WHAT IF I DON’T ENROLL?• Your benefit elections from 2014 will roll over with 2015 plan designs if you currently have J.B. Hunt
benefits. Regardless of whether you make changes to your benefits, you should review your plans, ensure your doctors are still in-network, view the 2015 prescription drug formulary, and review your beneficiaries.
• You and your covered spouse will not receive the medical premium discount unless you take the HRA by November 30, 2014, even if you’ve already taken it this year. You must also agree to participate in a wellness program if you are contacted by a member of the Personal Health Team.
• You and your covered spouse will lose your tobacco free discount in 2015 if you don’t certify you are tobacco free by November 30, 2014.
Remember: Benefit elections can only be changed during Open Enrollment unless you have a qualifying event such as a birth or marriage. Read more about qualifying events at myjbhunt.com/people.

— Beginning November 1, 2014, you and your spouse will have access to Keas.
— Earn a medical premium discount by completing the Health Risk Assessment (HRA) and opting into the wellness program on Keas by November 30, and, if contacted, participating in the wellness program.
— Certify you are tobacco free on Keas by November 30 to receive an additional medical premium discount.
• The out-of-pocket maximum for the PPO/OAP Plan is increasing from $6,000 to $6,600. This year, the maximum will include pharmacy.
• If you elect to continue to contribute to an FSA during Open Enrollment, you may roll over up to $500 of any unused funds from 2014 to 2015. In the past, all unused funds were forfeited.
• The prescription formulary list for the HDHP and PPO/OAP plans is changing.

MONEY SAVING TIP #1 Take the Health Risk Assessment (HRA)
• Go to Keas to take the HRA, opt-in to the wellness program, and earn $10 a week off your medical premium—$20 if your covered spouse also takes the HRA.
• If you receive a letter from the Personal Health Team, you must participate in the wellness program they suggest to keep your medical premium discount.
MONEY SAVING TIP #2 Certify You’re Tobacco Free
• While on the Keas site, certify you’re tobacco free to earn another $10 off your medical premium each week. It jumps to $20 if your covered spouse certifies tobacco free, too.

EXPLORE YOUR MEDICAL PLAN OPTIONS
HDHP Administered by Cigna
The High Deductible Health Plan (HDHP) offers a lower medical premium deduction out of your paycheck than the PPO/OAP. Though this plan has a higher deductible, the higher benefit coverage saves you more on what you pay out-of-pocket. In-network covered services generally apply to the annual deductible and 10% coinsurance.
You have the option to open a Health Savings Account (HSA) when you enroll in the HDHP. You can choose to contribute pre-tax dollars to this savings account to help pay for qualified health care expenses, including your out-of-pocket cost for medical, dental and vision services.

HDHP PHARMACY BENEFIT Administered by Express Scripts
You must meet your annual deductible before the plan begins to pay a portion of your prescription drug expenses. However, if you take a medication on the Preventive Drug list, the deductible is waived, and you pay a copay. View the full Preventive Drug list at myjbhunt.com/people.
PPO/OAP PLAN PPO Administered by BCBS; OAP by Cigna
This plan has a lower annual deductible compared to the HDHP, but you pay considerably more in monthly premiums. You pay a copay for in-network primary care provider office visits. Deductible and 20% coinsurance apply for all other in-network services. In addition, the out-of-pocket maximum for the PPO/OAP plan is higher than the out-of-pocket maximum for the HDHP plan.
PPO/OAP PHARMACY BENEFIT Administered by Express Scripts
You pay a copay for covered generic and preferred brand-name drugs. Check two things to make sure you’re getting the best price for your prescriptions: Review the plan’s drug list (called the Formulary) to ensure your medication is covered and verify your pharmacy is in the Express Scripts Network. Non-preferred brand-name drugs and prescriptions filled at a non-participating pharmacy are not covered.
DHMO PLAN Administered by Kaiser Permanente

J.B. HUNT’S 401(k) PLANOpen Enrollment is a great time to enroll in J.B. Hunt’s 401(k) plan or change your contribution amount. Our plan allows you to save for your retirement through payroll deductions. You decide how much to contribute from your paycheck up to the IRS limit ($17,500), and J.B. Hunt will match $.50 for each dollar you save, up to 6% of your contributions. Since your contributions are taken out of your paycheck before taxes, you will save even more. HEALTH SAVINGS ACCOUNT (HSA) Administered By ConnectYourCare
You have the option to open an HSA when you enroll in the HDHP during Open Enrollment.
Contributions Contribute as little as $100 a year and up to the maximum allowed by the IRS ($3,350 employee only, $6,650 family). If you are age 55 or older, you can contribute up to an additional $1,000 per year.
Paying for care This money can be used to pay for your share of qualified health care expenses like medical, prescription, dental, and vision. Or you save the money in this account for future health care expenses.
Saving money HSAs have some of the best tax benefits of any savings account. The money you contribute to your HSA is pre-tax (before any payroll taxes are withheld), which means any interest you earn on your account is also tax-free.
• Your HSA contributions are tax-deductible.
• You can spend this money on most eligible expenses without paying taxes.
• Your account will grow tax-free through interest or increasing investments.
Unused balance If you have money left in your account at the end of the year, it will automatically roll over and is yours to keep whether or not you continue benefit coverage or even employment with J.B. Hunt.
Interest With your HSA account, you are earning interest monthly.
How to enroll You can enroll in an HSA during Open Enrollment if you choose the HDHP plan. You can make changes to your contribution amount at any time during the year.
FLEXIBLE SPENDING ACCOUNT (FSA) Administered By ConnectYourCare
There are two types of FSAs you can contribute to: a Health Care FSA and a Dependent Care FSA. If you currently contribute to an FSA, you will need to re-enroll and select a new contribution amount for 2015 during Open Enrollment.
Health Care FSA • Reimburses you for eligible health care expenses
• For 2015, you can contribute any amount between $100 and $2,500.
Dependent Care FSA
• Reimburses you for child care or elder care expenses
• For 2015, you can contribute any amount between $100 and $5,000.
Limited Use FSA If you have an FSA but also contribute to an HSA, any FSA contributions you make for health care reimbursements become “limited use” for dental and vision expenses only.
Saving money You contribute “pre-tax” money to your FSA (before any payroll taxes are withheld).
Unused balance Up to $500 can roll over at the end of the year.
Interest You do not earn interest with an FSA.
