C H E C K L I S T A N D R E Q U I R E M E N T S    

Introduction  The  revised  Graduation  Transitions  program  is  designed  to  support  every  BC  graduate  in  the  acquisition  of  the  attributes  that  characterize  students  graduating  in  all  schools  across  British  Columbia.  These  attributes  include  intellectual  development;  social  development;  and  career  development  as  prescribed  on  the  website  http://www.bced.gov.bc.ca/graduation/.    Grad  Transitions  12  is  a  requirement  for  graduation.  Students  who  do  not  complete  the  requirements  of  the  program  will  not  be  eligible  for  graduation.    Graduation  Transitions  is  intended  to  prepare  students  for  a  successful  transition  to  life  after  secondary  school.  In  order  to  meet  this  goal,  Graduation  Transitions  encourages  students  to:  

•   take  ownership  of  their  own  health  and  learning  •   examine  and  demonstrate  connections  between  their  learning  and  their  future  •   create  a  plan  for  their  growth  and  development  as  skilled,  healthy,  knowledgeable,  

participating  citizens  •   exhibit  attributes  of  a  BC  graduate  

 Graduation  Transitions  consists  of  FIVE  components:  

•   Introduction  (Personal  Information)  •   Personal  Health  •   Community  Connections  •   Career  &  Life  Transition  planning  •   My  Passion  

 The  Grad  Transitions  course  is  worth  four  (4)  credits  towards  graduation.  Students  must  complete  the  requirements  listed  for  each  area  of  reflection  in  order  to  receive  the  four  credits  for  graduation.    Upon  successful  completion  of  the  program,  a  Requirement  Met  (RM)  mark  will  appear  on  the  final  report  card.    All  Grade  12s  will  be  using  myBlueprint  (www.myBlueprint.ca)  to  fulfill  their  Grad  Trans  requirements.    As  part  of  your  Grad  Transitions  you  will  use  myBlueprint  to  perform  career  explorations,  plan  for  post  graduation  (whether  it  entails  further  education,  work  or  travel)  and  produce  a  digital  portfolio.    Each  of  the  following  components  include  content  that  should  be  included  in  the  My  Portfolio  &  Reflections  and  activities  to  be  completed  in  myBlueprint.

First things first  

q   Set  up  a  myBlueprint  account  (see  video  on  www.dwpoppy.ca  website)  q   Meet  with  Ms.  Wolgram  in  the  Career  Centre  (Tuesdays  or  Fridays)  

   

Who I am  Required:  1  boxes  in  myBlueprint  digital  portfolio  titled  “Who  I  Am”    In  myBlueprint  digital  portfolio:  

q   Add  a  Custom  Box  and  title  the  Box  “Who  I  Am”  q   Upload  a  current  picture  of  yourself  and  post  q   Click  on  the  menu  icon  at  the  top  right  of  the  Box  and  Edit  Colour  and  choose  a  colour  for  the  

box    In  the  Reflection  Box  write:  

q   Your  Name  (first  and  last  name)  q   Something  unique/interesting  about  your  family  (family  background,  traditions,  etc.)  q   What  achievements  you  have  made  over  the  past  3  years.  Include  grades,  awards,  certificates,  

certifications  (which  can  also  be  uploaded),  personal  changes,  volunteer  work,  etc.    q   What  are  your  hobbies  q   Your  most  important  personal  strengths  and  why  q   How  your  time  at  DWP  has  shaped  who  you  are.  

 Add  additional  boxes  if  you  wish  but  title  them  all  “Who  I  Am”  and  keep  the  box  colour  the  same      

Personal Health  Daily  physical  activity  and  a  commitment  to  fitness  and  physical  and  emotional  well-­‐being  are  key  to  healthy  living.  In  this  component  of  Graduation  Transitions,  students  develop  the  knowledge,  attitudes,  and  habits  needed  to  be  healthy  individuals  by  maintaining  a  personal  health  plan  and  participating  in  at  least  150  minutes  per  week  of  moderate  to  vigorous  physical  activity  in  each  of  Grades  10,  11,  and  12.  Boards  are  responsible  for  providing  options  to  give  students  a  number  of  choices  for  how  they  meet  the  daily  physical  activity  requirements  at  school,  at  home,  and/or  in  the  community.      Required:  3  boxes  in  myBlueprint  digital  portfolio  titled  “Personal  Health”    Things  to  do  first:  

q   Create  a  Physical  Activity  Chart/Calculation(s)  (use  excel  or  word)  showing  how  you  obtained  the  minimum  required  120  hours  of  physical  activity  in  grade  12  (remember  a  PE  course  like  PE12  is  equivalent  to  100  hours).

In  myBlueprint  digital  portfolio:  q   Add  a  Custom  Box  and  title  it  “Personal  Health”  q   Upload  the  Physical  Activity  Chart  and  post  q   Edit  the  colour  of  the  box  and  choose  a  colour  that  has  not  been  used  

 In  the  Reflection  Box  write:  

q   How  you  stay  physically  active  (include  pictures  of  your  involvement)    q   What  physical  fitness  means  to  you;  how  physical  activity  is  integrated  into  your  life.  q   2  areas*  of  health  and  wellness  you  are  good  at;  include  specific  examples  of  why  q   2  areas*  of  health  and  wellness  you  need  improvement  on;  include  specific  examples  of  why  q   How  you  plan  to  incorporate  physical  activity  and  wellness  into  your  life  post-­‐graduation.  q   How  do  you  manage  stress?    Techniques,  strategies,  etc..      

