Placing a composite filling is not a simple task as there are many steps that must be completed properly to obtain a good immediate result, but more importantly a good long-term result (Fig. 1 Pre-Op).

should have rounded internal line angles which will allow for easy adaptation of materials during placement. Any sharp internal line angles, boxes or grooves should be avoided as they can be difficult to get good adaptation which could create areas of composite that are not adapted well. This would lead to possible voids and heightened stress due to C-factor stress on materials during polymerization.

In some restorative cases where a proximal surface is to be restored additional tools will be necessary. One such tool is the use of a tooth guard when preparing the tooth so as not to cause iatrogenic trauma to the adjacent tooth or restoration. A caries detector solution should also be implemented to verify that any remnants of tooth decay have been completely removed. Furthermore, bond strengths are higher when adhesion is performed on affected tooth structure and can be lower on infected tooth structure by as much as 30% (Fig. 2).

Proper isolation is critical to the success of any bonded restorative material. In this case the implementation of a rubber dam was utilized. The first step is to properly prepare the cavity preparation. Following the removal of an existing restoration and/or decay, the final preparation

An additional tool that is required when restoring the interproximal surfaces is a matrix system that can contain the filling material, recreate the same anatomical missing tooth structure and re-approximate the interproximal contact. In this example, the TriodentV3 Ring matrix system and wave wedge were utilized to create the ideal

Apply G-Premio BONDTM to entire preparation and wait 10 seconds (Fig. 5). Next, dry the adhesive for 5 seconds using maximum air pressure. Then light cure for 10 seconds.

The application of a thin layer of G-ænialTM Universal Flo on the pulpal floor is an excellent first increment for adaptation and additional strength. The application of G-ænialTM Universal Flo should be only on the pulpal floor and not touching theaxial walls (Fig. 6). As a first increment, it offers excellent adaptation

anatomical position, shape and pressure for the interproximal contactto be built out of composite. Additionally, the gingival margin was sealed with the wave wedge securely pushing the V3 matrix or super curved matrix tightly against the tooth, while theTriodent V3 ring holds the matrix against the proximal and cavosurface margins and at the same time displaces the teeth slightly (Fig. 3).

Depending on the tooth substructure that is being worked on or the dentist's preference in adhesive bondingtechnique, G-Premio BONDTM (a universal, eighth generation adhesive bonding agent that achieves outstanding performance in all etching techniques and situations) can be implemented. It can be used as a total etch, selective-etch or self-etch. If enamel is present, it is highly recommended to do a selective-etch with phosphoric acid prior to placing the adhesive bonding agent (Fig. 4).

The use of cavity cleansers, disinfectants and glutaraldehyde-based products are not necessary.

and can help minimize the amount of stress and C-Factor on the adhesive interface with the tooth. This is again followed by light curing for 10 seconds. G-ænial Sculpt® is to be applied next in 2mm increments to minimize any polymerization stress on the tooth structure (Fig. 7).

G-ænial Sculpt® is a compactable, universal composite that features the same high density, uniform dispersion nano-filler technology found in G-ænialTM Universal Flo. Incrementally layered composites still are shown to offer the best long-term performance when placed properly.

G-ænial Sculpt® is easy to shape, sculpt and polish. Using a condenser to adapt followed by various carving or shaping instruments for creating primary and secondary anatomy (Fig. 8).

The composite is then cured to completion followed by the removal of excess composite using various knives, carvers and fluted carbide burs followed by polishing points or cups (Fig. 9).

The final appearance of G-ænial Sculpt® after removal of the rubber dam is an ideal reproduction of toothstructure in both shape, mechanical properties, color, polish and performance (Fig. 10).

Congratulations on 95th Anniversary

By using the model constructed as a healthy long-living society, contribute to the world'shealth-building.

In order to react flexibly to any kind of changes in the external environment, many measures are beingimplemented such as global personnel exchanges and holding of overseas Nakao School, changes in organi-zational structures, investment in new business aiming for solutions in dental care, challenging new fieldand so on. Though there are still trial-and-error occurring in some of these, we think such attempts are ameans of building one's own future. The lessons learned from the 95 years of history is the fact that “a strat-egy without action is meaningless.” What our predecessors realized one by one are engraved into the historyof GC as major results, and it is fervently wished that this be recognized in full.

