SANCTION FOR TEST: is in force on PLANT/HIGH VLAGE apparatus-No other SAFETY DOCUMENT shall be in force on the same item of plant/apparatus.LL. SENIOR AUTHORISED PERSON(SAP): Competent Person nominated in writing by the location General Manager/Add/Deputy to carry out these dutiesMM. Wherever reasonably practicable safety documents should be issued to the PERSONS doing the work provided he has adequate knowledge from safety point of view (VSTPP-e.g.. CMP ,Labour etc.) (GAS Plant-foreign expert)NN. If several groups are working SAFETY DOCUMENTS shall be isued directly (face to face) to recipient who shall responsible for the working party.

OO. PERMIT TO WORK (Preparation)SAP cannot physically check because of remoteness but safety precautions can be recorded in detail in SAP.PTW original to be retained by applicant & recorded in tune with LMI with precautions in record.SAP can check physically check location-Satisfactory/DoubtableAppropriate KEYS-FUSES or other means of securing isolations shall be placed in KEY-SAFELMI must specify what is safe custodySAP shall secure the local KEY SAFE by operating his CONTROL-KEY to lock the KEY-SAFE under his control.SAP when considered necessary can add/delete safety precautions to avoid danger.It basically concerned with SAFETY FROM SYSTEM &not general safety.

PP -ISSUE & RECEIPThaving recorded the items to be issued the SAP shall sign & issue the permit with a key safe together with item noted.QQ- TRANSFERWork not completed-Necessary to TransferRecord on reverse side of PTW-by recipient & Newcomer with all relevant instructions & briefing.RR- SUSPENSIONWhen it becomes necessary to temporarily discontinue work with & withdraw for limited period of time from Plant/APP without changing the safety precautions then suspension is used. eg Boiler Case for RadiographySS-CLEARANCE & CANCELLATION SAP-ensures withdrawal of working persons, earthen, T&P systems, work area clearance, Guards & access doors refitted.
