Step 1: Create an account online

You need to create an account online. Registering provides us with the information we need to return your passport to you after your interview. You will need the confirmation number from your Form DS-260 Online Immigrant Visa Application to register. Registration is free. Step 2: Schedule a medical exam in Haiti

After you complete Step 1, you must schedule a medical exam in Haiti. Click the “Medical Exam Instructions” button below for a list of designated doctors’ offices in Haiti. Please schedule and attend a medical exam with one of these doctors before your interview.

Step 3: Complete your pre-interview checklist

It is important that you bring all required original documents to your interview. We’ve created a checklist that will tell you what to bring. Please print the checklist below and bring it to your interview along with the listed documents.

PROCESS WILL BE DELAYED All immigrant visa applicants, regardless of age, require a medical examination prior to the issuance of a visa. Only a

physician accredited by the U.S. Embassy can perform this exam. It is your responsibility to schedule a medical exam

with one of the doctors listed below before your visa interview appointment at the U.S. Embassy. Medical examination

results from other physicians will not be accepted.

Items to bring to your medical examination

The doctor will need the following items to complete the medical exam forms:

Your visa interview appointment letter,

The confirmation page of your DS-260 Immigrant Visa and Alien Registration Application Confirmation

(https://ceac.state.gov/CEACStatTracker/Status.aspx)

The confirmation page from the online registration of your profile (see Step 1),

Your passport,

Six (6) recently taken passport-sized color photographs, and

A copy of your immunization records.

Any medical examination fees, including x-ray and blood test fees, must be paid directly to the examining physician. Fees will vary based upon the applicant’s individual health and age but may cost up to USD $500.00 per person. This includes any additional exams for tuberculosis, treatment for infectious diseases such as syphilis, and vaccinations. The medical examination fee without vaccines is $285.00 for adults and $205.00 for children under 15 years old. All fees are

paid to the physician and/or radiology facilities and laboratories.

During the medical exam

The medical examination will include a medical history review, physical examination, and chest X-ray, gonorrhea test, and blood tests (for applicants 15 years of age or older). The United States also requires tuberculosis (TB) testing for all applicants two years of age and older. Please be prepared to discuss your medical history, medications you are taking, and current treatments you are undergoing. Your original birth certificate (Extrait acte de Naissance issued by the National Archives) and a photocopy. Children under 2 years old should bring their original acte de naissance authenticated by the National Archives.

Medical examination results in a sealed envelope (if the physician gives you these results).

Original or certified copies of birth certificates for all children of the principal applicant (even if he or she is not accompanying).

Applicants who fall into any category listed in italics below should bring these additional documents:

For family-based visa applications:

The appropriate Form I-864 Affidavit of Support for each financial sponsor along with a photocopy of the sponsor’s IRS transcript or most recent U.S. federal income tax return, and any relevant W-2s.

Proof of your U.S. petitioner’s status and domicile in the United States (photocopy of a U.S. passport, naturalization certificate, or lawful permanent resident card).

Evidence of the relationship between the petitioner and visa applicant (such as photographs, letters, emails, whatsapp or facebook conversation, petitioner’s proof of travel to Haiti or transfer receipts).

If you are married: Your original marriage certificate and a photocopy.

If you were previously married: Your original divorce or spouse’s death certificate and a photocopy.

If you are older than 16 years of age: The original police certificate from your country of current residence and countries of previous residence in which an applicant has lived for one year or more. If these three items are all true, you must bring a more recent police certificate to the interview:

1. You are older than 16 years and have been living at a foreign residence for more than 6 months; 2. You obtained a foreign police certificate more than one year ago; and 3. You still live in the country that issued the police certificate.

