1. PREAMBLE

Thisdocumenthasbeengeneratedby theMTSDPTechnicalCommitteeandhasbeenprovided to industry as a guidance document for the safe and effective conduct andmanagementofDPoperations.

This document is not meant to replace any rules, regulations or owners’ operationalmanuals. It is a compilation of existing guidance and practices gleaned from varioussources in industry. It refers to standardsand guidance usedwithin the industryanddraws attention to them. It also incorporates additional information gleaned fromexperienceanddocument/standardsnotinthepublicdomain

This is not intended to be an all encompassing document covering all aspects of DPoperations. It focuses on a number of important DP operational themes, which, ifeffectivelymanaged,willresultinsaferDPoperationsandareductioninexposuretoDP

relatedincidents.TheguidanceprovidedinthisdocumentisnotdirectedatanyparticularcategoryofDPoperationorDPvessel. Itis intendedtoapply toanyDPoperationundertakenbyanytype and class of DP vessel operating in support of offshore oil and gas activities.Examples include MODUs, ships, construction and logistics vessels where dynamicpositioningisusedfor,oraiding,stationkeeping.

PART1

1.1 SCOPE

Part 1 addresses the themes that, if managed effectively, will contribute to safe andefficientDPoperations.

1.2 DEFINITIONS

1.2.1 DPSystem

TheDPsystemconsistsof componentsandsystemsactingtogethertoachievereliablepositionkeepingcapability. TheDPsystemincludesthepowersystem(powergenerationandpowermanagement),thrustersystemandDPcontrolsystem.

1.2.2 DPControlSystem

The DPcontrol system consists of all control components and systems, hardware andsoftwarenecessarytodynamicallypositionthevessel. TheDPcontrolsystemcomprisescomputer systems, sensors, displaysystems,position reference systemsand associatecablingandrouteing.

1.2.3 WorstCaseFailure

The Worst Case Failure (WCF) is the identified single failure mode in the DP systemresultinginmaximumeffectonDPcapabilityasdeterminedthroughFMEAstudy.

1.2.4 WorstCaseFailureDesignIntent

The WorstCaseFailureDesign Intent (WCFDI) is the single failure with the maximum

consequences that has been the basis of the design andoperationalconditions. Thisusuallyrelatestoanumberofthrustersandgeneratorsthatcansimultaneouslyfail.

1.2.5 RedundancyConcept

TheRedundancyConceptisthemeansbywhichtheWorstCaseFailureDesignIntentisassured.

1.2.6 CriticalActivityMode

CriticalActivityModeistheconfigurationthatthevessel’sDPsystemshouldbesetupandoperatedinsoastodelivertheintentofthevessel’sDPclassnotation. Theobjectiveisthatnosinglefailureshouldresultin exceedingtheworstcasefailure. EachDPvesselhasonlyonecriticalactivitymodewhichisuniquetothatvessel.

1.2.7 CriticalActivityModeofOperationTheCriticalActivityModeofOperation (CAMO) isgenerallya tabulatedpresentationofhow to configure the vessel’s DP system, including power generation and distribution,propulsionandpositionreferencesystems,sothattheDPsystem,asawhole,deliverstheintent of the vessel’s DP class notation. The CAMO tablealso sets out the operatoractionsshouldarequiredconfigurationnotbemet.

1.2.8 ActivitySpecificOperatingGuidelines

ActivitySpecificOperatingGuidelines(ASOG)aregenerallypresentedintabulatedformatandsetouttheoperational,environmentalandequipmentperformancelimitsconsiderednecessaryforsafeDPoperationswhilecarryingoutaspecificactivity. Thetablealsosets

outvarious levelsofoperator actionasthese limits are approachedor exceeded. TheASOGwillvarydependingontheactivityandareuniquetothatactivity.

Note: WhereDPMODUsarecarryingoutwellactivitiesthenomenclatureischangedtoWSOG(WellSpecificOperatingGuidelines).

1.2.9 TaskAppropriateMode

Task Appropriate Mode (TAM) is a risk based mode. Task Appropriate Mode is theconfigurationthatthevessel’sDPsystemmaybesetupandoperatedin,acceptingthatasinglefailurecouldresultinexceedingtheworstcasefailureandcouldresultinblackoutor loss of position. This is a choice that is consciously made. This mode may beappropriate insituations where it isdeterminedthat the risksassociated witha lossofpositionarelowand,wherethetimetoterminateislow.