Transmitting from Japan, which is now the world's top nation for longevity!

The memory is still fresh from last March when, in joint sponsorship with the Japan Dental Association andWorld Health Organization (WHO), the “World Congress 2015” was held in Tokyo. Here dental medicinepolicy recommendations based on evidence from Japan and overseas in addition to policy deliberations as tomeans of protecting the health of the increasing number of senior citizens were made. Due to these activities,issues in common to Japan's as well as the world's aging societies were clarified and approaches specified,thereby leading to the “Tokyo Declaration” being agreed to on March 15th. In line with this announcement,GC partnered with the World Dental Federations (FDI) aiming to solve the common issues for the world'sdental practice field, bringing together the dental associations not only of Japan but also of the U.S.,Germany and France for formation of a task force. In terms of specific activities, these are being carried outat a global scale. Along with those global activities, GC has already started on a path to realize the Vision2021: "Challenge to the No. 1 dental company of the world through making further contribution to thehealthy long-living society". Blessed by today's the beautiful weather, on February 12, 2016 in the largemeeting hall at Tokyo's Hotel Gran Pacific LEDAIBA some thousand Associates who gathered fromthroughout Japan and the world have exchanged greetings in a splendid mood. With the 100th anniversaryin mind, GC's challenge is thought to portend further expansion globally at this 95th anniversary celebrat-ing GC's foundation. The opening remarks by Managing Director Tetsuro Sakuma expressed thanks to thepredecessors all for supporting the company and warm words of gratitude to all the Associates who hadgathered at the venue for their daily efforts . Moreover, reference was made to the words of President Nakaoexhorting all to “Become the Clear Top Company in Japan” as given at the start of the year was referred to

Under a similar regulatory regi-men henceforth he called for more activities to be car-ried out under the recently-touted TPP deregulationsand the possibility of regulatory easing, in order tofind opportunities for attaining Vision 2021. He con-cluded by noting that combining everyone's power willlead to a chance for change, albeit while not losingsight of the goals in the process, to steadfastly proceed

President Kiyotaka Nakao Speech

President Kiyotaka Nakao's speech recounted GC's history, noting that it had become a company whichcontributes to dental health with help from Associates all over the world as a result of the fervor andincessant efforts on part of the Associates from the past and present. He continued, "In preparing for theanniversary, we are accumulating efforts under 'Vision 2021', targeting to become the No. 1 dental compa-ny of the world". In order to achieve this Vision, it is known well that GC has been pursuing a functionalmanagement system by clearly positioning “Functions” and “Departments” and cascading the tasks of a“Function” into each “Department.” Recently, we have been discussing the requirements for “the No. 1dental company of the world.”By "World's Top" there are various says of evaluating this. It may simply be in terms of total sales, orbeing able to produce revolutionary new products or being able to set the benchmarks...these are still indiscussion. With recent years seeing changes in the "external environment" occurring rapidly, we in dentistry arealso showing major changes. Yet trying to forecast on one's own this "external environment" or to imple-ment changes is no easy task. It may in fact not be saying too much to call this a task well nigh impossible."So what is to be done in this case? It is thought that what is most important even for us is 'how torespond flexibly to any change in the external environment.' And in order to make such flexibility secondnature, we must do what we are currently doing, 'create changes ourselves.' It is not to be reactive tochanges in the external environment but by one on own to create changes, such that we can gain 'organi-zational strength and resilience' to be able to withstand any environment... this is the only way to enableresponse to any possible changes in the external environment in the future and thus achive the goal ofbecoming the world's Number 1 dental company. This is the only way to build further prosperity, Ibelieve." continued the President.As an attempt to create change ourselves would be global human resources exhanges and holding of over-seas Nakao School, changing the organization structures, investing in new businesses that can providesolutions sought for by dental care, challenging new fields among other things as to many measures beingundertaken. Of these, there are still many undergoing trial-and-error but such attempts are thought to bethe means of building one's future by oneself."Moreover, we dental manufacturer have as our job the development of products that fits the clinicalneeds and produce these with the utmost quality so as to deliver these in the best condition to our cus-tomers. Thus today, our products, services and the like are 'corporate quality' and have gained very highpraise. This is all due to the 'incessant effort' of our Associates I believe. By corporate quality, by havingall Associates keep customer-oriented for everything from research, manufacture and sales as well as allrelated section, the company in total can continue to be improved and thus turn it into a major strengthfor us. As such, by downplaying the viewpoint of customer orientation and de-emphasizing this stance, we

