Scope 1.1 This practice covers the qualicat ions, responsibiliti es, and authority of individuals and institutions providing medical direction of emergency medical services. 1.2 This practice addresses th e qualications, authorit y, and responsibility of a Medical Director (off-line) and the relation- ship of the EMS (Emergency Medical Services) provider to this individual. 1.3 This practice also add res ses compon ent s of on-lin e medical direction (direct medical control) including the quali- cations and responsibilities of on-line medical physicians and the relationship of the prehospital provider to on-line medical direction. 1.4 This practic e addre sses the rela tions hip of the on-line medical physician to the off-line Medical Director. 1.5 The aut hor ity for contro l of med ica l ser vic es at the scene of a medical emergency is addressed in this practice. 1.6 The requirements for a Communic ation Resour ce are also addressed within this practice. 2. T erminology 3.1  Description of Terms Specic to This Practice 3.2  communi cation resource—an ent it y respons ible for imple men tat ion of dir ect med ical contro l. (Al so known as medical control resource.) 3.3  deleg ated pract ice—on ly phy sicians are lic ens ed to practice medicine; prehospital providers must act only under the medical direction of a physician. 3.4  direct medical control —when a physician or authorized communic ation resourc e perso nnel, under the dire ction of a physician, provides immediate medical direction to prehospital providers in remote locations. (Also known as on-line medical direction.) 3.5  emergency medical services system (EMSS) —all com- ponen ts needed to provi de comp rehen sive preho spit al and hospital emergency care including, but not limited to; Medical Direc tor , trans port vehicles, trai ned pers onnel , access and dispatch, communications, and receiving medical facilities. 3.6  intervener physicians —a licensed M.D. or D.O., having not previously established a doctor/patient relationship with the emer genc y pati ent and will ing to acce pt respo nsibi lity for a medical emergency scene, and can provide proof of a current Medical License. 3.7  medical directi on—wh en a physician is ide nti ed to devel op, impl ement , and evaluate all medi cal aspects of an EMS system. (syn.  medical accountability.) 3.8  medical director off-line —a physician responsible for all aspects of an EMS system dealing with provision of medical care. (Also known as System Medical Director.) 3.9  on- line medic al physi cian—a physi cian imme diat ely available, when medically appropriate, for communication of medi cal dire ction to non-p hysi cian prehospital provi ders in remote locations. 3.10  prehospit al provider —all perso nnel prov iding emer- gency medical care in a location remote from facilities capable of providing denitive medical care. 3.11  protocols—standards for EMS practice in a variety of situations within the EMS system. 3.12  standi ng ord ers—str ictl y dene d writ ten order s for actions, techniques, or drug administration when communica- tion has not been established with an on-line physician. 1 This practice is under the jurisdiction of ASTM Committee F30 on Emergency Medical Serv ices and is the dir ect resp ons ibi lity of Sub commit tee F30 .03 on Organization/Management.