Example1:- ADPMODUmayoperateinthismodeduringtimeswhenTimetoTerminateisshort,andinCriticalActivityMode,whenTimetoTerminateislong.

Example2:- ADPpipelayvesselmayoperateinthismodewhenmorethan500m fromasurfaceormissioncriticalsubseaasset,andinCriticalActivityModewheninside500m.

1.2.10 ThrusterandGeneratorOperatingStrategy(TAGOS)

Adocumentthatprovidesinformedguidance,usuallyderivedfromareviewoftheFMEAand if necessary, validation from personnel knowledgeable about vessel specificinformation, onappropriateconfigurationsofthrusters,generatorsandpowerdistribution,and associated constraints, so as to enable correct choices to be made to provideoptimumlevelofredundancy.

1.2.11 DPCapabilityPlots

DPCapabilityPlotsdefine by theoretical calculation the vessel’s capability tomaintain

positioninvariousenvironmentalconditions,(i.e.wind,seastateandcurrent)and,whereappropriate,takingaccountofcertainexternalforces,suchaspipetensionand,invariousthruster/ power configurations, including all thrusters running, loss of most effectivethrusterandfollowingworstcasefailure.

1.2.12 DPFootprintPlots

DP Footprint Plots are constructed, by observation onboard the vessel in real timeconditions. Theplotsareofthevessel’sDPstationkeepingperformanceandlimitationsinvarious environmental conditions (wind, seastate and current) and in various thruster/power configurations, includingall thrusters running, lossofmost effective thrusterandafterworstcasefailure.

Note ItisacknowledgedthatDPFootprintPlotsforDPMODUsmaybelessrelevantincomparisontoDPvesselsusedforotherapplications.

OwingtotheiralmostcontinuousworkprogramsDPMODUswillrarelyhavetheopportunitytorecordDPFootprintPlotsfordegradedthrusterconfigurations.

1.2.13 TimetoTerminate

TimetoTerminate(TTT)iscalculatedastheamountoftimerequiredinanemergencytophysicallyfreetheDPvesselfromitsoperationalactivityfollowingaDPabortstatusandallowingittobemanoeuvredclearandtoproceedtosafety.

Forexample:- IncaseofaDPdrillingvesselthismaybethetimeneededtoreleasefromthewellhead. ForaDPdivingvesselthismaybethetimeneededforthediver(s)toreturn

3.1.1 ItisrecommendedthatDPvesselowners/operatorsshouldmaintainthedocumentationlistedinthetablebelowandshoulddevelopandimplementassociatedprocessesforthepurposesof:

•  ensuringthesafeandeffectivemanagementofthevesselinDP

•  ensuringthetechnicalsuitabilityofthevesselforeachDPactivityitisrequiredtocarryout

•  determiningtheconfigurationforthecriticalactivitymodeofoperationandthetaskappropriatemode

•  understanding the vessel’s station keeping capabilities following the worst casefailure

4.3 DPFMEA

4.3.1 TheDPvessel’sDPFMEAisthemostimportanttechnicaldocumentinthelistofrequireddocuments. TherequirementforaDPFMEAhasitsoriginsinIMOMSC/Circ645(1994)

“Guidelines for Vessels with Dynamic Positioning Systems”. These benchmarking

guidelinesprovidethe foundation forall subsequentDP rules, regulationsandguidanceissuedbyClassandotherindustrybodies,suchasIMCA.

4.3.2 The IMO Guidelines require all DP vessels to be subjected to survey and testing inaccordancewith IMO’sspecified guidelines. This includesinitialandperiodic completesurveyand testingof allsystemsandcomponentsrequired tokeeppositionaftersinglefailuresassociatedwiththevessel’sassignedDPequipmentclass.

4.3.3 TheperiodiccomponentintheIMOGuidelinesrequiresthecompletesurveyandtestingtobe repeated every five years to ensure full compliance with applicable parts of theguidelines.

4.3.4 InadditioncompliancewithIMOGuidelinesrequiressurveyandtestingafterdefectsarediscovered and corrected and, after a DP related accident and, whenever significantrepairsoralterationsarecarriedout,soastodemonstratefullcompliancewithapplicablepartsoftheguidelines.