 *  Areas  of  health  &  wellness  to  discuss  should  include:  ~  nutritional  habits   ~  time  management  ~  positive  health  decisions   ~  regular  exercise  ~  stress  management   ~  emotional  health  management  ~  adequate  rest/sleep   ~  support  network  ~  spirituality  (vales,  morals,  belief)      In  myBlueprint  digital  portfolio:  

q   Create  at  least  2  more  boxes  titled  “Personal  Health”  and  upload  pictures  of  you  involved  in  some  kind  of  physical  activity  (swimming,  dancing,  skiing,  team  photo  etc.)  

q   In  the  reflection  boxes  describe  what  you  are  doing  in  the  pictures  q   Make  sure  all  the  boxes  are  the  same  colour  

 Add  additional  boxes  if  you  wish  but  title  them  all  “Personal  Health”  and  keep  the  box  colour  the  same      

Community Connections  Life  after  graduation  includes  the  world  of  work  and  community  involvement.  As  part  of  Graduation  Transitions,  students  gain  employability  skills  through  participation  in  at  least  30  hours  of  work  experience  and/or  community  service  in  grade  12.    Required:  2  boxes  in  myBlueprint  digital  portfolio  titled  “Community  Connections”    In  myBlueprint  digital  portfolio:  

q   Upload  the  Work  and/or  Volunteer  Experience  Verification  Form(s)*  to  a  box  titled  “Community  Connections.”    Complete  as  many  forms  as  required  to  verify  the  minimum  30  hours.  Alternatively,  upload  a  pay  stub  with  hours  worked  or  letter  indicating  volunteer  hours.    

q   Complete  and  upload  the  Student  Self-­‐Evaluation  of  Employable  Skills*  form  to  another  box  titled  “Community  Connections”

Career and Life  Graduation  Transitions  requires  students  to  explore  personal  and  career  goals  during  their  secondary  school  years  and  create  a  plan  for  life  after  graduation.  Transition  planning  is  an  ongoing  process  throughout  the  graduation  program  years  and  requires  the  completion  of  a  transition  plan,  culminating  in  a  final  Grade  12  presentation.    Required:  6  boxes  in  myBlueprint  digital  portfolio  (there  may  be  more  depending  on  the  number  of  post  secondary  institutions  and  careers  explored)    Things  to  do  first:  

q   Make  an  appointment  with  Ms.  Wolgram  to  discuss  career  opportunities  q   Complete  myBlueprint  Post  Secondary  Planner  and  Occupational  Planner  q   In  myBlueprint  under  Goal  Setting,  make  3  Specific  goals  (personal,  educational,  career)  you  

wish  to  accomplish  over  the  next  5  years  and  how  you  plan  to  meet  them.    Your  goals  must  be  SMART:  Specific,  Measurable,  Attainable,  Realistic,  Time-­‐bound  

q   In  myBlueprint  Financial  Planner  create  a  financial  plan  for  your  first  year  after  high  school    In  myBlueprint  digital  portfolio:  

q   Add  a  box  titled  “Career  and  Life”  and  upload  a  current  resume  (you  can  use  myBlueprint  to  create  a  Resume)  

q   Add  a  second  box  titled  “Career  and  Life”  and  upload  a  current  Cover  Letter  (you  can  use  myBlueprint  to  complete  a  Cover  letter)

that  you  are  passionate  about.  •   What  is  it  that  motivates  you?    What  drives  you?  •   Are  you  an  amateur  filmmaker?    A  pastry  chef?    Have  you  fallen  in  love  with  CrossFit?    Do  

you  moderate  an  internet  forum?    Do  you  play  the  bagpipes?      •   We  want  you  to  freely  talk  about  something  that  people  may  or  may  not  know  about  you  

already.  •   In  addition  to  talking  about  this  topic,  you  MUST  bring  in  an  object  (item,  trophy,  

instrument,  etc.)  that  represents  your  passion  that  you  can  speak  about.      •   Be  prepared  to  answer  questions  about  this  topic  as  you  are  the  EXPERT  on  it.  

q   Upload  artifacts  (pictures,  awards,  certificates  etc.)  to  illustrate  your  passion  q   Write  about  your  passion  in  the  reflection  box

What Next?  

q   When  you  have  completed  all  the  activities  in  the  Grad  Trans  package  your  portfolio  should  minimally  contain  13  boxes:  

o   1  box  titled  “Who  I  Am”  o   3  boxes  titled  “Personal  Health”  o   2  boxes  titled  “Community  Connections”  o   1  box  titled  “Post-­‐Secondary  Plan”  o   1  box  titled  “Occupational  Plan”  o   1  box  titled  “Financial  Plan”  o   3  boxes  titled  “Career  and  Life”  for  Resume,  Cover  Letter  and  Letter  of  Reference  o   1  box  titled  “My  Passion”  

 q   Make  sure  your  myBlueprint  digital  portfolio  is  arranged  in  an  organized  manner  (colour  coded)  

and  contains  the  documents,  pictures,  videos  and  anything  else  you  want  to  talk  about.    

q   Before  your  actual  interview,  practice  your  presentation  with  a  family  member  and  ask  for  feedback  and  have  them  fill  in  the  Practice  Presentation  Form  (Grad  Transition  Final  Presentation  and  Assessment  are  included  at  the  end  of  this  package).  

 q   Your  presentation  date  and  time  will  be  booked  for  you  and  the  schedule  will  be  posted  on  the  

school  website.    It  is  your  responsibility  to  complete  your  portfolio  and  be  ready  to  present  on  or  before  the  scheduled  date.    If  you  are  completed  early  and  wish  to  present  your  portfolio  contact  Ms.  Mallinson  or  Mr  Yu.  

On the day of your presentation:  

q   Treat  your  presentation  as  an  actual  interview  and  dress  appropriately  (business  casual).    

q   You  will  be  presenting  your  myBlueprint  portfolio  and  everything  you  need  for  your  presentation  should  be  included  in  your  myBlueprint  portfolio  so  make  sure  it  is  complete.
