How did we create North Lincolnshire’s Education, Health and Care Plan?

A working group representing parents, schools, colleges, early years providers, health and local authority education / social care teams met to consider the task.

What did the working group do?thought about the needs of children & young people, parents and professionals and how the plan could represent their views

looked at what had been created by pathfinder local authorities and identified the best elements within these plans

developed and refined North Lincolnshire’s Education, Health & Care Plan through 10 versions which were refined by consulting the partners and the teams they represented.

We consulted more widely

by talking to parents of pupils with Statements of SEN at consultation meetings

by talking to children in mainstream and special schools and the Disabled Children’s Forum

North Lincolnshire’s Education, Health & Care Plan is child, young person and family focussed is written from the child & young person’s viewpoint identifies need and celebrates achievement is aspirational identifies outcomes expected in the longer term and also within the lifetime

of the annual plan ensures we are all accountable for delivering the outcomes puts children, young people and their families at the centre of the planning

process

Principles of Person Centred Planning the person is at the centre of the planning process family and friends are partners in planning the plan shows what is important to a person now and for the

future and what support they need the plan helps the person to be part of a community of their

choosing and helps the community to welcome them the plan puts into action what a person wants for their life and

Our parents have been very positive about it.

I really like the new Annual Review meeting. It’s better for the child. Dillon really likes being part of the planning and looks

forward to his “special meeting”.

Visiting professionals have also found the new format useful, thorough and more personal. I have found the format relaxed,

refreshing and inclusive. I am able to get a much clearer picture of how the the child is getting on at home and at school.

At Home – having my special space – my bedroom, having Lady Bella to play with and look after. Hoovering when I feel upset or cross.

At School – Literacy in the unit – I can think better. It is quiet. I like the stories and dvd’s. Reading every day.Big Maths in a small group. We go slower than in class. I can have more thinking time and play more games.Friends’ Group.

•Learn its . I need to practise more at home with Mum. I don’t need to use the timer. I am calmer when I have thinking time .

•I need to remember to be kind to other people. I need to remember to use kind words about me and other people. Circle Time and Friends’ Group will help with this.

•I need to try and stay calm when the work is a bit hard. I need to be proud and happy when I do well.

•I need to try not to get angry or upset if I get something wrong. •I need to have another go.