4.3.5 The IMO Guidelines do not make clear distinction between vessels of different DPequipmentclass.

4.3.6 This IMOrequirement has been interpretedby theDP community such that thesurveyrequirementismetbyaDPFMEA(orFMECA2)andthetestingrequirementbyDPFMEAProvingTrials.

4.3.7 Accordingly,allDPvesselsofDPClass2or3arerequiredtohaveaClassapprovedandstampedDPFMEAandDPFMEAProvingTrials.

4.3.8 Definitions:AnFMEA isasystematicanalysisof systemsandsub-systemstoalevelofdetail that identifiesallpotentialfailuremodesdownto theappropriatesub-systemleveland their consequences. A FMECA is an extension of an FMEA that adds a riskassessmentofeachfailuremodetodetermineitscriticality.

4.3.9 InadditiontocomplyingwiththeIMOGuidelinesandtherelevantDPrulesofthevessel’sClassificationSocietytheDPFMEAshouldachieve thestandardsofdetailandanalysiscontainedinthefollowingindustryguidance;

•  IMCA M04/04 2004 “Methods of Establishing the Safety and Reliability of DPSystems”

Note FMEAs are a requirement to obtain DP Class 2 and 3 notation. Whilst notstipulated as a class requirement for DP 1 vessels owners/ operators areencouragedtosubjecttheirDP1vesselstotheDPFMEAprocess.

Note ParticularattentionshouldbepaidintheDPFMEAtotheinterfacesbetweentheDPsystem andothersystems thathavethepotential toaffecttheDPsystem,suchaswherethevesselisfittedwithanEmergencyShutDown(ESD)system,pipetensionersystemorfiremonitorsystem.

4.3.10 KeyDPpersonnel,includingthevesselMaster,DPOs,EngineersandElectriciansshouldhaveadetailedknowledgeoftheDPFMEAandshouldusetheinformationprovidedtobefullyinformedaboutthecapabilitiesandlimitationsofthevessel’sDPsystem.

4.3.11 TheresultsfromaDPFMEA,inparticularissuesrelatedtothevessel’sworstcasefailureand significant single point failures, should be used in the formulation of operational,emergencyresponseandplanningdecisions.

4.4 DPCAPABILITYPLOTS

4.4.1 DP CapabilityPlots should be calculatedfor the vessel. Guidanceis provided on DPCapabilityPlotsinIMCAM140Rev1,“SpecificationforDPCapabilityPlots”.

4.4.2 These theoretical plotsare calculated from detailedinformation of thevessel’shull andsuperstructure form and available thruster power. The calculations should useenvironmentaldata(seastate,windandcurrent)appropriateto theareainwhichtheDPvesselistooperate.

4.4.3 Theseplotsshouldshowthelimitingwindspeed360degreeenvelopesforthescenariosbelow, where each point on the envelope represents the wind speed at which it iscalculatedthatthevesselwillbeunabletomaintainpositioninDP.

4.4.4 DPCapabilityPlotsshouldincludethefollowingscenariosatcurrentspeedsof0kts,1ktand2kts,oratothercurrentspeedsthatarerepresentativeofthelocationinwhichtheDPvesselistooperate:

•  Fullyintactpowergenerationandthrusters.

•  Lossofmosteffectivethruster(s).

•  Followingtheworstcasefailure.

Note TheDPCapabilityPlotsshouldbeprovidedinaformatthatisintuitivetotheuseronboard(e.g.PolarPlot).

4.5 DPFOOTPRINTPLOTS

4.5.1 DP Footprint Plots should also be produced on board. DP Footprint Plots are nottheoretical. They are actual measurements of the vessel’s DP station keepingperformanceintheactualenvironmentalconditionsandthrusterconfigurationatthetimetheplotwastaken. DPFootprintPlotsshouldbetakenwheneveropportunitiesarise,suchasduringstandbyperiods,weatherdowntimeoronarrivalatthefield. PlotsshouldbetakenforthethrusterconfigurationsusedintheDPCapabilityPlots,i.e.fullyintact,lossofmosteffectivethruster(s)andafterworstcasefailure.

4.5.2 Some DP systems have a software application that produces DP Footprint Plotselectronically. DPOscanalsoproduceDP FootprintPlots bymanualmethods using aplottingsheet.