Sending documents to the U.S. Embassy

If the U.S. Embassy requests additional information or documentation from you, you will need to submit it at a local DHL branch. Visit www.ustraveldocs.com/ht for information on document submission. Rescheduling or cancelling your interview

If you are unable to attend your appointment, please email support-haiti@ustraveldocs.com for a new appointment date. There may be a significant wait before the next available appointment, so please attempt to attend the date already assigned. For some family-based and employment preference visa categories, a visa became available within the month you have been scheduled by NVC. There is no guarantee that a visa will still be available on the date of your rescheduled interview. Please carefully consult the Visa Bulletin before you decide to reschedule your interview. Security screening procedures

All visitors to the U.S. Embassy in Port-au-Prince must follow certain security procedures. Any visitor who declines to be screened by U.S. Embassy security personnel will be unable to enter the embassy. To avoid delaying your entry and that of those in line behind you, please bring only what is required for your interview. Accompanying persons

The following persons may accompany a visa applicant to their interview and must show their ID card at the embassy:

Interpreter: Applicants may bring ONE interpreter if they do not speak English, French, or Kreyol well enough to participate in an interview.

Special Needs Visitors: Applicants may bring ONE person to help if they are elderly, disabled, or a minor child. Attorneys are not permitted to accompany clients into the waiting room or to their interview. Immigrant visa fees

If you have not paid all required fees to either the National Visa Center, please be prepared to pay these fees on the day of your interview. All fees may be paid in either U.S. dollars or Haitian gourdes equivalent. We accept cash and credit cards only. After Your Interview

A consular officer can make a decision on a visa application only after reviewing the formal application and interviewing the applicant. There is no guarantee that you will receive a visa. Do not sell your house, car or property, resign from your job or make non-refundable flight or other travel arrangements until you have received your immigrant visa. If more information is needed

Sometimes a consular officer is unable to make a decision on a visa application because he/she needs to review additional documents or the case requires further administrative processing. When additional documents are requested, the consular officer will give you a refusal letter that asks you to submit additional documents. The letter will include instructions on how to send those documents to the embassy. Administrative processing takes additional time after the interview. Most administrative processing is resolved within 60 days. However, the timing varies based on the circumstances of each case. Before inquiring about the status of administrative processing, please wait at least 60 days after your interview. What happens after visa approval

Passport, Visa, and Sealed Immigrant Packet – We will place your immigrant visa on a page in your passport. Please review your visa to make sure there are no spelling errors. Your passport will be delivered free of charge to a DHL pickup location. You must have registered your delivery address online (see Step 1). We will also give you a sealed envelope containing documents that you must give to U.S. immigration authorities when you arrive in the United States for the first time. Do not open this envelope. You must carry it with you; do not put it in your checked luggage. If you receive X-rays during your medical examination, carry those with you and give them to the U.S. immigration authorities. USCIS Immigrant Fee – All individuals who are issued immigrant visas overseas must pay an Immigrant Fee to U.S. Citizenship and Immigration Services (USCIS) prior to traveling to the United States. This fee is for processing your residency status and printing your Permanent Resident Card. The only people exempt from paying this fee are: children entering the United States under the Hague Process, returning residents, biological children under 18 of US citizens and people traveling on a fiancé(e) (K) visa. When You Should Travel – You must enter the United States before the expiration date on your visa, which is usually based on the validity of your medical exam. Your visa cannot be extended and all fees are nonrefundable. The principal applicant must enter before or at the same time as other family members with visas. Getting a Green Card – Your Form I-551 Permanent Resident Card, also known as a green card, will be automatically mailed to the address in the United States that you write in your visa application form. This is a very important document that proves you have permission to reside in the United States. If you plan to travel outside the U.S. before your green card arrives: Please consult USCIS’s and CBP’s websites for rules about what documents you need to re-enter the country. We also recommend you check with the airline to ensure you are in compliance with their rules. Once your card is issued, you should not stay outside of the United States for more than one year. If you do, you will lose your status as a Lawful Permanent Resident. Children’s Issues – In the United States, children are required to have certain vaccinations before they can enroll in school. We recommend that you bring your child’s complete vaccination records with you to the United States. If your child is adopted, you have full custody as a result of a divorce, or you share custody with another parent, you should bring a copy of all applicable adoption or custody papers from the authoritative court in your home country. You will need these papers (translated into English) for issues such as school enrollment, medical care, and eventual citizenship. VOTRE DOSSIER SERA RETARDÉ Tout postulant, quel que soit l’âge, est tenu d’avoir une évaluation médicale avant d’être éligible à recevoir un visa. Seul

un médecin accrédité par l’Ambassade des Etats-Unis est abilité à éxécuter cet examen. C’est votre responsabilité de

fixer un rendez-vous en vue de compléter votre examen médical avec l’un des médecins listés ci-dessous avant votre

entrevue à l’Ambassade Américaine. Tout résultat d’examen médical venant d’un autre médecin ne sera pas accepté.

