B. PRE-QUALIFICATION QUESTIONNAIRE

SECTION 1 – MANDATORY QUESTIONS

The Applicant must respond to all questions in this questionnaire using the attached format. The response to each question must refer specifically to the Applicant's engineering office for which qualification is being sought.

1. Has the Applicant independently executed at least two (2) engineering, procurement and construction projects in extending onshore underground, coated and wrapped (30) inch pipelines (minimum diameter), in accordance with the specification of the American Petroleum Institute API 5L, designated to transit crude oil, or gas liquid with all the relevant accessories, motor operated valves, fittings, pigging launchers and receivers, gas scrubbers, gas slug catchers, from basic design through construction, including all the associated civil, electrical and instrument works, which individually has a value equal to or in excess of US$ 70,000,000 and where the commissioning dates were within the past five (5) years?

Details of the projects, which enable the Applicant to answer affirmatively, must be provided in Section Three of this questionnaire.

2. Has the Applicant's Purchasing Department executed purchase orders for major onshore oil and gas pipeline project materials within the past (5) years of a total value in excess of US$ 45,000,000 and an overall procurement period not exceeding eighteen (18) months?


Applicant is requested to submit an attachment to Section 3 of this questionnaire, details of such Purchase Orders executed with dates including values, material description, procurement period and continents, /countries from which materials were purchased.

3. Is Applicant's Engineering Office staffed with numbers of personnel exceeding each of those listed below with not less than two (2) continuous years relevant experience with the Applicant?

6. Can the Applicant confirm that it can perform EPC contracts of value equal to or in excess of US$ 70,000,000 each with all the following work performed with its own resources, manpower and expertise, without requiring to subcontract:-

SECTION 3 – APPLICANT'S PREVIOUS EXPERIENCE

1. Attach (as attachment 1 to this Section 3) a listing of all major relevant engineering, construction and procurement contracts completed over the past ten years from the Applicant's own operational office. Only contracts that were awarded in the Applicant's own name should be listed. Where the contract awarded was a subcontract for a larger project, or was for a portion of work related to a larger project, then the relationship and scope of the work performed by the Applicant must be clearly defined so that it is clear that the Applicant was not the prime contractor for the entire project.

The following information must be provided for each project listed:-

a) Name of Client,

b) Title, Location and Type of Contract,

c) Name of Project Manager responsible for supervision,

d) Applicant's scope of work,

e) Contract Price (approximate),

f) Approximate man-hours expended by Applicant (planned v. actual),

g) Date of Commencement,

h) Date of Completion (planned vs. actual),

i) Percentage of contract subcontracted out, and the nature of the service

subcontracted,

j) If this contract was a part of a larger project, state percentage of the larger

project, which this contract represented.

2. Provide (as Attachment 2 to this Section 3) detailed information in a typical format to 1 above for each contract which enable the Applicant to answer affirmatively to Section 1, Mandatory Questions for the required two projects.

Applicant is advised that the minimum possible number of contracts to be detailed is two. The Applicant is invited to submit further examples of contracts complying with the mandatory questions up to a maximum of 10 contracts example.




SECTION 3 – APPLICANT'S PREVIOUS EXPERIENCE

3. Provide (As Attachment 3 to this Section 3) details of Purchase Orders executed with dates including values, materials description, procurement period and continents/countries from which materials were purchased which support Applicant's affirmative response to question 2 of Part I and Part II of Section 1: Mandatory Questions.

SECTION 4 – APPLICANT'S CURRENT WORKLOAD

1. Attach (As Attachment 1 to this Section 4) a listing of all major a listing of all major engineering, construction and procurement projects (exceeding 20,000 engineering man-hours) presently under progress in the Applicant's operational office, including the following information:

2. State the overall value of other work in hand with the Applicant, which has not listed in 1 above as follows:-

b) Number of man-hours required until completion.

c) Total normal monthly man-hour expenditure on these other work items.




SECTION 4 – APPLICANT'S CURRENT WORKLOAD

3. Attach (As Attachment 2 to this Section 4) separate charts for total engineering design, total procurement and total construction supervision resources versus the data on present workload per item 1 and 2 above. These charts should utilize the data provided by the applicant in Section 5 of this questionnaire.

4. Attach (As Attachment 3 to this Section 4) a listing of the total number of projects presently in the proposal stage in the Applicant's operational office, along with the forecasted approximate total installed cost, total estimated engineering man-hours, total procurement man-hours and total construction supervision man-hours. It is not necessary to list these projects individually or give specifics of each, but merely combine in summary form for the overall total.

2. EQUIPMENT AND FACILITIES

1. Is the Applicant's office equipped with Computer Aided Design (CAD) (2D or 3D), Engineering and Drafting Systems and fully integrated computerized project control and scheduling system capable of indicating periodical progress and performance in design, specifications, purchasing and project construction, including analysis of the overall project progress.

2. Attach (As Attachment 4 to Section 5) a sketch plan of the Applicant's main offices indicating layout and areas. Also, attach brief descriptions of the number and type of CAD workstations and software and of the project management software and systems in use.

3. Does the Applicant own and operate any fabrication or manufacturing facilities related to Construction of Storage Tanks?

Describe (As Attachment 5 to this Section 5) all such manufacturing or fabrication facilities, including sketch plans and location, which would possibly be used if the Applicant had a Major Onshore Pipeline Facility Project in Kuwait. Indicate the kinds of products manufactured and the quality standard to which such products are made.

6. State which parts of a typical Engineering, Procurement and Construction (EPC) oil/gas facility project the Applicant would normally expect to subcontract.