can consider ourselves to be facing grave danger; thebottom line is that our main competitor is 'one's ownhaughtiness'.""Upon improving corporate quality, saying 'this is ok'or 'this is enough' means the end. Thinking somethingto be 'perfect' is just the beginning, and one must con-tinue to go forward.It is now a short while before the 100th anniversary ofthe company's founding is marked. We in manage-ment had once again discussed often what the themeof making the GC Group into 'the World's Number 1dental company' by 2021 means in preparing for the95th anniversary.”“As a result, we think the key for achieving the Visionis the enhancement of GQM2021 for improving thecorporate quality, and set the "8 KPIs" which consistof indicators directly linked to 3 aims GQM 2021 andkey performance indicators in management". Taking this occasion, we would like to declare that we challenge to realize our Vision 2021 by achieving theworld's No. 1 level in each of 8 KPIs, together with all GC Associates. Based on the spirit of 'SEMUI' andusing the 'Unity of Nakama' that has continued unwaveringly since our company foundation as aweapon." the President concluded, noting that further expansion is to be made definite.

Upon celebrating the 95th anniversary of the company's foundation, Chairman Makoto Nakao spokewarmly of the Associates past and present plus of their supportive families in starting off, then turned hisattention to the previous day's (Feb. 11th) Global KI Presentation. GC America Goes on Safari in Kenya

In the middle of the Maasai Mara in south Kenya is an oasis of hope and health for all the Maasai tribe people. The Maasai Dental Clinic was established by Dr. Ray Damazo, the safari dentist.

Utilizing air abrasion, the very first sealant program in the Mara has begun thanks to donations of GC Fuji IX GP®, GC Fuji TRIAGE®, and G-ænialTM Flo. Using very conservative diamonds and the air abrasion, those stains can be removed (Fig. 2). The teeth are then etched, bonded, and a very white shade of GC composite bonded over the surface of the teeth. When the mothers see what we can do for their children, tears of joy stream down their faces. No one has ever offered this service to them and the Maasai line up in the morning and are willing to wait all day for their turn to have this life-changing procedure done, all thanks to the amazing generosity of GC America and their great products.

Guaranteed Passivity forImplant Prosthetics

In the most ideal scenario, full arch implant planning is performed well in advance of the actual surgery. This planning should be based on an in depth analysis of the particular patient’s anatomy, and a number of other factors. Trials can also be conducted in relation

to desired tooth positions, how these positions will function, what kind of esthetics are desired, and phonetics testing is also of high concern.

Unfortunately, well planned cases are more rare than they should be. But how can implant position challenges be overcome? This is the first of a number of articles relating to different positional challenges regarding implant prosthetics. In particular, this article will deal with the importance of passivity in full arch implant cases.

The best way to deal with a long spans between implants is to rigidly and precisely connect them. Not all materials lend themselves to accomplishing this over long spans. Implant companies and milling centers have for many years now provided CAD/CAM Titanium milled bars to rigidly and precisely connect implants, but what about the variables in the current techniques leading up to the milling of a Titanium bar? Are these variables acceptable when it comes to the precision and passivity required to provide splinting protection of multiple implant sites?