Documents à apporter lors de la visite médicale

Le médecin aura besoin des documents suivants en vue de compléter les formes médicales:

Votre lettre de rendez-vous

La Page de confirmation de votre DS-260 Immigrant Visa and Alien Registration Application Confirmation (https://ceac.state.gov/CEACStatTracker/Status.aspx)

La page de confirmation l’enregistrement de votre profil en ligne (voir Etape 1),

Votre passeport,

Six (6) photos d’identité récentes en couleur (format passeport), et

Une copie de votre carnet de vaccinations.

Tous les frais médicaux, incluant la radiographie et les examens sanguins, doivent être payés directement avec le médecin qui effectue l’examen. Les frais pour l’évaluation médicale varieront, selon l’âge et l’état de santé, d’un postulant à un autre, mais peut coûter jusqu’à US 500.00 dollars par personne. Ces frais incluront tout examen additionnel requis pour la tuberculose, la syphilis et les vaccins. Les frais de l’évaluation médicale sans les vaccins s’élèvent à: US$285.00 pour les adultes et US$205.00 pour les enfants de moins de 15 ans. Les frais sont payables en dollars ou en gourdes. Tous les frais sont payés avec les médecins, les radiologues ou les laboratoires. Pendant l’examen médical

L’évaluation médicale inclura une révision de l’histoire médicale, un examen physique général, une radiographie

abdominale, un examen de gonorrhée et les examens sanguins (pour les postulants de 15 ans et plus). Les Etats-Unis

exigent également des examens de tuberculose (TB) pour tout postulant agé de deux ans et plus. Soyez prêt à discuter

de votre histoire médicale, de vos prescriptions médicales, et vos traitements actuels. Plus d’informations générales sur

les exigences médicales sur l’immigration américaine sont disponibles ici. Il est requis que tout postulant prenne certains

vaccins avant de recevoir leur visa. Les critères de vaccinations sont disponibles ici. Vous pouvez également vous réferrer

Instructions en Vue de la Visite Médicale

Une fois votre examen médical terminé, le médecin vous remettra les résultats dans une enveloppe scellée. NE PAS OUVRIR L’ENVELOPPE. Apportez l’enveloppe avec vous à l’entrevue. Les clichés de radiographie vous seront remis. Il n’est pas nécessaire de les apporter avec vous à l’entrevue. Cependant, vous devez les emporter avec vous quand vous voyagerez pour les Etats-Unis pour la première fois en tant que nouveau résident. L’examen médical doit être encore valide quand vous entrez aux Etats-Unis.

La page de confirmation de la forme DS-260 que vous avez soumis en ligne - Ceac. State. Gov/iv

L’original de votre Extrait de naissance (venant des archives nationales) et une copie. Tout enfant de moins de

2 ans devra apporter l’original de leur acte de naissance muni d’une authentification de signature délivrée par

les Archives Nationales.

Le rapport médical sous enveloppe scellée (si le médecin vous a remis les résultats).

L’original des extraits de naissance des enfants du demandeur (même si il ou elle ne voyage pas).

Tout demandeur faisant partie d’une des catégories ci-dessous devra avoir ces documents additionnels

Pour les applications basées sur la famille :

La forme I-864 (Affidavit de Support) pour chaque sponsor financier accompagnée d’une copie du

relevé des revenus annuels incluant les formes 1040 et W2.

Preuve du statut légal de votre sponsor et de son domicile aux Etats Unis (photocopie de la page

biographique du passeport américain, du certificat de naturalisation ou de la carte de résidence).

