BEING A BY- LAW to authorize The Corporation of the City of Timmins to enter into anagreement with Greer Galloway Group respecting bridge and culvert inspections, in the City ofTimmins.
WHEREAS Council of The Corporation of the City of Timmins considers it desirable to enter intosuch an agreement with Greer Galloway Group.
NOW THEREFORE THE Council of The Corporation of the City of Timmins enacts the followingas a By-law:
1. THAT The Corporation of the City of Timmins enter into an agreement with GreerGalloway Group respecting bridge and culvert inspections, in the City of Timmins, for thesum of $71, 804.00 + HST, a copy of which agreement is attached hereto and marked asSchedule "A" to this By- law.
2. THAT the Mayor and Clerk be and are hereby authorized to execute the said agreementon behalf of the Corporation and to affix thereto the Official Seal of the Corporation.

P 1. INTRODUCTION AND UNDERSTANDING OF RFP OBJECTIVES
1. 1 Overview
The City of Timmins is seeking a qualified and experienced Engineering Consultantto complete detailed visual inspections and prioritized maintenance / rehabilitation
t plans for the City' s structures. The City has also requested that a detailed deckcondition survey and structural evaluation be completed at the Airport Road Bridge,that a fatigue inspection and remaining fatigue life analysis be completed at theFeldman Road Bridge, and that an updated crack map be completed at thePorcupine River Bridge.
The main objective will be to appraise the current condition of the City' s bridges andculverts and to identify maintenance, repair and rehabilitation needs so as to
I. maintain the infrastructure in a safe condition and to protect and prolong its usefullife. Based on the findings of the inspections a 10 year budget plan for themaintenance and rehabilitation of the infrastructure will be developed.
The work will also serve to satisfy the City's legislated inspection requirements underthe Public Transportation and Highway Improvement Act and Ontario Regulation160/02.

F1.2 Scope of Work
The scope of work for this assignment involves the tasks outlined below.
a) Arrange and attend a project kick off meeting with City staff to review andcomplete details of the proposed work. Agendas and minutes will be circulatedas required.
b) Completion of a detailed visual inspection of forty-five ( 45) structures includingfifteen ( 15) bridges, two ( 2) overpasses, four (4) pedestrian bridges, and
twenty-four (24) culverts. Each detailed visual inspection will be completed inaccordance with current OSIM procedures and forms, and will include but not
be limited to the following:
i) Inspections carried out by an experienced structural engineer who islicensed by the Professional Engineers of Ontario
H) An element-by-element assessment of each structure
Hi) Assessment of the severity and extent of material defects for each element
iv) Assessment of condition state for each element
v) Assessment of suspected performance deficiencies for each element
vi) Identification of maintenance needs on each structure
c) Individual OSIM Inspection Report ( cerlox bound) for each of forty-five (45)structures signed and sealed by a Professional Engineer licensed in theProvince of Ontario, will be completed on the current OSIM Forms and willinclude:
i) Annotated high resolution photos of each of the major bridge elements
including plan view, elevations, and underside of structures. The structurenumber will be used to identify which structure the photo applies to.
ii) Individual pictures of all bridge elements / components and major defectsand deterioration
iii) Detailed description of the structure including composition and
measurements
iv) Quantitative and qualitative records of material defects categorized underdefined condition states, suspected performance deficiencies and
maintenance needs for each element
v) Identification of areas of the structure where routine maintenance is required
vi) Identification of additional detailed investigations and associated cost
estimates as required