4.5.3 DPFootprintPlotsservetwomainpurposes.

1. They provide a scatter plot of vessel positions at regular intervals around therequiredsetposition(thisshowsaccuracyofstationkeeping)

2. Theyalsoprovidecomparisonpointsonthelimitingwindspeedenvelopegiven inthetheoreticalDPCapabilityPlots(thisshowswindspeedsatwhichitwasseenthatthe vessel was unable to maintain position, thus validating or contradicting thetheoreticalDPCapabilityPlotsforthevariousthrusterconfigurations.)

4.5.4 DP Footprint Plots serve other purposes, including learning and familiarisationopportunitiesforDPOsandinprovidingsnapshotsofvesselstationkeepingbehaviourforspecificlocationsandactivities.

4.5.5 Theoretical DP Capability Plots and DP Footprint Plots combine together to enhance

knowledgeandunderstandingofthevessel’sDPstationkeepingability.

Positionreferencesystemsshouldbeselectedwithdueconsiderationtooperationalrequirements,both with regard to restrictions caused by the manner of deployment and expectedperformanceinworkingsituations.

4.6.2 Positionreferencesystemscompriseabsoluteandrelativesystems. Anabsolutesystem

givesvesselgeographicalposition. Arelativesystemgivesvesselpositioninrelationtoanon-fixedreference. Arelativesystemcanbeusedasanabsolutesystemifinstalledonapointthatisafixedgeographicalposition. And,anacousticabsolutesystemcanbeusedasarelativesystemifattachedtoanon-fixedasset.

4.7.2 ThefollowingselectedDPcontrolmodesarerelevanttospecificDPactivities.

TargetFollow Enables the DP vessel to follow a moving target and is used, forexample,tofollowanROValongapipeline.

HeavyLift Takesaccount of theeffects of theload transfer on the mass of thevessel and the additional lateral force, normally by reducing gain andrelaxingtheDPcontroller.

4.7.3 ExternalForceCompensation- Wherethemeasuredexternalforceactingonthevessel,whichisseparatefromtheenvironment,isincludedintheDPcalculationandtreatedasa

forcefeedforward. Thismode isused toaccountforpipetensions inapipelayerandhawsertensioninashuttletanker.

4.7.4 FireMonitor Compensation - Used to compensate for the varying forces exerted onvesselfromthefiremonitors.

Weathervane EnablestheDPvesseltorotatewiththewind,currentandwavesaroundafixedormovingpointcalledtheterminalpoint. Neithertheheadingnorthe position of the DP vessel is fixed. The heading of the vessel iscontrolledtopointtowardstheterminalpoint. Thepositionofthevesselis controlled to follow a circle, called the setpoint circle, around theterminalpoint. Thismode isappropriate for connected shuttle tanker/FPSOoperations.

Caution It is not uncommon for the term “weathervane” to be used to denote“minimumthrust”, i.e. where theDP controlsystemallowsthevessel’sheadingto rotate tominimize externalforcesactingonthevesselandthrusterrequirements. Thesetermsshouldnotbeconfused.

4.8 TRIALSANDCHECKLISTS

4.8.1 ArangeoftrialsandchecklistsistobeprovidedforeachDPvesselandimplementedasaverificationthatthevessel’sDPsystemcomplieswithand,isoperatedinaccordancewith,applicablestandardsandguidelines. Thisverificationprocessshouldconfirmthefailuremodes and their effects on the systems and equipment analysed in the DP FMEAdocument (to include the Worst Case Failure) and the vessel’s station keeping abilityfollowing itsWorstCaseFailure. Owners/operators shouldreferto the following listoftrialsandchecklistswhendevelopinganappropriateregimefortheirownDPvessels. ThelistbelowisatypicallistforaDPdivesupportvessel.

4.8.2 DPFMEAProvingTrials: Aseriesoftestsusedtoproveexpectedeffectsoffailuremodesfound in the FMEA desktop analysis. These tests should also include the testing ofinterfaces between different vendor systems and equipment. These tests should be

conducted immediately following launching of a new build vessel and followingmodifications,additions,upgradesrepairs.

