Why Are We Here? - Recent Events1. Fiber Cut #1 (06/23/11): Miller-Long working on the North Concourse project under BE&K ( ) g g p j
cut into the ductbank 30 minutes after telling Duke Utility Locator & BE&K site rep that they were done digging. Ductbank was properly marked. Root Cause: Process Not Followed
2 Chilled Water Line Cut (07/25/11): MIS working on Learning Center Utility project under2. Chilled Water Line Cut (07/25/11): MIS working on Learning Center Utility project under DUES cut into the chilled water return line feeding the Bryan Research Center. MIS had requested a locate that morning and Duke Utility Locator gave them a time he would arrive. The backhoe operator decided to begin the work and hit the line as Duke Utility Locator was getting out of his truck at the siteLocator was getting out of his truck at the site. Root Cause: Process Not Followed
3. Natural Gas line (08/01/11): During a locate with Duke Utility Locators for the Open Space Project next to the Cancer Center, an MIS employee was helping probing and hit the 2” plastic line with a metal tipped fiberglass probe line. Root Cause: Wrong Tool For Job
4. Fiber Cut #2 (08/02/11): Bateman Civil Survey, as sub-contractor to S&ME, working on the WCSP project under OPM to locate utilities jack hammered into the OIT ductbankthe WCSP project under OPM to locate utilities, jack hammered into the OIT ductbank after saw cutting the roadway. Neither Bateman Civil Survey, S&ME, nor OPM contacted DUES or NC One-Call for locating. Bateman assumed that the month-old locate on the nearby chilled water lines was sufficient. Root Cause: Process Not Followed

Excavation High Risk Zones or Major Construction Projects
Definitions Definitions– High Risk Zone - any area where damage to the underground utility
infrastructure can have an impact to significant number of buildings on campus, to hospitals, to patient clinics or to research facilitiesMajor Construction Project any new building or building addition on Duke– Major Construction Project - any new building or building addition on Duke property
Duke Project Manager or Supervisor/Manager, at least one week before request for locate of utilities shall request Duke Utilities & Engineeringrequest for locate of utilities, shall request Duke Utilities & Engineering Services (DUES) Operations/Project Manager to have a Pre-Excavation Meeting with all appropriate parties
Meeting shall cover:– Review scope of excavation – Excavation/project schedule – Drawings of planned work g p– Drawings of any logistical issues (traffic impact, etc.) – Name of contractor(s) doing work – Temporary utilities or shutdowns if required– Review Risk Mitigation Plan

Excavation Permit Process1 If work area is in a High Risk Zone or a major construction project (new1. If work area is in a High Risk Zone or a major construction project (new
building or building addition), then Pre-Excavation Review must be held prior to project starting. If it is not, go to Step 2.
2. Contractor completes Permit Request Form and is signed off by Duke Project Manager or representative
3. Contractor or requestor must attach a drawing(s) and sketch(es) of planned work
4. Contractor performing the work calls NC One-Call Center to request utility locate 48 hours or more prior to need date
