• In order to request a background check, you must fill out form I-783.

• Please make sure to include your complete mailing address and telephone number and/or e-mail address.

Fingerprints

• After filling out the application form, you must obtain your set of fingerprints.

• A standard fingerprint form (FD-258) will be provided by the fingerprinting agency.

• Depending on where you live, you must either go to:– Local law enforcement agencies like the Sheriff’s office

or Police Stations.– Privately owned locations which have Live Scan

systems.