4.8.3 Endurance Trials:- (new build/ system modifications class requirement): To prove theoperationoftheDPsystemunderloadconditionsforat least4hourswithoutsignificantalarmsoftheDPsystem.

4.8.4 Annual DP Trials:- A series of tests of fault and failure conditions relevant to the DPSystem. Thetestsshouldbedesignedtoprovesystemredundancy,asdefinedintheDPFMEA, system and equipment functionality, to validate repairs and preventivemaintenance,andtesttheoperationofprotectionanddetectiondevicesandresponsessoastodemonstratethatthevessel’sDPsystemremainsfitforpurpose. AnnualDPTrialsshouldbeperformedataspecificonceayearwithin3monthsoftheanniversaryofthe

previousyear’s trials. AnnualDPTrialsalsoprovide theopportunitiesfortrainingof thevessel’screwandenhancementoftheirknowledgeoffailuremodesandtheireffects.

Note Experience has shown that, owing to heavy operational demands, DP drillingunitsare notusuallyable tomeet the abovecriteriafor conductingAnnualDPTrialsataspecific timeonceayear. ThishasbeenrecognizedbytheindustrywithpublicationofspecificguidancerelatingtotheconductofAnnualDPTrialsforDPMODUs. ThisisgiveninIMCAM191,“GuidelinesforAnnualDPTrialsforDPMobile Offshore DrillingUnits”. Theguidancesetsout a regimeof annualtesting that is conducted on an incremental basis throughout the year asopportunitiesarisebutneedstobecompletedwithinatwelvemonthperiod.

4.8.7 DP Location Set Up Checklist: A series of checks to demonstrate that the vessel isproperlysetupfor thelocation,in particular thesatisfactoryperformanceof thepositionreferencesystems.

4.8.8 Pre-DiveChecklist:- Aseriesofchecksperformedpriortocommencingdivingoperations.Mainandbackupcommunicationtestsshouldbeincludedinthisprocess.

4.8.9 ECR Checklists: Aseriesof checks and tests thatverifythatthe vessel’sset upandconfigurationofsystemsandequipmentmeettherequirementsofthenecessarymodeofoperationasdeterminedbytheCriticalActivityModeofOperation (CAMO)andtheDP

ClassortheTaskAppropriateMode(TAM).4.8.10 500mChecks:-Aseriesofchecksandtestsperformedbeforeenteringthe500mzoneof

anassetinwhichsetupmodeandfunctionsareverifiedandconfirmed. Approvalisthenobtainedtooperateincloseproximitytotheasset. Mainandbackupcommunicationtestsshouldbeincludedinthisprocess.

4.8.11 WatchStatus/6HourChecklist: AseriesofchecksandtestsperformedbytheDPOstoverifyandconfirmthesetupoftheDPsystempriortotakingovertheDPwatch.

4.8.12 PostDPIncidentTrials: Testsperformedtoensurethatthecorrective/repairmeasurestakenfollowingaDPincidenthaveproperlyaddressedthecausesoftheincidentandthatthevessel’sDPsystemisinasafeandoperablecondition.

4.8.13 PostDPModificationTrials: Aseriesofchecksandteststhatareusedtodeterminetheeffectsofmodificationsand/oradditionsontheDPsystemandthevessel’ssubsequentstationkeepingperformance.

4.9 DPOPERATIONSMANUALS

4.9.1 AvesselspecificDPOperationsManualshouldbepreparedforeachDPvessel.

4.9.2 ThevesselspecificDPOperationsManualisthemostimportantoperationaldocumentin

the list of required documents. The requirement for a manual has its origins in IMOMSC/Circ645(1994)“GuidelinesforVesselswithDynamicPositioningSystems”.

4.9.3 The IMO Guidelines require a series of checklists, test procedures and DP operatinginstructionstobeincorporatedintoonemanual.

4.9.4 EachClassificationSocietyhasitsownspecificrequirementsforaDPOperationsManual,eachwithdifferentrequirementsforcontent.

4.9.5 Therecommendationssetoutbelowaddressareas thatarenotnecessarilyrequiredbyclass. Theyarebasedoncurrentindustrypractices.

