The OrthoCarolina Wellness Program, brought to you in partnership with Health Advocate, can help you take charge of your health and improve your well-being—plus, you can earn rewards by participating in well-being activities.

The OrthoCarolina Wellness Program is designed to support, educate and motivate employees on the journey of living a healthy lifestyle. The program begins on January 1,2020. There are two levels of incentives.

Level 1 - Premium reduction The deadline to complete Level 1 activities is September 30, 2020.Level 2 - HSA contribution The deadline to complete Level 2 activities is November 15, 2020.

Eligibility and RequirementsThe Wellness Program is available to team members enrolled in an OrthoCarolina medical plan. Team members must complete the required steps described in this guide to earn the incentives.

New team members hired between January 1 - August 31, 2020 have 90 days from their date of hire to complete the physician health screening form. All remaining Level 1 items—preventive care (one screening required), Personal Health Profile and Tobacco Affidavit—must be completed by September 30, 2020.

New team members hired between January 1 - August 31, 2020 have until November 15, 2020 to complete Level 2.

New team members hired between September 1 - December 31, 2020 must submit the physician health screening form and complete all remaining

All About Your IncentivesLevel 1 items—preventive care (one screening required), Personal Health Profile and Tobacco Affidavit—by March 31, 2021.

New team members hired between September 1 - December 31, 2020 are not eligible to earn the Level 2 incentive.

Team members that declined the OC health insurance in 2019, but elected it during open enrollment for the 2020 benefit year have 90 days from January 1, 2020 (March 31, 2020) to maintain the reduced wellness rate (Level 1 incentive) in 2020 by completing the following items:

• Submit a physician health screening form (the physician health screening form may be submitted for visits to PCP between January 1, 2019 - January 1, 2020)

• Personal Health Profile• Preventive care screening (only one screening

required) see page 5 for eligible screenings • Tobacco affidavit

Team members who elected benefits in 2019 for 2020 are not eligible to earn the Level 2 incentive.

Team members who have a life event during the year have 90 days from the date of the life event to earn the reduced wellness rate for 2020 by submitting the physician health form. The physician health screening form may be submitted for visits to the primary care provider within 1 year of the life event. Team members who have a life event are not Level 2 eligible.

1. Get screened - Complete a health screening through your doctor*If you did not participate in the onsite biometric screenings in the Fall of 2019, you must complete your screening with your physician. Current employees: Your physician will need to complete the physician health screening form and submit to Health Advocate by 4/1/2020. New hires: Please see new hire guidelines on Page 2. *Obtain a physician form by emailing [email protected] or [email protected]

2. Meet healthy outcomes - Based on your screening values, meet all of the recommendations for important health standards or show improvement from last year.

*Note: Waist Circumference (WC) always takes precedence over BMI. HbA1c always takes precedence over Glucose.

If you do not meet the standards, or did not make improvements in all areas from last year, you must work with a Wellness Coach to qualify for the premium incentive. A Wellness Coach will help to motivate and support you as you strive to reach your health goals. Coaching is unlimited, but qualifying for the incentive entails 6 engagements with a coach over 3 or more months. The gap between engagements cannot be longer than 45 days. Coaching must begin prior to August 1, 2020. Connect with a Wellness Coach over the phone 866.799.2691, email [email protected] or secure web messaging through the Health Advocate website or app.

3. See your doctor for your age and gender appropriate screenings. The following screenings qualify and only one preventive exam from this list is needed each year. Call Health Advocate for assistance to determine appropriate screenings and coverage. Points are tracked by completing the Wellness Commitment.

4. Complete your Personal Health Profile (PHP) Complete your PHP to learn more about your current health status and steps you can take to improve it. Click “Take my PHP” from your to-do list on the Health Advocate website or app.

5. Be tobacco-free or complete a tobacco cessation program. Attest to being tobacco-free via the online Tobacco Affidavit on your to-do list or complete the 12-week Health Advocate Tobacco Cessation Program – online or with a Wellness Coach. You’ll create your own Quit Plan, get 1:1 support and learn new tips and strategies to become – and stay – tobacco-free!

Screening Type Frequency and age recommendations

Annual Physical/Well-Woman Exam Every 36 months for adults ages 19−39; every 12 months for those 40 and over

Breast Cancer Screening Every 12−24 months for women ages 50−74

Cervical Cancer Screening Every 36 months for women ages 21− 64

Colon Cancer Screening Ages 50−75; options are a fecal occult blood test every year, a sigmoidoscopy every 5 years or a colonoscopy every 10 years

Preventive Dental Visit Every 6 to 12 months

Osteoporosis Screening Women over age 60 depending on risk factors

Prostate Cancer Screening Men age 40 and older

Skin Cancer Screening Every 12 months (recommended but not required)

Vision Exam Every 12 months

Complete Level 2 by earning 200 points by completing the wellness activities below to receive an HSA contribution. New employees hired between January 1, 2020 – August 31, 2020 have until November 15, 2020 to complete Level 2. New employees hired between September 1, 2020 - December 31, 2020 are not eligible to earn the Level 2 incentive. To qualify, you must be enrolled on the OrthoCarolina medical plan and have earned the Level 1 premium incentive. The Level 2 deadline is November 15, 2020.
