ABITBOL - JUIN 2015](https://reader034.fdocuments.in/reader034/viewer/2022051520/579057f31a28ab900c9f5c8b/html5/thumbnails/75.jpg)
7. L’échec trop chronophage
8. L’échec est anxiogène
9. L’échec coute cher
10. L’échec coute cher en patients
11. peu d’échecs de bridges
12. Les collages ont fait des
progrès phénoménaux
7. Failure is too time-consuming; the loss of time up to one year from the time the first time implanting and installation of the final crown after 2nd poses; more with a sinus,
8. Failure is very anxiety provoking for the patient and for us, whether or not predictable; the patient puts his body in question, he believes that his body rejects progress of science, he gives the dentist in question, it puts the technology in question, he loses confidence in all. And the practitioner is helpless.
9. The failure costs too much especially if you have improperly installed the indication; it may even be necessary to change the treatment plan.
10. you lose a lot of patients because they speak a lot about you, Sometimes we lose the patient himself,
11. there are not a lot of bridge failures
12. bounding, phenomenal progress

Conclusion inspirée du DR MARC BERT, op.cit.:

![Page 78: FINAL REPORT NYU GILLES ABITBOL - JUIN 2015](https://reader034.fdocuments.in/reader034/viewer/2022051520/579057f31a28ab900c9f5c8b/html5/thumbnails/78.jpg)