4.9.6 Themanual should contain sufficiently detailed instructionand guidance to enable thevessel tobe operated safely inDPandsafelyexecute its intendedactivities. Thiswill

include a clear statement on the DP philosophy for the vessel, the organisation,

responsibilitiesand roles of keyDP personnel, trainingand competency, watchkeepingand manning, vessel technical data and layout, vesselDPcapabilities, operating limits,operating modes, the planning and preparation of DP operations, DP operating

procedures,emergencyprocedures,DP incidenthandling andalert systemsandvesselspecifictrialsandcheckliststhatapplyuniquelytothevessel.

4.9.7 Themanual should specificallyaddressoperational interfaces betweendifferent vendorsystemsandequipmentto ensurethat theyareconfiguredandoperatedproperly. Thisshould include interfaces between systems and equipment thatare not part of the DPsystem but whichmayaffect the DP system, suchasEmergency Disconnect Systems(EDS)onMODUs,tensionersystemsonPipelayvessels,etc.

4.9.8 Thevesselspecificmanualmayalsocontaingenericcontent,suchascompanypolicies,proceduresandstandingorders.

4.9.9 Themanual should represent the way the vessel is operated in DP. For complicatedpowersystemsand/orthrusterconfigurations,itmaybeusefulprovidetheoperatorwitha

thruster and generator operating strategy (TAGOS) to assist in the decision on whatgenerators and thrusters to use for different circumstances and different equipmentavailabilities.

4.9.10 Guidance on thecontents of vessel specificDPOperations Manuals is provided in thefollowingIMCAguidelines,M103,M109,M117(contingencytraining)andinclasssocietyDPrules.

4.10 ACTIVITYOPERATIONALPLANNING

4.10.1 In recognizing that exposure to risks manifests itself during vessel operations it isrecommendedthatactivitiesperformedbyDPvesselsshouldbesubjecttoplanningandpreparation. Inplanningandpreparingfortheactivitiesthefollowingshouldbeconsidered

and,whereappropriate,documented:

•  Configuration for the Critical Activity Mode of Operation (CAMO) or, whereappropriate,theTaskAppropriateMode(TAM).

•  PreparationoftheActivitySpecificOperatingGuidelines(ASOG),includingonboarddiscussionwithallrelevantstakeholdersaspartofthepre-projectexecution/activity.

•  Discussiontobeincludedinpre-projectexecutionreadinesschecklist.

•  Capabilitiesofthevessel,bothintactandresidualcapability,following WorstCaseFailure(WCF).

•  Limitationsimposedbywaterdepth.

•  Consequencesofalossofpositionand/orheading.

• 

Limitationsimposeduponoperationsbyresidualcapability.

•  SIMOPSand marinevessel interactionandconsequences arisingfromchangeofstatus(GreentoBlue,YelloworRed).

•  Theactivitybeingperformedandthenecessarytimetoterminatetobringvesseltoasafepositionupontheonsetoffailure.

4.10.2 Activities should include day to day operations, any specific operation relevant to thedesign of the vessel, as well as any unique operations the vessel is called upon toperform.

4.10.3 Appropriate measures should be in place to clearly identify critical tasks/ operationalphasesoftheactivityandtoensurethatthevesselissetupinCriticalActivityModeof

4.10.4 OnaDPMODU,ASOGsareknownasWSOGs. SomeownersrefertoitasWSOC(WellSpecificOperatingCriteria)

4.11 COMMUNICATIONS

4.11.1 Thevessel shouldbeequipped with theappropriateprimaryandsecondaryequipmentneededtocommunicatebetweenallparties(stakeholders)whilstcarryingouttheintendedtask.

4.11.2 Effectiveinternalandexternalcommunicationsisakeytooltomanagerisk.

4.11.3 Communicationsinthiscontextincludevoice,visual(lights/displays)andaudiblemeans(alarms). MeansofcommunicationarenotlimitedtotheabovebutincludeintegratedITsystems using wireless network technology that combine communications with otherfeatures,includingAISandDGNSS.

4.11.4 Operational specific visual and voice communications should ensure that the pertinentinformationflowsbetweenthekeyoperatingpointsaswellastoandfromassetsand/orothervesselsthatmightbeaffectedbytheoperationbeingcarriedout.

4.11.5 Theseoperatingpointsmaybeonboardthevesselaswellasonotherfacilitiesinvolvedwith the activity. Communication protocols are to be set up to provide pertinentinformationregardingintent,currentstatusofplannedaswellasunexpectedeventsduringtheexecutionoftheactivity.

