We categorize these Medical Service Providers in three groups based on how you can access them:

Ø Frontline provider: a Medical Service Provider whom a Resolution Insurance Member can access service by providing their Resolution Insurance Membership Card. page 2 to 13

Ø specialist providers: a Medical Service Provider who is a Specialist to whom a Resolution Insurance Member can access service by providing their Resolution Insurance Membership Card. page 14 to 22

Ø Referral providers: a Medical Service Provider who is a Specialist and Resolution Insurance Members can only access their services once referred by a Frontline Provider. page 23 to 26
