The purpose of this document is to provide instructions for the development ofa Safety Work Group (SWG). SWG's are established by Restoration Services,Incorporated (RSI) under the U.S. Department of Energy (DOE)Environmental Technical Services II (ETSII) contract at the PortsmouthGaseous Diffusion Plant (PORTS), Piketon, Ohio, as an on-going safetyinitiative to promote employee awareness, ownership, and commitment tosafety in all aspects of their daily life.

The goal for the SWG is to ensure that safety is given primary consideration inall aspects of the work that is performed by ETSII personnel, from the stages

of planning through completion, no matter the task or the complexity.

The scope of this procedure applies to the RSI/ETSII personnel. SWGs are

established to successfully identif, and resolve safety issues, discuss and learntopics, as well as effectively accomplish the work while incorporating theIntegrated Safety Management Systems (ISMS) Guiding Principles and CoreFunctions. These instructions will provide SWG members and the assignedSafety Advocate (SA) guidance on developing and maintaining a SWG.

l. The Responsible Manager for each SWG will ensure that time is allottedfor the Safety Advocates to prepare and SWG to function.

1. Volunteer, or be selected from each SWG, to lead the SWG; if possible,

this should be a non-managerial employee.2. Assist with the coordination of SWG meetings.3. Lead, coordinate activities, and guide discussions for the SWG meeting.

a. Communicate information, topics of interest, and lessons learned to the

SWG. This can be done by using the safety topic examples providedin Attachment A.

b. Topics and discussions can be flexible, however at least one topic shallbe work related.

4. Lead safety walk downs for your group.

5. Identif,r, report, track, and facilitate safety issues to completion.6. Work with SWG members to resolve issues.

7. Carry unresolved issues or pertinent information to RSI Management and

RSI Safety and Health.8. Network with other SA's to share concerns and discuss common issues.

9. Maintain the SWG binder on meetings and/or walk downs performed.10. Communicate with RSI Management on the progress of your SWG.

l. Participate as a member of a SWG based on the organization to which youbelong (e.g., SWG: Environmental, Safety, Health and Quality [ESH&Q])

Schedule next SWG meeting/walk down

SA l. Document each meeting and/or walk downs2. Maintain a record of each meeting in a binder for that group. See

Attachment B. These are to be forwarded to the RSI Records within 30

days of the meeting or walk down.

A safety issue is any problem, whether mechanical, electrical, physical, orprocedural, observed in the work place that has potential to render or cause

harm to an employee if not corrected. Safety issues are usually identified bySWG members during the course of a normal day, but can also be identifiedduring the walk down by the SWG.

EMINENT DANGER1. Stop work immediately iÎ ahazard poses an eminent danger and could

cause immediate harm to employee(s).2. Report emergencies by dialing 9l I from a hard-wired phone or

740-897-2444 from a cell phone. You can also call the PORTS Plant ShiftSupervisor (PSS) at 740-897-3025.

Housekeepingo Never leave trash or debris haphazardly around your work area.

o Clean up any spills or leaks immediately.o Remove boxes and other hazards from aisles, walkways, or stairways.

Electricalo Do not daisy-chain extension cords, power strips, or a combination of both.

o Never use damaged cords.

o Ensure electric equipment is approved by Underwriters Laboratory (UL) or another NationallyRecognized Testing Laboratory NRTL).

o Pull the plug, not the cord, when disconnecting an electrical device.

o Do not block access to electrical panels.

o Do not plug a space heater into an extension cord.

Slips, Trips, and tr'allso Do not run electrical cords or hoses in walkways.o Be aware of uneven walking surfaces.

o Use three points of contact when walking up stairs and ascending or descending a ladder.

o Wear proper footwear and ensure good tread.

o Review Emergency Action Plans posted throughout the buildings.

Vehicle Safety/Distracted D rivingo Do not text and drive; it's also good practice to not talk on the cell phone while driving.o Keep a2 to 3 second cushion between you and the car in front of you.o Keep 100% of your attention on driving at all times.

o Sound your horn before backing out.

o Perform a 360 degree walk around before moving your vehicle.o Drive defensively and yield to pedestrians.

o Wear seat belts at all times.

Temperature Extremeso Drink plenty of fluids. V/aiting until you are thirsty is too late. Proper pre-hydration is keyo Never leave another person, child, or pet inside a hot car.

o Gradually build up tolerance to hot weather by increasing exposure time.o Know signs and symptoms of heat-related illness.

o During cold weather try to cover all exposed skin.

o Wear multiple layers when working outdoors in the cold.

o If clothing becomes wet, change into dry clothes.

Ofnice Safetyo Do not leave fully extended file cabinet drawers unattended.

o Load file cabinet drawers from the bottom drawer and work up to avoid tipping of cabinet;

distribute weight evenly.

o Do not store heavy objects overhead. Heavy objects should be stored on the floor.o Load capacity ofshelves or storage should never be exceeded.

o Keep chairs and rolling mats in good working order.

Ergonomicso Keep your feet on the floor or solid on a foot rest while sitting at your desk

o Bend at your knees and keep the back straight as possible, not at the waist.o Limit single person lifts to less than 50 lbs. or 1/3 your body weight.o Micro-breaks should be taken every hour to stretch and rest your eyes.

o Don't be a slouch!

Chemical Safetyo Lead, Asbestos, and UFo can be found around plant site. Please know the area you are entering

and the hazards and controls in place.

o Read labels and become familiar with the products being used.

o Review and understand Safety Data Sheets (SDS) before using products.

o Never store incompatible materials such as oxidizers and fuels together.

Biological Hazardso Inspect your work area for excessive bird/rodent droppings, poison ivy, or poison oak

o Wear long-sleeved shirts and pants.

o Use insect repellent.o Secure pants legs and shirt cuffs.o Check yourself and others for ticks when out in the field.

Hazardous Energyo Any time the unexpected energization or startup of machines or equipment, or release of stored

energy, could harm employees, Lock-out/Tag-out (LOTO) is required.

o Do not service or perform maintenance work on equipment or systems without priorauthorization.

o Do not attempt to operate any switch, breaker, valve, or other energy isolating device where it islocked or tagged out.

o Energy isolation verification shall be performed before servicing or maintenance can begin.

Radiological Hazardso Practice the As-Low-As-Reasonably-Achievable (ALARA) principals: Time, Distance and

Shielding.o Comply with radiological postings, signs, tags and labels.

o Always perform work under the direction of applicable Radiological Work Permits (RWP).

o \ùy'ear anti-contamination clothing and respiratory protective equipment as stated in the RWP.

Spill Prevention and Recoveryo In the event of a spill involving hazardous or toxic substances, take the following actions

' Immediately report the spill;. Stop what you are doing and place the equipment in a safe configuration;. Warn co-workers in the area;. Isolate the area so others do not enter the zone;. Minimize exposure by moving upwind and away from the incident.