4.11.6 Continuity of communications during foreseeableemergencysituations should be takenintoaccount.

4.11.7 Communicationsshouldbetakenintoaccountwhendetailingtherolesandresponsibilitiesofkeypersonnelduringtheplanningstagesfortheintendedtask,ensuringthatacommonworkinglanguageandterminologyisusedatalltimes.

4.11.8 GuidanceoncommunicationsisprovidedinM103,M182andM205.

Note The importance of communication to be emphasized by incorporating into theASOG.

4.12 DPPLANNEDMAINTENANCE

4.12.1 DP vessels should have a structured planned maintenance system that specificallyaddresses maintenance of the vessel’s DP system, equipment and support systems.RelevantguidanceisgiveninIMCAM109.

4.12.2 Plannedmaintenanceshouldaddress allequipment that hasan impact on thevessel’sstationkeepingcapabilities. Thisshould includeindirectcomponentssuchasgeneratorcircuit breakers, bus tie breakers, etc. Maintenance should include regular cleaning,calibration,andtestingofequipmentasoutlinedinmanufacturer’srecommendationsandindustryguidelines.

4.12.3 Recordsofplannedandunplannedmaintenanceshouldbekeptinanauditableformat,eitherhardcopyorappropriateelectronicformat. These recordsshouldincludevendorservicerecordsaswellasmaintenanceperformedbyvesselpersonnel. These records

shouldbekeptonboardfortheperiodspecifiedbytheowner/operator.

4.12.4 AminimumnumberofrequiredcriticalDPsparesshouldbemaintainedonboard. Thecriticalspareinventoryshouldbemonitoredviaaformalinventoryprogramthatiscloselylinkedto theplannedmaintenancesystem. Thisshouldassistingettingbacktonormal

operatingconditionafterequipmentfailureorDPincident.

Note Theclient’sduediligenceprocessisusuallythemaindriverinthecriticalpathtoreturntonormaloperatingcondition. Typically,thisalsoinvolvesvendorsupportaswellastheclient’sDPconsultant.

Note MaintenanceonDPrelatedequipmentwhilstconductingDPoperationsshouldbecontrolledbya documentedpermitto worksystemandshouldalwaystake intoconsiderationthepotentialalterationof failuremodesandincreasedcriticalityoffailureconsequencesonDPcapabilitiesand/orredundancy.

4.13 DPINCIDENTS

4.13.1 DP vessels should be provided with and operate appropriate DP incident reporting,

investigationandclosingoutprocedures. Thisshouldbeinaccordancewithvesselowneroroperators’and, ifapplicable,clients’processes. Documentedrecordsshouldbekeptandbecapableofauditing.

4.13.2 WhereCAMO,TAMandASOGareusedastoolstomanageDPoperationsasuggestedapproachisthat,apartfromtheexceptionsinthenotesbelow,anyreactivechangeofDPstatusfromGREENtoYELLOWorREDshouldberegardedasaDPincident,andshouldbereported,recordedandinvestigated.

4.13.3 ItisrecommendedthatreactiveYELLOWandREDDPincidentsareinvestigatedassoonaspracticableaftertheDPincidentand,whererelevant,trialsarecarriedoutaspartoftheinvestigationprocess.

Thepurposeoftheinvestigationandthetrialsshouldbe:

•  toassistinidentifyingtherootcausesoftheincident

•  toverifyandvalidatethatmeasurestoaddresstherootcauseareeffective

•  tovalidatethat effectivemeasureshavenot introducedanyadditional potential tocausefailures(bothhiddenandapparent)

4.13.4 In theevent of theoccurrenceofaDP incident relatingtothe vessel’sconfigurationasdescribedintheDPFMEAandotherdocuments,itissuggestedthattheFMEAproviderisinvolvedintheincidentinvestigation. ThiswillfacilitatelessonslearnttobeimplementedintotheDPFMEAandprovingtrialsprogram.

4.13.5 Owners/ operators of DP vessels are encouraged to share lessons learned from DPincidents with the widerDPcommunity. DP systemsand equipment vendorsare alsoencouraged todo likewiseandtoshare informationonunexpectedfaults, featuresandfailuresthatareidentifiedinoperation. IMCA’sstationkeepingincidentreportingschemeprovides a suitable channel for disseminating relevant information throughout the DPsector.

