2001
History of the Fifth Grade Health Survey (5GHS)
5GHS was developed in 1998 by the Maternal and Child Health Service of the Oklahoma State Department of Health.
Prior to 1998, little was known about the health of young Oklahomans. We had no clear way to measure and address health care availability and the general health of young children in Oklahoma.
The pilot survey was done in 1998 and then it was conducted again in 2001.
5GHS measures multiple aspects of health care:
General health of child Dental health Physical activity and obesity Tobacco smoke Access and use of health care services Barriers to care Health insurance Emotional and mental health
Procedures We obtain a list of all public school fifth grade
classrooms in Oklahoma from the Department of Education.
The Superintendent, of those randomly selected classrooms, is notified which classrooms in their school was chosen and they decide whether or not to participate.
If participating, the school sends a flyer home with the child giving the parents some information about the survey and letting them know to expect it in a few weeks.
Procedures
Then the school sends the surveys home with the students and the parents complete the survey, returning it to the teacher, via the student, in the provided manila envelope.
Each student who returns the questionnaire receives some kind of small incentive, such as a story book.
The teacher sends the surveys back to the OSDH for us to compile.

It contains 71 multiple-choice questions
The questions are presented in a straightforward and sensitive manner
Students have a period of about 2 weeks to complete the questionnaire and return it to their teacher.
Response Rates for 2001 There were 3,438 students enrolled in the
selected classrooms and 2,146 surveys were returned=62%
There were 278 schools selected and 138 participated=50%
