I, Linda S. Cook, being duly swom, dispose and say as follows:
1. I am the Respondent in the above-captioned case.
2. Pursuant to the Order of this Court of July 11, 2007, I, prepared the AFFIDAVIT
OF COMPLIANCE (See Exhibit A) and gave it to Mary Mahnen, our office clerk, to
FedEx overnight on August 9, 2007.
3. Mary Mahnen on August 9, 2007 FedEx it overnight to the Ohio Supreme Court,
30 East Broad Street, Columbus, Ohio 43266-0419, the address listed in the 2004-2004
Toledo Bar Legal Directory. (See Exhibit B)
4. The AFFIDAVIT OF COMPLIANCE was received on August 10, 2007 at 10:27
am by J. SUTTON. (See Exliibit C)

5. I received a letter dated August 14, 2007 from the Ohio Supreme Court, JoElla,
Deputy Clerk, stating that they did not receive the AFFIDAVIT OF COMPLIANCE until
August 13, 2007 and because it was late they could not file it. (See Exhibit D)
6. I pulled the confirmation notice and read it to JoElla and that is when I realized
Mary Mahnen had FedExed it overnight to the wrong address. (See Exhibit C)
7. JoElla explained to me that I could do nothing until I received the ORDER TO
SHOW CAUSE from the Ohio Supreme Court.
8. I received the notice of Certified mailing on September 15, 2007.
9. I picked up the Certified mailing of the ORDER TO SHOW CAUSE on
September 17, 2007.
10. Wherefore, I am submitting my ANSWER TO SHOW CAUSE and resubmitting
my AFFIDAVIT OF COMPLIANCE for filing. (See Exhibit E)

AFFIDAVIT OF COMPLIANCE OF RESPONDENT, LINDA S. COOK
I, Linda S. Cook, being duly sworn, dispose and say as follows:
1. I am the Respondent in the above-captioned case.
2. Pursuant to the Order of this Court of July 11, 2007, I have notified all clients
being represented in pending matters and any co-counsel of my permanent disbarment
and my consequent disqualification to act as an attorney after the effective date of the
order, and in absebce of co-counsel, also notified the clients to seek legal service
elsewhere, calling to any urgency in seeking the substitution of another attomey in my
place.
3. I have delivered to all clients being represented in pending matters, any papers or
other property pertaining to the client or have notified the clients or co-counsel, if any, of

a suitable time and place where the papers or other property may be obtained, calling
attention to any urgency for obtaining such papers or other property.
4. I have refunded any part of fees or expenses paid in advance that are unearned or
not paid, and I have accounted for any trust money or property in my possession or
control.
5. I have notified opposing counsel in pending litigation, or in the absence of
counsel, the adverse parties, of my disqualification to act as an attorney after the effective
date of the Order issued by this Court, and filed a Notice of Disqualification with the
Court or agency before which the litigation is pending for inclusion in the respective
files.
6. I have sent all notices required by this Order by certified mail with a return
address where communications may thereafter be directed to me.
7. I have retained and maintained a record of the various steps taken by me pursuant
to the Court's Order.
8. I am surrendering my Certificate of Admission to Practice to the Clerk of this
Court with my Affidavit.
9. I am unable to surrender my Registration card for the 2005/2007 bi-annum as I
cannot locate it.

10. I am, with this Affidavit, paying to the Clerk of Court the sum of 2,372.10 as
ordered by the Court.

The affidavit was received on August13, 2007 but, unfortunately, was due on August 10, 2007. The Clerk's Office isprohibited from filing untimely documents by S. Ct. Prac. R. XIV, Sec. 1(C). Motions towaive the timeliness requirement are also prohibited by the rule.
If the Court issues an order to show cause why you should not be held in contempt forfailing to timely file an affidavit of compliance, you will be pennitted to file an affidavitof compliance along with a response to the order to show cause.
