BackgroundElectronic Visit Verification, or EVV, will be implemented by the Iowa Medicaid Enterprise (IME) on a date that is to be determined, following a robust period of stakeholder engagement and program research. In December 2016, the 21st Century Cures Act was signed into law. This law requires EVV for personal care and home health services. The IME is working in collaboration with the Managed Care Organizations (MCOs) to develop and implement the EVV program.

EVV will be used to monitor the delivery and utilization of personal care and home health services in non-traditional settings and will provide verification of the visit with location information and a time stamp. EVV will be used to ensure quality and program integrity (PI).

In addition to the PI characteristics associated with EVV, the department also considers this to be a valuable mechanism to ensure that members are receiving quality care outlined in their person-centered service plan. This system will help provide real time alerts when a provider is late or misses a medically necessary service included in a member’s person-centered service plan.

The IME conducted a survey from June 16 to August 15, 2017, to gather input from providers. This survey is one component, of several, designed to gather information directly from stakeholders. The information gathered from this survey, in combination with face-to-face meetings and other engagement, will inform the decisions that determine key aspects of the Iowa EVV program.

SurVEy dataThe data in this report is based on provider responses to the IME’s EVV Provider Survey conducted using Survey Monkey®. Survey responses were collected from June 16, 2017, to August 15, 2017. A total of 957 respondents who identified as providers participated in the IME EVV Provider Survey. Survey responses are separated by provider type, which allows for more detailed and meaningful analysis.

SuRVEy DATA LIMITATIOnSThere are a number of survey data limitations that are important to point out.

The goal of this survey was to gather responses from providers. The first, required, question asked the respondent if they are a provider. The survey data reflects responses from self-identified providers. The IME did not authenticate or validate respondents’ claim to be a provider. Providers were asked to identify their provider type. There may be inconsistencies in the way respondents identified their provider type. Similarly, there may be inconsistencies in the way respondents identified their current system capabilities, and their estimates for future needs.

It is important to pay attention to the number of ‘Answered’ and ‘Skipped’ respondents beneath each question as some questions are only answered by a subset of a given provider group. For example very few Individual CDAC providers responded that they have some existing EVV system, and thus the following questions about that system has a very low number of responses and should be considered accordingly.

When looking at responses from provider type ‘Hospice,’ notice the limited number of respondents.

When respondents indicate they have a large number of employees, this does not mean that all employees provide personal care services or would be required to use EVV.

Survey results for provider type ‘Other,’ includes responses from provider types who would never be required to use EVV.

thEmES / oBSErVationS

GEOGRAPHIC DISTRIBuTIOnRespondents indicate a heavy mix of urban and rural members. Most provider types indicate more rural than urban members. This is important as it relates to connectivity. It is important to note the survey did not define urban versus rural.

EDuCATIOn/TRAInInG There appears to be a great lack of understanding on what EVV is, and why it is being implemented. Survey responses indicate there is a perception that this is an IME led initiative. Education on the requirements of the 21st Century Cures Act will be important to ensure cooperation and collaboration.

Education on cost is going to be important. Providers will expect the worst if they have no idea

what to expect.

Providers will need a lot of training. Based on survey responses, providers want support to be provided by the IME, the MCOs and the providers’ corporate entities. It appears there is a preference for IME to take the lead in providing support and training.

Survey results for provider type ‘Other,’ includes responses from provider types who were not targeted for completion of the survey and would never be required to use EVV. This indicates there will be a need for education and training for providers who will not be required to use EVV, to make them aware that they are not impacted. It may be beneficial to leverage providers with EVV experience to advise on the business process. The IME may partner with them to assist their peers with implementation.

Many respondents answered ‘Don’t know,’ to open ended questions which emphasize the need for robust communication and education. This must be ongoing and may require some sort of ‘help desk.’

There were a lot of negative responses in the open ended questions. This highlights the need to educate on the positive aspects of EVV, such as quicker claims processing, reduced administrative burden, etc. Many CDAC agencies don’t use a clearinghouse. Educating on the benefit of streamlined claims and billing will be helpful in gaining support and participation from providers.

Many providers are questioning whether they will be required to use multiple EVV systems. This may be a source of frustration and will require significant education.

Based on responses, we will need to get training to providers when they have availability, such as a webinar they can watch and re-watch on their schedule.

Early education on ‘EVV 101’ is recommended.

COSTCost is clearly a concern by both, providers who will be required to use EVV and those who will not. Costs seem to be a significant factor for providers.

There appears to be an expectation that the IME or MCOs will bear the cost of implementing and operating an EVV system.

PROVIDER nETWORk Some respondents indicated they may no longer participate because of the EVV requirement. In the past, when we have changed reporting requirements, we’ve seen a drop off in providers. We need to be aware that some providers may quit if they have to implement EVV or can’t afford to implement EVV. We need to make sure if we lose providers, we are aware of who

we lost and make plans on how to get that member moved to other providers who continue to provide services.

