(Application is on Pages 3-4) ELIGIBILITY CRITERIA To be considered for this scholarship, you must: 1. Be a current and graduating 12 th grade student of West Shore Jr. Sr. High School 2. Have attended West Shore Jr. Sr. High School, continuously for grades 9-12 3. Be accepted/conditionally accepted for full-time enrollment to attend An accredited junior or community college in the State of Florida, or an accredited four- year college or university in the United States 4. Cumulative GPA of 3.5 or higher unweighted SELECTION PROCESS The Scholarship Committee will consider these criteria in making their selection: 1. Community service – Document community service and hours completed to date 2. Leadership activities while enrolled at West Shore Jr. Sr. High School 3. Letter of recommendation (teacher, coach or community service advisor) 4. One page maximum 500 words – typed Essay – discussing something you’ve done that’s made a difference in your community 5. Financial need 6. Other known financial assistance received by the applicants will also be taken into account 7. A semi-finalist will also be selected in the event that the single award recipient is unable or for some reason becomes ineligible to utilize the award 8. All decisions of the scholarship selection committee are final DEADLINE TO APPLY Applications must be received by Monday, March 16, 2020. Applications received after the March 16, 2020 deadline will not be considered. NOTIFICATION Award letter will be delivered/mailed April 8, 2020.

AMOUNT OF AWARD

The annual award for the Key Club Scholarship will be $1,200.

METHOD OF DISBURSEMENT

Award is paid by Brevard Public Schools made payable to the Key Club Scholarship recipient

and should only be used for tuition, books and fees.

SCHOLARSHIP DURATION

The “Key Club Scholarship” is a one-time only scholarship for a single award.

REVOCATION OF SCHOLARSHIP

This scholarship may be revoked by the Committee of the “Key Club Scholarship” for any of

the following reasons:

1. Students not successfully completing all academic requirements for graduation as set

forth by the administration of West Shore Jr. Sr. High School and Brevard Public

Schools.

2. Any act of misconduct which would bring discredit to the image of the scholarship is

subject to review by the Committee of the “Key Club Scholarship” and possible

revocation.

HOW TO APPLY

DIRECTIONS FOR SUBMITTING SCHOLARSHIP APPLICATION:

1. Complete the application in full. A plain sheet(s) of paper with your name at the top

may be used for additional information on each question.

2. Submit the completed application to the Guidance Department, Attn: Mrs. Leslie

Kellner, by March 16, 2020.