viii) All comments/ recommendations made in the 2011 OSIM inspection reportswill be commented on in our updated 2013 OSIM inspection reports.
d) Fatigue inspection and remaining fatigue life analysis of the truss panel weldsof the Feldman Road Bridge ( T5A) including the detailed visual inspection of allwelds, magnetic particle testing ( MPT) of all cracked welds and magneticparticle testing of a minimum of 15% of the total number of fatigue prone/critical
welds. This work will be completed by l& F Engineering Corp, an expierencedweld inspection engineering company. In order to calculate the remainingfatigue lift, the AADT and Truck % for Feldman Road will be required. Our fee
is based on the assumption that the City will provide us this information. If the
f- City cannot provide the required information, Greer Galloway can arrange tohave traffic counts completed at an extra cost.
e) Detailed deck condition survey of Airport Road Bridge ( TW- 1A) completed byrGolder Associates. The actual configuration and material strength of the
superstructure will be confirmed through destructive testing and sampling. Astructural evaluation of the bridge will then be completed in accordance with the
Canadian Highway Bridge Design Code ( CHDBC). The detailed deck condition
survey will include the following:
A close up visual inspection of the asphalt wearing surface of the bridge
A half-cell corrosion potential survey of the bridge deck on a 1. 5m grid
p A delamination survey carried out on exposed concrete elementsincluding barrier walls, curbs, and sidewalk as applicable
Surface deterioration mapping of the asphalt wearing surface
Extraction of six ( 6) sawn asphalt samples from each bridge deck
A covermeter survey of the exposed concrete deck within the sawnasphalt sample areas to determine the spacing of reinforcing steel
Extraction of ten ( 10) cores from the bridge deck for visual examination
and to confirm the size of reinforcing steel present in the deck
Laboratory testing of selected cores including compressive strengthtesting, chloride ion content measurements and air void systemparameter testing.
Two ( 2) hardcopies and one ( 1) electronic copy of all reports will beprovided to the City.
f) Updating crack mapping of Porcupine River Bridge at Frederick House LakeRoad.

vi) Separate individual reports will be prepared for the fatigue inspection oftruss panel welds of Feldman Road Bridge, and for the deck condition
survey and structural strength evaluation at Airport Road Bridge. The
reports will include findings, results, detailed design calculations, and
recommendations.
h) Completion of a prioritized ten year budget plan for the maintenance /
rehabilitation of the City' s structures including a prioritized ten year repair /replacement work schedule.
i) All inspection data will be entered into the City of Timmins MDW databasehoused by OGRA).
j) Verifying and updating Table 1 — Appendix A of the Request for Proposal ( ENG
2013- 08) following completion of all detailed visual inspections and structuralstrength evaluations.
1. 3Documentsi
The City will be provided with two ( 2) draft reports ( 1 bound copy + 1 electronic) and
three ( 3) final reports ( 2 bound copies + 1 electronic copy) signed and sealed by alicensed Professional Engineer in the Province of Ontario. The electronic reportssubmitted will be in the original file formats ( word, excel, autocad) PDF file formatswill not be used. Three ( 3) separate binders will also be submitted ( one for each ofthe three parts of Timmins) and will include the completed OSIM inspection forms for
all the structures. A divider with name tag will be used for each structure in thebinders.

2. 1 Company Profile
The Greer Galloway Group Inc. was established in 1965 and offers a completerange of professional engineering consulting services. Our compliment of
professional staff includes Civil, Mechanical, Electrical, and Structural Engineers as
well as technologists and support staff. Today, the company is comprised of officesFz
in Peterborough ( Head Office), Belleville, and Kingston.
Greer Galloway boasts a structural division which has been providing bridgeinspections, design, capital planning, and construction management for over 40years. In addition to the structural group, Greer Galloway has a road design group,building services group and two additional divisions: WaterSpec and Terraspec.WaterSpec is based in the Belleville office and focuses on water and wastewater
plant operations, compliance with provincial regulations and objectives, facility
troubleshooting, design and engineering reports. Terraspec is based in
Peterborough and is a geotechnical company which specializes in soils andfoundation investigations, materials testing and environmental investigations. TheGreer Galloway Group also has a partner group, Jp2g based in the Ottawa area.Between all offices, The Greer Galloway Group Inc. employs a staff of approximately100 persons.
The Greer Galloway Group Inc., is registered with Professional Engineers Ontarioand holds a Certificate of Authorization to practice the disciplines of civil, structural,

Completion of twelve OSIM bride inspections for the Town of Smiths Falls in2013.

