Individuals are expected to utilize electronic procedure forns while performaning thier job assignnents. While there are controlled electronic files forprocedures, which include procedure forms, these are read only and do not allow the user to utilize the procedure forms to perform workelectronically. Individual users/groups can (and do) obtain electronic procedure forms from the uncontrolled MS WORD electronic procedure files.These forms can be utilized after confirming they match the current revision of the controlled procedure.
However, there have been Instances where Indiduals have performed work utilizing the wrong revision of electronic procedure forms. Creating andmaintaining controlled procedure forms In electronic format would eliminate this Human Performance Trap.

20031760 Oxygen Injection System shut down results in Action level 1 condition forFeedwater dissolved oxygen

At approximately 1630 the oxygen injection system tipped off-ine resulting In a decrease In condensate and feedwater D.O. to approximately 17 ppb.*The problem was noticed In the control room based on fluctuations with the D.O. recorder. A Chemistry technician reset the Oxygen Injection Systemat 1950 at which time Feedwater dissolved oxygen was restored to >30 ppb . Note: Per the EPRI Water Chemistry Guidelines, Action Level I forfeedwater D.O. Is entered when dissolved oxygen Is <30 ppb.

Procedure OP 3126 Rev.16 Apendix E page 5 directs replacement of 4 KV SWGR control fuse by Installing 15 amp fuses for the close circuits and35 amp fuses for the trip circuits. The switchgear Contrd circuit fuse selection verification list shows the fuses for breaker 3V4 (8 are listed) as sizeNON-15. CWD 325 shows the fuses as 15 amp fuses.MM 98-033 Installed 15 amp fuses In the affected circuit for breaker coordination. WO 02-001742-000 verified that the proper fuses were installedunder MM 98-033. Refer to attached e-mail for System Engineering operability assessment.

W mailed BVY 03-70, 7131103, overnight to Bob Pulsifer. On 8/1103, prior to mailing the original to the DCD, I verified the pages of the calculations.
Following are the errors that were found when packaging the AST submittal for mailing this moming. Bob Pulsifer's copy was mailed out 7/31103prior to Identification of these minor packaging errors.
1. Post LOCA Suppression Pool pH - Non-Proprietary version: last page is *page 15 of 19.' Pages 16 thru 19 were Intentionally deleted, since theyprovided Exhibits 1-4, which were proprietary. Therefore, on the bottom of page 15 of 19, I wrote, 'Thus, pages 16 thru 19 are not provided."Replaced page 15 of 19 with the attached replacement page..
2. Calc VYC 2312, Post LOCA Drywell High Range Monitor Response..., pages 190,191, and 192 were duplicated. We removed these duplicatedpages. Removed these duplicated pages from Pulsifer's copy.
3. Radiological Evaluation of a DBA LOCA, Proprietary Version: Between pages 56 and 57, there Is an unnumbered figure, Inadvertently Inserted Ina few of the copies. This figure was removed, as It is not part of the submittal.
On August 12. the NRR PM, Bob Pulsifer phoned me with Identification of additional errors. His copy (the copy that was sent out after 6 pm 7/31/03to make the deadline) was missing both the non-proprietary and proprietary versions of one of the calcs. He was able to get the caics from DCD.The NRR reviewer noticed In a quick page-through that the appendices in cabc QA06 are labeled 0A05. We are still attempting to get resolution onthis matter from the contractor, Pulstar.

The number of administrative errors that occurred with the AST submittal is unacceptable.
Licensing should ensure that submittals to the NRC are complete and correct prior to mailing.

At 1330 a Secuirty Officer was doing a routine gate opening at the main vehicle access gate. The first gate Is controled by a electric motor andwas opened for him. The secound outside gate is controled by pushing it open manually. In doing this the officer pushed the gate open and wMen itreached the end of its run It bounced back at him. The officer again pushed it open. He then turned to open the the vehicle barrier gate with his backto the sliding gate. The vehicle barrier gate takes a hard pull to get it to initially move. As the officer tugged and started walking with the barrier therolring outer gate started to roll close and the officers left wrist became pinched between the vehicle barrier and the rolling gate. The pain from injurycaused the officer to become light headed. A medical emergency was called and the officer was looked at. The officer refused medical treatment andwas given a Icepack and water. A non motorized gate with no locking device in the open position were contributing factors to this accident.
