PRE-QUALIFICATION NOTICE FOR EMPANELMENT OF

CONTRACTORS/VENDORS/SUPPLIERS

ESIC MH RNR Bangalore invites application on prescribed format from eligible contractors for

empanelment of contractor/vendors/suppliers for civil/electrical/electro-mechanical works of the ESIC

buildings in Bangalore.

Civil/Electrical/Electromechanical Works:-

The applicant firm/company should fulfill the criteria to be eligible to be empanelled. The details

of eligibility terms & conditions and application form are available on ESIC's Website: www.esic.nic.in

(or) http://esipgirnr.kar.nic.in which may be downloaded.

Interested parties desirous of being considered for inclusion in the panel may submit their

application complete in all respects in the prescribed format alongwith the requisite documents so as to

reach Medical Superintendent, ESIC MH, RNR, Bangalore latest by 12.30 hrs on 06th April'2015.

ESIC reserves the right to reject any (or) all applications without assigning any reasons and to

restrict the list to any number deemed suitable by it, if too many applications are received satisfying the

basic pre-qualification criteria, ESIC also has the right to reject all the applications and go in for re-

advertisement without assigning any reason.

1.0 NOTICE INVITING APPLICATION FOR EMPANELMENT OF CONTRACTORS / VENDORS/

SUPPLIERS FOR CIVIL/ELECTRICAL/ELECTRO-MECHANICAL WORKSCONTENTS

Medical Superintendent, ESIC MH Rajajinagar, B'lore invites applications for empanelment of

contractors from the firms who are registered with CPWD/State PWD/MES/Railways/MSEDCL,Semi

Govt. Organizations and other Autonomous bodies under Government of India as contractors/vendors

for undertaking various civil works at ESIC MH RNR Bangalore.

1.1 .a) Scope of works – Civil Works

A) The Panel of civil work Contractors to be mainly used for Civil Engg works like earth work

excavation, Brick masonry, stone masonry, plastering RCC work, painting, Flooring, Plumbing Sanitary,

water proofing, demolition, structure steel fabrication work in Aluminium partition, furnishing works and

other allied works for ESIC Model Hospital costing upto 5 lakhs at ESIC MH RNR on selection.

B) Cost wise sub classification:-

The contractor classified under lowest cost limit cannot quote for the works costing more than

the limit. However the contractor classified under higher class can participate for works under lower

class

For the works costing more than Rs. 5 Lac, an open tender will be called and for such tenders,

the Empaneled contractors under this process meeting the stipulated criteria of tender can also

participate.

1.1 b) ELIGIBILITY CRITERIA FOR CIVIL WORKS:-

Prospective agencies will be required for civil works/Electrical/ Electro-Mechanical Works to

fulfill following criteria for participating in the tendering process.

a) The Firm / Contractor shall be registered with Government / Semi Government organization

like CPWD/State PWD/MES/Railways/MSEDCL, Semi Govt. Organizations and other autonomous

b) The Firm / Contractor shall be registered with Income Tax and Service Tax Department as

per Govt. rules & regulations.

C) The Firm / Contractor should have average annual turnover (gross) for the last three years of

Rs. 1,50,000/- (Rs. One Lac & Fifty Thousand Only) and should not have incurred any loss in more

than two years during the immediate last 3 consecutive financial years ending on 31.03.213.

d) The Contractor should have carried out similar type of works during last five years as below:

I) Three CIVIL related works costing not less than 2,00,000/- or

II) Two CIVIL related works costing not less than 3,00,000/- or For 'B' Class Category

III) One CIVIL related work costing not less than 4,00,000/- Contractor

E) 1) Three Civil related works costing less than 1,00,000/- or

2) Two Civil related works costing less than 2,00,000/- 'C' Category

1.2. a) Scope of Work – Electrical/ Electro-Mechanical Works:-

(A) The panel of electrical Engineering contractor to be used for electrical and electrical related

work such as electro mechanical works, bore well pumps, motors, operation and maintenance of

111cvH.T sub station, D.G Sets, U.G Cable laying connecting with system, cable jointing using by

suitable cable joint kits and other allied works for ESIC Model hospital costing in upto 5 lakhs at ESIC

Model hospital, Rajajinagar, B'lore on selection.

(B) Classification of works :

The contractor classified under technical capacity on electrical installed, approved and

authorized by electrical inspectorate of Govt. of karnataka.

For the work costing more than Rs. 5 lakhs on open tender will be called and for each

tenders, the empaneled contractors under the prior meeting the stipulated criteria of tender can also

participate.

1.3. ELIGIBLE CRETERIA FOR ELECTRICAL VENDORS/SUPPLIERS:-

1) It should be registered Electrical shop as shop of establishment.

2) He should be authorized dealers for spare parts.

3) He should have VAT registered No.

4) He should be authorized stockist.

5) Vendors/Suppliers must have minimum 10 lakh turn over per year.

1.4 ISSUE OF APPLICATION FORM:

The application form for empanelment shall be downloaded from the ESIC Website:

www.esic.nic.in (or) http://esipgirnr.kar.nic.in/ . The Demand Draft of Rs. 1,000/- drawn in the

name of ESIC Fund A/c No: 1 towards the cost of application form should be enclosed alongwith the

submission.

1.5 SUBMISSION OF APPLICATION:

The completed empanelment document shall be submitted to the Medical Superintendent, ESIC

Model Hospital, Rajajinagar, Bangalore on or before 12.30 hrs on 06th April'2015.

Medical Superintendent, ESIC MH, RNR, B'lore reserves the right to accept the offer in full or in

parts or reject summarily or partly without assigning any reason(s) thereof.
