What’s behind the Skin Color? Did you know that different pigments determine the skin color? •  MELANINS (eumelanin and feomelanine) yellow, brown to black; •  HEMOGLOBIN pink and oxygenated; •  CAROTENE yellow – orange nuances.   Under normal conditions, melanins determines the skin color. The melanocyte, a dendritic cell that resides in the basal layer of the epidermis, is the seat of melanin synthesis. Pheomelanin and eumelanin derived from the same initial pathway. The enzyme tyrosinase catalyzes the conversion of tyrosine to DOPA and subsequently into DOPA-quinone.   The synthesis of melanin is related to 2 conditions: 1.  type of racial and constitutional factors (phototype); 2.  exposure to external agents (ultraviolet radiation).  

The Pigmentation Disorders Often a change in the skin color is the manifestation of an ectodermal extracutaneous disorder. This is an unsightly condition of interest, dermatologically characterized by hypopigmentation (leucoderma) or hyperpigmentation. There are different forms of hyperpigmentation: •  The hyperpigmentation caused by accumulation of melanin

(hypermelanosis) or originated from darkening causes (blood, exogenous pigment, carotene);

•  Hypermelanosis congenital: neurofibromatosis, xeroderma pigmentosum and Mongolian spots;

•  Hypermelanosis acquired: secondary to systemic diseases, drugs, UV, post-traumatic results or treatments;

WHAT IS MELASMA? Melasma (Chloasma, mask of pregnancy) is a localized facial hyperpigmentation more common in women, triggered by exposure to sunlight and hormonal changes. CAUSES: genetic predisposition - oral contraceptives BODY PARTS AFFECTED: Forehead – Temples – Cheeks - Upper lip.

How localized hypermelanosis has been cured? What about new treatments? Historically, the most used substance in treatment is hydroquinone, in concentrations ranging from 2% to 4%, or higher in association with sunscreens. The classic formulation of Kligman contained hydroquinone, tretinoin and a corticosteroid with low activity. Concentrations of hydroquinone up to 8% -10% have been created to lighten the skin of black race individuals, but they turned out to be very dangerous for the human health. Also due to its indiscriminate use and to its CARCENOGIC effects, hydroquinone is no longer available and has been banned by the European Union (The 24th Directive 2000/6/EC).