Completion of forty-two (42) OSIM bridge inspections as well as structuralstrength evaluations for seven ( 7) structures for the City of Timmins in 2011 .

r 3. QUALIFICATIONS AND EXPERIENCE OF TEAM MEMBERS
r For this project the responding office will be Greer Galloway Belleville. The presentnumber of employees in the Belleville office is thirty (30) consisting of a dynamicprofessional group of key management personnel, experienced design engineers,technologists, and technicians and dedicated support personnel. The professional
expertise and dedication of the staff allows Greer Galloway to pay close attention toclient needs and provide professional, cost effective, and forward thinking solutions.
All team members will be available to ensure that this project meets all of therequirements and expectations of the City. Brief descriptions of key personnel andproject roles are outlined below. Expanded resumes can be found in Appendix A2.
Steve Blakey, P. Eng. — Senior Project Advisor
Steve is the Branch Manager of Greer Galloway' s Belleville office. He has over 25years' experience in municipal engineering. Steve has completed numerous
engineering investigations including geotechnical, class environmental assessments,servicing studies, infrastructure rehabilitative studies, as well as detailed design andconstruction management of municipal infrastructure. He has managed projects of
several thousand dollars to several million dollars. Steve' s responsibilities range from
project co-ordination, preliminary design and design management, environmentalassessment and implementation, project budgeting and scheduling, detailed
specifications and tendering, as well as construction administration. For this
assignment, Steve will be the Senior Project Advisor and will interact with and advise
the project team throughout the course of the assignment.
Quan Tan, P. Eng. — Project Manager/Senior Structural Engineer
Mr. Tan is a University of Waterloo graduate who completed a Masters in StructuralEngineering with a focus on bridge engineering. Mr. Tan has over eight years ofexperience in structural engineering and has an extensive knowledge of the currentstructural inspection and rehabilitation technologies and standards. Mr. Tan has
completed the design and inspection of numerous bridges for our clients in the
IFPsurrounding area. Mr. Tan will act as the Project Manager and primary inspector onthis assignment overseeing all project activities and approving final reports.
Jennifer Young, BaSc —Junior EngineerMs. Young is a University of Toronto graduate who completed her degree with a focusin Structural Engineering. Ms. Young has completed numerous OSIM bridgeinspections, participated in the rehabilitation and replacement of bridges andconducted structural evaluations per CHBDC. Ms. Young will assist with the structureinspections, structural evaluations, and completion of all reports.

April Chamberlain — Technologist
Ms. Chamberlain is a graduate of Loyalist College who completed an advanced
diploma in Civil Engineering Technology. Ms. Chamberlain has formal OSIM StructureInspection training through the Ontario Good Roads Association (OGRA) and hasexperience with OSIM inspections, reports and compiling data into the Municipal DataWorks system ( MDW). On this project Ms. Chamberlain will assist with the completionof the reports, and the ten year plan.
Vlad Rogachyov, P. Eng. - I & F Engineering Corp.For this assignment, a team from I & F Engineering Corp. in Concord, Ontario willcomplete the magnetic particle testing ( MPT) and fatigue analysis of Feldman Bridge.I & F personnel have remarkable Canadian and international experience in buildingsand bridges design, inspection and construction administration, as well as research
rand development (R& D) of engineering products. Their engineers and technicians arecapable of providing a full range of engineering and consulting services. I & F will be
available to complete the works within the City' s required timeline.
Golder Associates — Bridge Deck Condition Survey for Airport RoadFor this assignment, the Greer Galloway Group Inc. will be carrying Golder Associatesfor the Bridge Deck Condition Survey at the Airport Road Bridge. Golder Associateswas founded in Toronto, Ontario in 1960 and offers services in soil mechanics,
foundation engineering, environmental management, planning and design as well asmany others. The team from Golder Associates is listed below and all members willbe available during the City of Timmins required time frame.
Andrew C. Balasundaram, Pavement and Materials EngineerGolder' s work will be carried out under the overall direction of Mr. Andrew C.Balasundaram, a Pavement and Materials Engineer and Principal in Golder'sMarkham office. Mr. Balasundaram will provide technical guidance as required, will
ensure that adequate resources are assigned to the project, and will review and
assign the final report.
Stephen R. Boyd, Senior Materials Engineer
Mr. Stephen R. Boyd, a Seniors Materials Engineer based in Golder's Whitby officewill be the Project Manager. Mr. Boyd has over 17 years of experience related to
concrete and materials engineering, including condition surveys of bridges and otherstructures and is approved in RAQS for Bridge Condition Surveys.
Philip Barnhill, E. I. T. ( Pavement and Materials Engineer)Mr. Philip Barnhill, based in Golder' s Whitby office, will be the Field Crew Leader andwill work under the direct supervision of Mr. Boyd. Mr. Barnhill will supervise the fieldcrew, co- ordinate the field activities and prepare the reports. Junior engineers, as well

