REQUIRED FORMS
PRIOR TO PARTICIPATING IN OR TRYING OUT FOR ANY SPORTS TEAM AT MEMORIAL A STUDENT-ATHLETE MUST HAVE THE FOLLOWING FORMS
TURNED IN:•Valid Physical (good for one year from the date of the
exam)•Emergency Medical Form
•Concussion Form•Minutemen Commitment Form

ELIGIBILITY
•All 7th graders are eligible for the fall
•8th graders must have above a 2.0 GPA and must also be passing 5 of
their classes to be eligible
PROBATION
•Anyone between a 1.0 – 1.999 is eligible for probation and passing 5 classes
•Being on probation requires the student-athlete to attend a mandatory study table a minimum of 3 days per week to remain
eligible for athletics
•Cannot be on probation two consecutive grading periods
INELIGIBLE
•Anyone who is not passing at least 5 classes or is below a 1.0 is ineligible
for Fall Sports
ATHLETIC CODE OF CONDUCT
• All athletes are expected to follow the guidelines contained with the Athletic Code of
Conduct and to be free of tobacco, alcohol and/or drugs
• Any student-athlete who violates any part of the code of conduct will face discipline set
forth by the code of conduct
• All offenses are cumulative and the policy takes effect in the 7th grade
ATHLETIC CODE OF CONDUCT
TOBACCO, ALCOHOL, AND OTHER DRUG USE POLICY
1ST OFFENSE-20% TO 30% OF SEASON
2ND OFFENSE-ONE CALENDER YEAR
3RD OFFENSE-PARTICIPATION IN ATHLETICS AT MENTOR WILL BE
DENIED
INSURANCE
• All students must be covered by insurance to participate in athletics
• If you are not covered by insurance, please see Mr. Jeckel and information
will be given on the Mentor Schools endorsed insurance program.
COMMUNICATION
If there is concern about your child’s playing time or something that is happening at practice please follow the following protocol.
Discuss the matter with your child then discuss the issue or issues
at hand in the following procedure. If your questions are not resolved continue to discuss your concerns with the next person the
list.

PAY TO PARTICIPATE
The Mentor Public Schools has a pay-to-participate fee for all student-athletes.
•High School Athletes-$200.00 for the first sports
•Middle School Athletes-$150.00 for the first sports
•Each additional sport or if a sibling has already participated will be $50.00
EQUIPMENT/UNIFORMS
•All athletes are responsible for school items issued.
•The athlete is responsible for the replacement cost of lost or
damaged equipment
DETENTIONS/SUSPENSIONS
•Any discipline (detentions, suspensions, etc.) can lead to possible
denial of participation in upcoming games
•If constant discipline problems persist this can lead to dismissal from the
team
GAME DAYS
• The coaching staff will advise the team on the dress code for the next game the night
before.
• Boys are expected to wear dress pants, shirt and tie or game jersey/spirit wear
• Girls are expected to wear a dress, dress pants/skirt with a blouse, or game
jersey/spirit wear
TEAM ATTENDANCE
•Must attend all practices and games, unless prior arrangements are made
personally with the coach by note, email or phone call
•Be prepared to practice on the field or designated area with proper equipment
at the time announced
TEAM ATTENDANCE
•Missing practices to play another sport or go to Indians/Cavs games, Cedar
Point, Kalahari, etc. or because you don’t feel like going is NOT acceptable and can lead to denial of participation in the next
game
•Parents are expected to pick up their child within 15 minutes of practice/games
ending
SCHOOL ATTENDANCE
•Student-athletes must be in school for at least ½ of the school day to
participate in practices or games/meets
•Any student-athlete who is not in school at least 1/2 of the school day will
be denied participation in that day’s practice or game/meet.
INJURIES
•If you become injured at practice you must notify a coach immediately
•It is highly recommended that you see a sports medicine doctor before
going to see your regular doctor
INJURIES
•Lake Health injury clinic on Tyler Blvd. on Monday and Wednesday from 7:30-
10:30
•If a doctor has restricted you from participation you must have a note from that doctor clearing you to play. Parents cannot write a note clearing their child to
participate
PTA CONCESSIONS
The PTA needs your help at volleyball games and football games with the
concession stand.
Money raised from the concession stand helps fund the PTA budget.

DENIAL OF PARTICIPATION
•The administration and coaching staff reserve the right to deny
participation in practice or games if a student-athlete violates any of the above rules including the Athletic Code of Conduct and any school
rules.

•Starting on Monday, August 15, 2011 practices will now be from 4-6 p.m.
•We will be on this schedule for the rest of the year. On some Fridays we will end practice early at 5:00-5:30 because of scouting or varsity home football games
GAMES
•Every Wednesday at 4:30
–Except the games against Shore/Ridge which will be at MHS at 5:00 and 6:30, respectively
•We will dress up on game days. Students will wear a shirt, tie and dress pants.
– NO JEANS or SHORTS!
GAMES
•“B” GAMES –For players who do not get a lot of playing time in the Wednesday games.
–Mostly 7th graders but some 8th graders will play
–Games are on Monday at 4:30
–We will dress up for all games!
MIDDLE SCHOOL DAY
Middle School Day•Friday, September 9th vs. St. Ignatius
–Please arrive at 6:30 with your game jersey on in the weight room
•BEHAVIOR AT ALL MHS FOOTBALL GAMES–School event and all school rules apply
–Representing Memorial, your team, your family and yourself!
PRACTICE PACKAGE•The package contains: t-shirt, shorts, and their helmet decals
•At the equipment handout will give each player an order form for the practice package
•Orders are due on Friday, August 5th
•Parents can order packages or extra t-shirts
•It is not mandatory that you purchase the practice package!