ExISTInG InFRASTRuCTuREThe most common piece of existing infrastructure to support an EVV program is a computer with internet access.

An Electronic Health Records (EHR) program appears to be one of the most common pieces of existing infrastructure to support an EVV program according to respondents (excluding CDAC). We are not sure whether these EHR programs support full EVV functionality.

There appears to be some prevalence of mobile internet access.

Access to member landlines or smart phones appears to be very limited.

Survey results indicate a need for devices. Providers may expect MCOs to pay for these devices.

There appears to be a lack of internet access. We need to have solutions for solving connectivity issues.

ExISTInG EVV SySTEM (OR SIMILAR)There is confusion regarding what EVV is. Many respondents cited systems as EVV that are not EVV.

Providers who named an existing EVV (or similar) system, identified 22 unique systems. The IME researched those systems and found 11 unique systems that appear to include all of the functionality that will be required for EVV. The analysis of these systems EVV capabilities is based on claims published on the software makers website. See the analysis on the following page. The IME should identify how prevalent these 11 systems are, and work towards ensuring the EVV system is able to work with these existing systems. We need greater insight into who is using these 11 systems, and whether providers have activated all the functionality.

SuMMARySurvey results validated a lot of assumptions of the internal IME work group. Counter to the assumptions of the work group, landlines appear to be far less prevalent than we expected. The results demonstrated a general fear and lack of understanding. The survey results help catalog the risks and opportunities. This helped identify areas that need to be addressed to allay provider concerns. It will be a challenge to prepare providers for a level of oversight they have not had in the past. The results raise questions about potential expenses and how those expenses are going to be paid.

System determinations may be difficult. What works for one provider/service type may not work for all.

A primary question is whether there will be 1 or 3 systems, possibly 4. This will inform what types of devices will be needed, what training will be needed and what costs will be. until we know the answer to this key question, education and calming fears will be a challenge.

2 THat depends on if we get to continue with AxisCare or have to switch entirely to something new.

It depends if each MCO has their own - which would be ridiculous because we'd have to train staff

on all 3 or schedule only staff who know each system with those clients. It would be a scheduling

nightmare. Some caregivers need a lot of "handholding." They don't make much money and don't

always understand the importants of correct documentation and clocking in/out.

CGs can use their smartphones it shouldn't cost much. However, if we have to use ipads or

something else - that's a huge cost. Turnover is high too across the industry so just tracking

staff to monitor this program. We participated in the pilot with United. It never got off the ground

because they had difficulty getting the GPS system to work. Additionally, our staff start at various

places in the community, not always at the members home. There was also the problem, during

the pilot, that was never worked out regarding multiple members living together and multiple staff

clocking in at same time. Staff will not be willing to use up their data for this program and agencies

will funding to help cover these costs. Other states that have already implemented EVV, confirmed

this.

3 Again the technology costs for this would approach $100,000 for our agency. Staff will not want to

use their cell phones to without being reimbursed for data usage. We do not have computers in all

of our locations. Our staff do not start work at the member's location frequently, they are in the

community. Using the members' computers or smart phones should not be considered, it is for

staff not for members. That is a huge infringement.

2 All of our staff. There does not seem to be any other place for comments so I will make them here.

To require EVV at all HCBS locations is absurd. Most of our locations have multiple members and

multiple staff. To require all staff to register themselves for each of the members are very

cumbersome. Again, we do not start working with several of our members at their homes. We

meet them in the community often at a variety of locations. That is what HCBS is about. EVV may

make sense for Home Health agencies and individual CDAC providers but not for all HCBS

services.

4 This will be incredibly expensive, upwards of $50,000 or more. We would need to hire additional

staff to monitor this program. We participated in the pilot with United. It never got off the ground

because they had difficulty getting the GPS system to work. Additionally, our staff start at various

places in the community, not always at the members home. There was also the problem, during

the pilot, that was never worked out regarding multiple members living together and multiple staff

clocking in at same time. Staff will not be willing to use up their data for this program and agencies

will funding to help cover these costs. Other states that have already implemented EVV, confirmed

this.

effective. We have nearly a third of our staff in the field at any one time. We cannot afford to

provide devices/data plans to all of them. We have looked into this on numerous occassions.

4 All of our staff. There does not seem to be any other place for comments so I will make them here.

To require EVV at all HCBS locations is absurd. Most of our locations have multiple members and

multiple staff. To require all staff to register themselves for each of the members are very

cumbersome. Again, we do not start working with several of our members at their homes. We

meet them in the community often at a variety of locations. That is what HCBS is about. EVV may

make sense for Home Health agencies and individual CDAC providers but not for all HCBS

services.

6 i would like to think the expense woudl be covered by the payor since it is an additonal

requirement which would require additiona expense. We also need clear expections and a timeline

to implement effectively