4. APPROACH AND METHODOLOGY
r The inspection work will be completed by a two ( 2) member team using appropriateequipment (e.g. watercraft, chest waders, man lift, ladders, etc.) and safety procedurese. g. floatation vests, harnesses) as necessary.
t It is intended that the field inspection portion of this project will be scheduled so as to
correspond with the project kick off meeting. The visual inspections will be completedover a continuous one week period followed immediately by in office work on thestructural evaluation and reports.
Based on the results of the visual inspections and the reports a ten year prioritizedbudget plan and schedule for the required repairs / maintenance and rehabilitation will
be completed. Budget estimates will be based on recent project costs for similar work.Greer Galloway personnel will work with the City to ensure prioritization andrecommendations are appropriate for current and future use requirements for eachstructure.
All electronic reports will be submitted to the City in Excel, Word or AutoCAD formats.Following approval from the City of the draft report. all OSIM data will be uploaded ontothe MDW database and all final reports will be completed and submitted to the City.
While conducting the detailed visual inspections, quantities and measurements will becollected for the Airport Road Bridge which requires a structural strength evaluation.The information collected will be used, in conjunction with the results of the detaileddeck condition survey, to analyze the structure in terms of structural strength and loadcapacity following the guidelines in Section 14 of the Canadian Highway Bridge DesignCode.
fL All reports and evaluation work will be internally reviewed to ensure accuracy and
completeness.
t The work will be carried out in accordance with the Occupational Health and Safety Act.And in accordance with the Ontario Traffic Control Manual Book 7 — Temporary
Conditions, as required). The City' s confined space policy will be adhered to asrequired. Greer Galloway personnel are trained in first aid, fall arrest and confinedspace entry. Roadwork and Inspection work will be conducted in accordance with theOntario Traffic Control Manual. The City will be informed, in advance of the inspections,the days on which the inspections will be conducted.

6. 1 Quality Assurance and Control
The Greer Galloway Group implements a quality control plan on all engineeringprojects. A copy of the Greer Galloway Quality Assurance and Control plan isavailable upon request.

6. 2 Insurance
The Greer Galloway Group Inc. carries comprehensive general liability andautomobile insurance, and professional liability insurance in the amounts of
5, 000,000 (each for general and automobile) and $ 2, 000, 000 ( per occurrence),respectively. Detailed insurance information & WSIB can be found in Appendix A5.
6. 3 Health and Safety
7 The management of The Greer Galloway Group Inc. is vitally interested in the healthand safety of its employees, and are committed to taking every reasonableprecaution for the protection of workers. The Greer Galloway Group has its ownOccupational Heath and Safety Plan, which is implemented to comply with theOccupational Health and Safety Act, and is available upon request.
6. 4Summary
We trust the foregoing demonstrates the Greer Galloway Group' s understanding andrappreciation of the City of Timmins professional service needs.
We are well trained and very knowledgeable in this field. We have completed
hundreds of OSIM inspections and associated reports, structural evaluations, andprioritised capital plans. We also have vast experience in structure rehabilitation andreplacement with numerous Municipalities and we look forward to demonstrating thevalue of service we can provide to the City of Timmins.

