Reruptures,Reinjuries,andRevisionsataMinimum2-YearFollow-up:ARandomizedClinicalTrialComparing3GraftTypesforACLReconstruction
Objective:Toevaluatethepredictivefactorsfortraumaticrerupture,reinjury,andatraumaticgraftfailureofanteriorcruciateligament(ACL)reconstructionataminimum2-yearfollow-up.
Design:Double-blindrandomizedclinicaltrial(RCT)withintraoperativecomputer-generatedallocation.
Setting:University-basedorthopedicreferralpractice.
Patients:ThreehundredthirtypatientswithisolatedACLdeficiencywereequallyrandomizedto(1)patellartendon(PT;mean,29.2years);(2)quadruple-strandedhamstringtendon(HT;mean,29.0years);and(3)doublebundleusingHT(DB;mean,28.8years).Threehundredtwenty-twopatientscompleted2-yearfollow-up.
Interventions:AnatomicallypositionedprimaryACLreconstructionwithPT,HT,andDBautografts.
MainOutcomeMeasures:Proportionsofcompletetraumaticreruptures,traumaticreinjuries(completererupturesandpartialtears),atraumaticgraftfailures,andcontralateralACLtears.Logisticregressionassessed5apriorideterminedindependentpredictors:chronicity,grafttype,age,sex,andTegnerlevel.
Results:MorecompletetraumaticrerupturesoccurredintheHTandDBgroups:PT=3;HT=7;DB=7(P=0.37).TraumaticreinjuriesstatisticallyfavoredPTreconstructions:(PT=3;HT=12;DB=11;P=0.05).Atraumaticgraftfailureswerenotdifferent:PT=16;HT=17;DB=20(P=0.75).Youngeragewasasignificantpredictorofcompletetraumaticrerupturesandtraumaticreinjuries(P<0.01).Higheractivitylevel,males,andpatientswithHT,DB,andacutereconstructionshadgreateroddsofreinjury.Noneofthesefactorsreachedstatisticalsignificance.ContralateralACLtearswerenotdifferentbetweengroups,buttrendssuggestedthatyoungerfemalesweremorelikelytohaveacontralateralACLtear.
Conclusions:MoretraumaticreinjuriesoccurredwithHTandDBgrafts.Youngeragewasapredictorofcompletetraumaticreruptureandtraumaticreinjury,irrespectiveofgrafttype.

Physicalinactivityisamajorandincreasingcontributortopoorhealthandprematuredeathacrosstheworld.Peoplecanincreasetheirhealth-enhancingphysicalactivity(PA)levelsthroughacombinationofpersonaleffortsandasupportiveandconduciveenvironment.Healthcarehasakeyroletoplaybutneedsmoreknowledge,organisation,andincentivestofulfilitspotential.
Physicalinactivitycausedover5.3milliondeaths,or9%ofprematuremortality,worldwidein2008.1Ifphysicalinactivityweredecreasedby25%,morethan1millionannualdeathscouldbeaverted.ThehealthimprovementofenhancedPAislargelyduetothelowerriskofcardiovasculardisease,metabolicdisease,andcancer.Equallyimportantly,increasedPAalsoimprovesqualityoflifeandhelpstopreventortreatmentalhealthdisorders.Readmore...