Preuves de liens de parenté entre le garant et le postulant (tels que photos prises ensemble, lettres,

courriels, conversations sur WhatsApp ou FaceBook, relevés de vos appels téléphoniques, les reçus de

transferts, preuve des voyages du garant en Haïti) etc.

Si vous êtes marié : l’original de l’extrait de l’acte de mariage, et une copie.

Si vous étiez marié auparavant : l’original de l’extrait de divorce ou de décès

Si vous êtes âgé de plus de 16 ans : l’original du rapport de police de votre pays de résidence et de tout pays dans lequel vous avez vécu plus d’un (1) an. Si vous répondez par l’affirmatif à ces trois critères ci-dessous, vous devez apporter un rapport de police récent lors de votre entrevue:

1. Vous êtes âgé de plus de 16 ans et vivez dans un pays étranger depuis plus de six (6) mois; 2. Vous avez un rapport de police de plus d’un an; 3. Et vous vivez toujours dans le pays qui vous aura émis le rapport de Police.

Envoi de documents à l’Ambassade des Etats-Unis

Si l’Ambassade vous demande des informations ou documents additionnels, vous devez les envoyer par la DHL. Visitez le site : http://www.ustraveldocs.com/ht pour toutes informations sur la soumission de documents. Annulation ou changement de date de rendez-vous

Si vous avez un empêchement et que vous ne pouvez pas venir à votre entrevue, envoyez un courriel à : support-haiti@ustraveldocs.com pour un nouveau rendez-vous. Il peut y avoir une attente importante avant le prochain rendez-vous disponible; prière donc d’essayer d’être présent à votre rendez-vous initial. Pour certaines catégories, le visa n’est devenu disponible qu’au cours du mois de votre rendez-vous assigné par le national Visa Center. Il n’y a aucune garantie en changeant de date de rendez-vous que ce visa soit encore disponible. Veuillez consulter le bulletin des visas (Visa Bulletin) avant de changer la date de votre entrevue. Procédures de sécurité

Tous les visiteurs de l’Ambassade des Etats-Unis à Port-au-Prince devront suivre les procédures de sécurité. Tout visiteur refusant de suivre ces procédures exécutées par le personnel de sécurité de l’Ambassade des Etats Unis se verra refuser l’accès à l’Ambassade. Pour éviter tout retard à votre accès et à celui de ceux en ligne en arrière de vous, veuillez vous munir uniquement de ce qui est requis pour votre entrevue.

Personne autorisée à vous accompagner

Les personnes suivantes peuvent vous accompagner lors de votre entrevue, et doivent être munies de leur carte d’identité :

Un interprète : Le postulant à droit à UN interprète s’il ne s’exprime pas bien en anglais, français ou Créole pour passer son entrevue.

Visiteurs aux besoins Spéciaux : Le Postulant peut être accompagné d’UNE personne pour l’aider s’il est handicapé, âgé ou un enfant mineur.

Les hommes de loi ne sont pas autorisés à accompagner leurs clients dans la salle d’attente ou lors de l’entrevue.

Frais de Visa Immigrant

Si vous n’avez pas encore payé les frais de visa avec le National Visa Center, vous pourrez payer ces frais le jour de votre entrevue.

Tous les frais seront payés en dollars américains ou l’équivalent en gourdes. Nous n’acceptons que les paiements en espèces et les cartes de crédit. Veuillez noter que les frais sont non remboursables si le visa vous a été refusé. La liste complète des frais à payer se trouve ici.

Ne pas planifier de voyage

Si votre visa est approuvé, nous garderons votre passeport à l’Ambassade pour les suites nécessaires (préparation du paquet scellé et du visa pour votre passeport). Nous vous retournerons votre passeport ainsi que l’enveloppe scellée par le service courrier désigné – DHL.

