Use Case

• Public health creates a care plan for a child to make this available to the providers

Information from newborn hearing screening is used by Public Health to create an initial Early Hearing Care Plan for each child. At a newborn /infant’s first well-child check, the PCP reviews the Early Hearing Care Plan received from Public Health to determine the next hearing care steps for each child. These steps for the PCP include a review of screening results and known risk indicators for hearing loss with the family. An assessment of additional risk indicators for hearing loss is also completed by the PCP. Based on results and risk indicators for hearing loss, the PCP is provided best practice guidance from Public Health for ongoing hearing health care.