The VariablesThere are currently a number of processes both on the clinical side, and in the laboratory that can potentially introduce small errors in fit when it comes to the existing protocols for milled Titanium bar hybrids or screw retained prostheses in general. For example, all dental gypsums exhibit a measurable amount of setting expansion. Measures are taken to lessen the impact of this property, ie. verification jigs, etc., however setting expansion still exerts stresses, even on splinted implant analogs.

Another potential variable is in the scanning and milling phases. The CAD/CAM process itself can introduce variables. As an example, different scanners scan at differing levels of accuracy, and the particular scanner’s level of compatibility with the mill can affect precision.

Eliminating the VariablesThe only reliable way to eliminate all variables and achieve true passivity is to passively lute components for the final prosthesis in the mouth, without allowing any opportunities for error introduction via setting expansion and/or the CAD/CAM process.

abutments has long been acknowledged. The CAL (California Abutment Luting) Passive technique circumvented all of the afore mentioned variables when developed in 1992.

There are also other solutions that utilize this passive luting concept. The Deutsch Removable Bridge ZR® for example, utilizes this concept to achieve maximum passivity with telescopic and conical 2˚ abutment to prosthesis attachment connections.

The following case demonstrates how one implant solution successfully achieves perfect passivity by means of an intra-oral luting technique.

The CaseThe patient previously had a bar retained, tissue supported plunger restoration over 4 Straumann tissue level implants, however the patient is elderly, and the milled bar was difficult to clean around. A free standing abutment solution was desired to improve the patient’s ability to cleanse around the implant sites. A consultation was arranged for all members of the dental team to meet with the patient and discuss options.

The chosen solution was a removable telescopic implant bridge, the Deutsch Removable Bridge ZR®. The workflow was discussed with the entire team and the patient so that all involved knew what to expect.

A maxillary impression was taken with open tray impression copings, and a soft tissue cast was made using Gi-Mask (Coltene) and vacuum mixed NEW FUJIROCK® IMP (Fig. 1). A bilaterally screw retained wax rim was fabricated and sent to the dental office for vertical and centric relationship records. The centric and vertical relations were recorded by the clinician and mounted on the articulator in order to provide tooth positions in wax (Figs. 2, 3).

to an intraoral pickup. The primary conical 2° abutments were designed digitally within the prosthetic space (Figs. 4-7), and milled in zirconia. Temporary secondary copings were created with a light curing material (Primopattern Gel Clear, Primotec, USA) and light cured. A verification key was also fabricated using a clear light curable material (Primosplint, Primotec USA) and excess material vents were cut into the key. In the clinic, the primary 2° zirconia hybrid abutments were placed and torqued to the recommended NcM. The temporary clear copings were placed, and the verification key was inserted and evaluated to ensure no binding on the temporary secondary copings. A bonding agent (Primostick, Primotec USA) was applied to both the temporary copings and the verification key. The verification key is partially filled with the clear light curing gel (Primopattern) and placed over the temporary copings. Each site is now light cured and the key is removed. The key is used to record and rectify the exact implant positions in the master cast.

The primary telescopes are re milled at the rectified implant positions, and final polished in preparation for gold electroforming. Gold is electroformed directly to the

surface of the 2° conical hybrid zirconia abutments (Figs. 8-10), and checked for fit. Once fit is verified, the tertiary structure is digitally designed and fabricated using SLM (Selective Laser Melting) technology (Figs. 11-14).

Final Passivity ControlThe final passivity control is made by using the patient as a master model of exact implant locations by performing an intraoral pickup of the gold secondary components in the tertiary SLM structure with cement (G-CEMTM Capsule) (Fig. 15). When this is done, all variables are eliminated. Any material or technique that could potentially introduce error is eliminated, and only then can we achieve 100% passivity with no micro movement.

After the final passivity control is performed, the master cast can be corrected, the artificial dentition can be meshed back with the corrected master cast, and the master cast can be duplicated as a final processing cast. The entire prosthesis in wax is transferred back to the articulator to verify tooth positions have not changed, and the case is prepared for wax elimination and acrylic injection and polymerization.

processing cast, sealed, and invested in the bottom portion of the flask with a Type IV reinforced gypsum (NEW FUJIROCK® IMP). Sprue channels are connected and a dual part silicone is placed over the entire wax surface leaving the occlusal surfaces of the artificial dentition exposed to ensure that the dentition stays in place rigidly. The top portion of the flask is closed together and invested. The flask is warmed in the wax elimination bath (Dreve Labormat SD), the flask opened, and wax is removed and all interior surfaces are cleaned thoroughly. The teeth are cleaned and prepared for processing.