Un Consul ne peut prendre une décision sur une demande de visa qu’après avoir révisé l’application et fait l’entrevue avec le postulant. Il n’y a aucune garantie que vous recevrez le visa. Ne pas vendre votre maison, voiture ou toute possession à votre nom, ne pas démissionner de votre emploi ou acheter un billet d’avion non remboursable ou compléter tout autre arrangement de voyage avant d’avoir reçu votre visa immigrant. Information additionnelles

Parfois un consul ne peut prendre une décision lors de l’entrevue et a besoin de plus de temps pour réviser les documents reçus ou entreprendre une étude administrative. Lorsque des documents additionnels sont requis, le postulant recevra une lettre avec la liste des documents à soumettre et la marche à suivre pour les faire parvenir à l’Ambassade. Une Etude Administrative prend du temps, jusqu’à 60 jours. Par contre chaque cas à ces spécificités et peut prendre plus de temps. Avant de nous contacter au sujet d’un cas en révision pour étude administrative, veuillez attendre 60 jours au moins après l’entrevue. Qu’arrive-t—il après avoir reçu le visa?

Passeport, Visa, Paquet scellé - Votre visa sera placé sur l’une des pages de votre passeport. Prière de vérifier votre visa pour vous assurer qu’il ne contient pas de faute d’orthographe. Votre passeport sera délivré gratuitement aux bureaux de la DHL que vous avez sélectionné. Pour cela il vous faut avoir enregistré votre profil en ligne (voir Etape 1). Une enveloppe scellée vous sera également remise, contenant des documents à remettre à l’immigration américaine, lors de votre entrée au Etats-Unis en tant que résident. Ne pas ouvrir cette enveloppe. Vous devez l’avoir en tout temps avec vous ; ne pas la mettre dans votre bagage sous l’avion. Si vous avez des radiographies garder les aussi avec vous et remettez les aux autorités de l’immigration Américaine.

Frais d’Immigrant - USCIS - Tout individu ayant reçu un visa immigrant devra payer des frais d’immigrant (Immigrant Fee) avec le bureau d’immigration (USCIS) avant de voyager vers les Etats-Unis. Ces frais serviront à traiter votre cas en tant que futur résident et à imprimer votre carte de résident permanent. Les seules personnes exemptes de ces frais sont les enfants entrant aux Etats-Unis sous la Convention de Hague, résidents de retour, les enfants biologiques en dessous de 18 ans d’un citoyen américain, celles voyageant avec un visa fiancé(e) (K). Quand voyager - Vous devez rentrer aux Etats-Unis avant la date d’expiration de votre visa. Cette date est basée sur la validité de votre examen médical. Votre visa ne peut être prolongé et les frais ne sont pas remboursables. Le postulant principal doit rentrer aux Etats Unis d’abord ou en même temps que tout autre membre de la famille ayant un visa immigrant. Recevoir sa Carte de Résidence - Votre forme I-551 pour la carte de résidence permanente, communément appelé « Green Card » / Carte Verte sera automatiquement postée à l’adresse aux Etats-Unis que vous avez fourni sur votre application de visa immigrant. C’est un document important prouvant que vous avez reçu la permission de résider aux Etats-Unis. Si vous planifiez de voyager en dehors des Etats-Unis avant d’avoir reçu votre carte de résidence, consultez les sites de USCIS et de CBP pour vérifier les documents nécessaires pour retourner aux Etats-Unis. Nous vous recommandons également de vérifier avec la ligne aérienne que vous êtes en conformité avec leurs règlements. Une fois votre carte de résidence émise, vous ne devez pas rester en dehors des Etats-Unis plus d’un an. Si c’est le cas, vous perdrez votre statut en tant que résident permanent.

Points liés aux enfants - Aux Etats-Unis, les enfants doivent être vaccinés avant de s’inscrire dans une école. Nous vous recommandons d’apporter les documents ou carnets de vaccinations de votre enfant avec vous. Si votre enfant est adopté, ou si vous en avez la garde complète ou vous partagez la garde avec un autre parent suite à un divorce, vous devez avoir une copie de tous les documents d’adoption ou de garde applicables auprès du tribunal compétent de votre pays d’origine. Vous aurez besoin de ces documents (traduit en anglais) pour faciliter l’inscription à une école, recevoir des soins médicaux et éventuellement pour la citoyenneté américaine.