4.14 REPORTINGANDRECORDKEEPING

4.14.1 OwnersoroperatorsofDPvesselsshouldhaveaneffectivereportingandrecordkeepingsystem.

4.14.2 Thereshould be aclear line of reportingDP related itemsonboard theDP vessel andbetweeneachdepartmentandthisshouldinvolvekeyDPpersonnel. Thereshouldalsobe a clear line of reporting between the DP vessel and the company’s shoresidemanagement. DPrelatedrecordsshouldbemaintainedonboardand,whereappropriate,atthecompany’spremises.

4.14.3 ThedocumentsandrecordscontainedintableA,Section3.2,shouldbeaccessibletokeyDPpersonnelandtootherinterestedparties.

4.14.4 DPvessels,onoccasion,carryoutactivitieswhichmaybeuniquetoprojectrequirements.Records of these activities including pertinent information from Hazards and RiskIdentification (HIRAs) should be made and kept onboard for future reference, both for

trainingand familiarisation processesaswell as forreference incaseof similar projectrequirementsinthefuture.

4.15 COMPETENCE

4.15.1 Definition- Competenceis theacquisitionofknowledge,skillsandabilitiesatalevelofexpertisesufficienttobeabletoperforminanappropriateworksetting.

4.15.2 DPvesselowners/operatorsshouldoperateastructuredcompetenceassuranceprogramthat isapplied toall keyDPpersonnel withspecial focus on ensuringvessel and taskspecificcompetence.

4.15.3 Vessel specific competency should, as a minimum, be demonstrated in the followingareas:

•  Operationalmodes

•  DPFMEAfamiliarization

•  DPOperationsManualfamiliarization

•  Project/activityrequirements

4.16 MANNING

4.16.1 Vesselsshouldcomplywiththestatutoryrequirementsforsafemanning.Additionally,forDPoperations,manningshouldbeinaccordancewiththefollowing:

Criticalactivitiesarethoseactivitieswhere theconsequencesofequipment failureorlossofpositionaregreaterthanundernormaloperatingcircumstances. Forexample,

criticalactivitiesonaDPdivesupportvesselwouldincludethoseoccasionswheretheTimetoTerminateislong,suchaswhenthediverisinsideaweldinghabitatorwherethediver’sworksiteisinsidetheconductortubesataproductionfacility.

The Master should not be considered as one of the required unlimited DPOs formeetingthemanningrequirements.

It is recognized that in practice, given the near term market conditions owners/operatorswillbechallengedtomeettherecommendedlevelofexperienceforDPOs.Owners/operatorsshouldrecognizetheassociatedriskfrominexperiencedpersonneland have plans in place to address them while striving to reach the recommendedexperiencelevels.

4.16.2 Engineers - There should be sufficient licensed engineers on board for all expectedoperations.

4.16.3 Atleastonelicensedengineershouldbeavailableatalltimes,shouldbeonwatchduringcriticalactivitiesandshouldhaveatleast6monthsexperienceonsimilarequipmentandoperations.

4.16.4 Theengineer should be fullycognisant of DP operations, familiar with thevessel’s DPFMEAdocumentandtheeffectsoffailuresofequipmentrelatingtothepositionkeepingofthevessel.

4.16.5 InDP 2or3operations, theengineershouldbe familiarwith thegeneral philosophyofredundancyasitrelatestosplitmechanical,electricalandancillarysystems.

4.16.6 Electrician/ Electrical Engineer - If required on board, an electrician should haveappropriatehighvoltagetraining/certification, ifapplicabletothevessel. Aswithvesselengineers,theelectrician/electricalengineershouldhaveatleast6monthsexperienceonsimilarequipmentandoperations.

4.16.7 The electrician should likewise be fully cognisant of DP operations, familiar with thevessel’s DP FMEA document and the effects of failures of equipment relating to thepositionkeepingofthevessel.

Notes Where the minimum experience requirements cannot be met a risk basedapproach should be taken to determine the suitability of personnel and anyadditionalsupportrequirementsforintendedoperations.

4.16.8 For low risk operations guidance is provided in IMCA M182. If used, the guidanceprovidedwithinthisdocumentshouldbevalidatedagainsttherisks.
