It is our privilege to provide care for the patients you refer to us. Our goal is to work in partnership with you to assure that your patients receive the best possible eye care and have a positive experience at the U-M Kellogg Eye Center.

In the last few years, to better serve patients needing advanced eye care, our clinical staff has grown and our services have expanded. We have a comprehensive Thyroid Eye Disease Center, an Ocular Oncology Clinic and a growing roster of experts in diabetes-related eye disease. Our Pediatric Ophthalmology faculty is also growing, with new specialists in pediatric glaucoma and pediatric retinal disease.

Information for Your Patients

All patients will be asked to provide or update personal information whenever an appointment is made. Please make sure your patients, or the individual scheduling the appointment, have the following information available:

• Patient’s complete legal name

• Address

• Date of birth

• Phone number or cell phone number

• Emergency contact

• Insurance carrier

• Referring physician and primary care physician contact information

The registration process will verify a Medical Registration Number (MRN) on file or generate an MRN which will be the patient’s permanent UMHS identifier.

What patients should bring:• Eyeglasses and/or contact lenses

• A list of all current medications, including the prescribed dosage

• X-rays, CT scans, photographs and any additional test results or copies of medical records relevant to the patient’s condition

• Insurance card or the name and address of their insurance carrier, their policy number, the name of the policy holder and their Social Security number

An artificial retina implant, or “bionic eye”, enhances vision for patients with late stage retinitis pigmentosa. Kellogg Eye Center is one of only 13 centers across the country to offer the Argus II Retinal Prosthesis System.