Gingival characterization is now made using a heat curing, stable acrylic (NATURE-CRYL® SUPER HI IMPACT). The appropriate colors are selected, fibers are removed, and the colors are mixed and placed into plastic syringes. Additional color markers and intensifiers can be added if desired.

When the acrylic in the syringes reaches an optimal working consistency, the syringe is used to place the lighter shade of acrylic into the inter proximal and cervical areas around the teeth to mimic a less vascular, blanched appearance that is present

in healthy gingival tissues. The lighter material is then manipulated with a kolinsky brush wetted with modeling monomer, and the darker, more vascular looking colors are added and also shaped and merged with a kolinsky brush (Figs. 16, 17).

The flask is closed and the acrylic (NATURE-CRYL® SUPER HI IMPACT) is mixed and placed into an injection capsule. The acrylic is injected into the flask and cured. Once the polymerization is completed and the flask is cooled for 30-45 minutes, the flask is opened and the prosthesis divested with an air chisel (Renfert Air Pillow).

The surfaces are shown to be very clean and require minimal finishing time due to the use of the dual part silicone putty (Anaxdent Matrix Form 70) (Fig. 18). The prosthesis is finished with a variety of carbide grinders and brushes, and finally polished carefully with pumice substitute and high shine polishing compound (Acryl-Marvel Dental Ventures of America Inc.) (Fig. 19).

On the day of delivery, the laboratory team is present for seating of the prosthesis. The patient and clinical team is very pleased with the outcome in terms of retention, stability, and esthetics (Figs. 20-22).

ConclusionMicromovement has been proven to contribute to implant screw loosening and eventually in some cases linked to implant failure. By adding techniques and prosthetic options to our “tool box” which provide 100% guaranteed passivity, we can successfully overcome these obstacles.

Using the patient as a master model has always been, and still is the only way to achieve a truly passive fit between prosthesis and multiple implant sites.

Dr. Arian B. Deutsch, CDTDr. Arian B. Deutsch, CDT, is a Certified Dental Technician who has studied at the university level in the United States and Canada. From 1993 to 2007, he served as General Manager for his father’s small, boutique laboratory, Eurotek Dental Concepts in Fairfield County, CT. At the laboratory, Mr. Deutsch specialized in the Swissedent Technique, taught by Dr. John P. Frush, DDS. From 2004 to 2006, he studied at George Brown City College’s IDEC Program (in Toronto, ON). In 2007, Mr. Deutsch moved to the Phoenix, AZ area, opening his own laboratory, Deutsch Dental Arts, specializing in “life-like rehabilitations” through a variety of traditional and unique implant solutions such as Telescopic Implant Bridges and the Marius Bridge. In 2009, he received specialized training from Daniel LeClerc, T.D. and Dr. Yvan Fortin, DMD, in Quebec City, QC, subsequently becoming the first accredited provider of the Marius Bridge in the Southwest United States. In 2012, he trained with Frank Poerschke, MDT, in Lutjenburg, Germany and became a Certified Merz TPiF (Total Prosthetics in Function) Trainer. In February of 2012, he was awarded first place in the Candulor’s Kunst Zahn Werk (Art of Dental Prosthetics) North American Competition. His articles regarding advanced clinical techniques and the current shortage of skilled Dental Technicians have appeared in journals such as: Inscriptions (the official journal of the Arizona Dental Association), Dental Laboratory Products, Inside Dental Technology and Labline (in Hungary). Mr. Deutsch has lectured nationally on a variety of topics, ranging from Obstructive Sleep Apnea to Advanced Clinical and Laboratory Techniques, aiding in the creation of predictable, life-like Dental Prosthetics.