Standard OSIM inspections were carried out on nine ( 9) municipal structures in 2011and twelve ( 12) municipal structures in 2013. Measurements and evaluations were

Municipality of Tweed OSIM Inspections - 2008
Fifty two ( 52) structures were OSIM inspected and evaluated for the Municipality.Several bridges had load restrictions and those bridges were given additional
attention to determine the applicability of the existing load limit, or of its need to bechanged. The assignment included a summary report with prioritized work andestimated costs. Bridge Condition Indices were also calculated for each structure for
fv• this assignment. Individual weight restriction by-laws were also prepared for thosebridges with load restrictions. Two steel bridges ( circa 1930), were analyzed in
detail including obtaining and testing steel samples. Two other bridges were foundto be structurally unsound and emergency were implemented to extend the servicelife of those structures.

31. PROPONENTS TO INVESTIGATE
Consulting firms submitting a proposal shall satisfy themselves by personal examinationof the site and, by such means, as they prefer, as to the actual conditions andrequirements of the Work.

BEING A BY- LAW to authorize The Corporation of the City of Timmins to enter into anagreement with Greer Galloway Group respecting bridge and culvert inspections, in the City ofTimmins.
WHEREAS Council of The Corporation of the City of Timmins considers it desirable to enter intosuch an agreement with Greer Galloway Group.
NOW THEREFORE THE Council of The Corporation of the City of Timmins enacts the followingas a By- law:
1. THAT The Corporation of the City of Timmins enter into an agreement with GreerGalloway Group respecting bridge and culvert inspections, in the City of Timmins, for thesum of$ 71, 804.00 + HST, a copy of which agreement is attached hereto and marked asSchedule "A" to this By- law.
2. THAT the Mayor and Clerk be and are hereby authorized to execute the said agreementon behalf of the Corporation and to affix thereto the Official Seal of the Corporation.

ADMINISTRATIVE REPORT
Bridge and Culvert Inspections
SUBJECT:
Complete bridge and culvert inspections to comply with provincial regulations
PURPOSE:
The City of Timmins last inspected its bridges and culverts in 2011. An inspection is
required every two years as mandated by the Province of Ontario. This report seeks to
receive approval from Council to award this work to a consulting firm and bring ourinspections to date.
EXPLANATION:
The City of Timmins has 45 structures which require an inspection every two years in orderto comply with Provincial Regulations. The 45 structures are broken down as follows:
15 bridges
2 overpasses
4 pedestrian bridges
24 culverts with spans greater than 3 metres
This assignment will provide an overview of the current condition of the City' s bridges andculverts and serve to identify maintenance, repair and rehabilitation needs to maintain theinfrastructure in a safe condition and to prolong its useful life. Based on these inspections,a 10- year budget plan for the 45 structures will be reviewed and updated.
This work will ensure the City' s compliance with the legislated requirements under thePublic Transportation and Highway Improvement Act and Ontario Regulation 160/02.
Only one proposal was received from the Greer and Galloway Group ( Belleville, Ontario).This consultant completed the last inspection of our bridges in 2011 and they are veryfamiliar with our infrastructure.
The cost to undertake this exercise is $ 71, 804 plus HST.

All proposal submissions received will be opened at City Hall. As this is a Request for Proposal
document for which a number of criteria will be evaluated, only the names of proponents who
have submitted a proposal will be identified at the opening, and the price submitted will not bedisclosed.
The City may accept a proposal in whole or in part, whether the submitted price is the lowest ornot, and may reject any or all proposals. There will be no requirement of this RFP, implied orotherwise, that the proposal representing the lowest submitted price would be selected orpreferred.
